IT IS MUTUALLY AGREED THEREFORE: That the Original Contract is hereby amended as follows:
1. CONTRACT TERMINATION
The COUNTY and CONTRACTOR mutually agree to terminate the CONTRACT effective as
of June 23. 2026.
2. SCOPE OF SERVICE
• Coordinated Entry: Upon termination of the contract, The Youth Connection is no
longer designated as the Coordinated Entry Lead Agency and shall not exercise any
authority, responsibility, or functions associated with that designation under this
contract.
• Emergency Shelter: The Youth Connection deactivated the CHGEmergencyShelter
project effective June 1, 2026. The inventory is transferred as outlined in the
Homeless Management Information Systems (HMIS) ticket request as follows, and
shall not be reimbursed for any expenses beyond June l .

ALL OTHER TERMS AND CONDITIONS of the original Contract and any subsequent
amendments hereto remain in full force and effect.
