CONDITIONS
* A Hydraulic Project Approval from the Washington State Department of Fish and Wildlife must be granted prior to
commencing work.wdfw.wa.gov
* Dock facilities must be maintained in a safe and sound condition.
* Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent upland
sediments from entering the aquatic environment. Erosion control measures must be in place prior to any clearing, grading,
or construction. These control measures must be effective to prevent soil from being carried into surface water by
stormwater runoff. Any discharge of sediment-laden runoff or other pollutants to waters of the state is in violation of state
regulations. Any work in or adjacent to waterways that will adversely affect water quality must receive specific prior
authorization from the Department of Ecology. Silt fencing, straw matting, etc. must be installed and maintained until seeding
or upland vegetation has become established around all areas disturbed or newly created by construction activities.

* A Department of the Army permit shall be obtained prior to the performance of any activities waterward of the mean higher
high water mark or ordinary high water mark or in wetlands.
* Archaeological or historical resources, such as ruins, sites, buildings, artifacts, fossils, or other objects of antiquity that
may have significance from an historical or scientific standpoint, which may be encountered by the developer/contractor,
shall not be further disturbed. If the developer/contractor encounters any such artifacts, he/she shall immediately stop work
and notify Mason County and the Washington State Department of Archaeology and Historic Preservation. Failure to do so
may result in civil penalties.
* Approved per dimensions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the
structure.
* All construction debris must be removed from the site after project completion to an approved location. Proper disposal of
construction debris must be on land in such a manner that debris cannot enter or cause water quality degradation of aquatic
environments.
* Landfill for residential development which results in the creation of new dry land waterward of OHWM or in biological
wetlands is prohibited.
* Water quality is not to be degraded to the detriment of the aquatic environment as a result of this project including but not
limited to concrete leachate containment.
* Each shoreline has a setback requirement for structures from the OHWM. Uncovered porches, decks, steps may project
into the required setback area, provided such structures combined are no greater than 200 square feet in area and project
no further than 15 feet into the setback.
* All other necessary permits from Mason County, Washington State, Federal Agencies, and/or other agencies/groups that
are required for this proposed development and construction must be obtained PRIOR TO DEVELOPMENT AND
CONSTRUCTION.
* Project must adhere to HMP/CLMP
* Performance monitoring of mitigation areas shall be conducted for a period of five years by the property owner or a
qualified professional at the cost of the property owner.
* Monitoring reports shall be submitted during Spring or Fall, no later than October 1st of each year, following the installation
of mitigation plantings. The first report shall be for"Year 0"and must document the baseline conditions of the installed
mitigation.This report shall be submitted to the County within 2 months following installation. Each subsequent monitoring
report will be evaluated from the conditions documented in the Year 0 monitoring report.
* Permanent photo points shall be established at each vegetation sampling plot and marked with either a central stake or
other permanent perimeter markers.A map showing these locations shall be submitted with the Year 0 Monitoring Report.
Representative photos in the four cardinal directions (N, E, S, W)shall be taken at the center of each sampling plot for each
year.
* Monitoring reports must include permit number, monitoring year, and the report author as identifying information.
Monitoring reports shall include photos taken in the four cardinal directions (N, E, S, and W) at each permanent photo point,
a copy of the mitigation planting site plan, and plant list from the project Habitat Management Plan. Document size shall be
8.5"x 11" with no more than 4 photos per page. Reports shall be submitted by email to the appropriate planner or Permit
Assistance Center.
* If the planting cannot be completed prior to construction, planting should occur the first planting window(October through
March)after construction activities are complete.Within two months of installation, an as-built form with photographs of the
planting area shall be submitted to the Department or on site inspection shall be required. All plants shall be clearly marked
or flagged.
* If mitigation standards have not been met at the end of the monitoring period, monitoring period shall be extended for
another 2 years. Another 2-year extension shall be required if standards are not met. A new mitigation plan shall be required
if standards have not been met after the two extensions.

Survival rate shall be 100 percent in Year 1,95% in Year 2, and 90% in Years 3-5. Direct counts shall be performed for
each year and can include natural recruits/volunteer native species. Any mortality shall be replaced by the end of the
growing season (late September-November).
* Prior to inspections the shoreline setback, and entire building envelope (including roof eaves, landings, and decks) must
be identified and staked.
Failure to do so will result in a failed inspection and require re-inspection at a later date.

project concerning beach access.
Currently, foundation excavations are complete at the top of bank for the proposed platform for beach
access whereas the concrete bases are founded on cemented hardpan.
Envirotech certifies that the subgrade is suitable for ensuing foundation construction,and that the intent of
our recommendations have been achieved.

* REQUIREMENTS FOR ROOF COVERINGS. Roof coverings shall be applied in accordance with the applicable
provisions of the current code and the manufacturer's installation instructions.
A drip edge shall be provided at eaves and gables of shingle roofs.
* The foundation/footing must be placed on undisturbed, firm-native soil. Proper frost depth, minimum 12 inches, shall be
observed below grade in undisturbed soils.
* CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING
DEPARTMENT AND THE ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All
construction must be in conformance with the international codes as amended and adopted by Mason County. Any
corrections, changes or alterations required by a Mason County Building Inspector shall be made prior to requesting
additional inspections.
* OWNER/BUILDER acknowledges submission of inaccurate information may result in a stop work order or permit
revocation.Acknowledgement of such is by signature below. I declare that I am the owner, owners legal representative, or
contractor. I further declare that I am entitled to receive this permit and to do the work as proposed. I have obtained
permission from all the necessary parties, including any easement holder or parties of interest regarding this project. The
owner or authorized agent represents that the information provided is accurate and grants employees of Mason County
access to the above described property and structure(s)for review and inspection. This permit/application becomes null &
void if work or authorized construction is not commenced within 180 days or if construction work is suspended for a period of
180 days. PROOF OF CONTINUATION OF WORK IS BY MEANS OF INSPECTION. INACTIVITY OF THIS PERMIT
APPLICATION OF 180 DAYS WILL INVALIDATE THE APPLICATION UNLESS OTHERWISE APPROVED.
* All construction must meet or exceed all local and state ordinances in addition to the International Codes requirements as
adopted and amended by Mason County and the State of Washington. Occupancy is limited to the approved and permitted
classification. Any non-approved change of use or occupancy would result in permit revocation.
* Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and
MUST NOT adversely impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either
private ditches and drains will meet requirements of the stormwater ordinance or prior approval will be granted to use an
existing utility and drainage easement dedicated for that specific purpose. For further information regarding this ordinance
contact the Mason County Public Works Department prior to construction at Ext 450

* All RED stamped approved plans are required to be on-site for inspection purposes. If an inspection is called for and
plans are not available on site,then approval will not be granted. In addition, a re-inspection fee (refer to current fee
schedule, minimum 1 hour)will be charged and must be collected by the Building Department prior to any further inspections
being performed or approvals granted.
* All building permits shall have a final inspection performed and approved by Mason County Building Department prior to
permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property
records on file with Mason County as being non-compliant with Mason County ordinances and building regulations.
* A sign is required to be posted in permanent manner in plain sight, visible at 2 feet away, weather resistant and durable
material, sign must read: Caution: Stairs must be up when not in use, damage may occur if left down due to water, wind and
floating debris.
* All changes to"approved" building plans that effect compliance with the international codes as amended and adopted, or
any other Mason County ordinance or regulation, must be reviewed and approved by Mason County prior to construction.

* Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to
the Mason County Building Department prior to construction. All engineering and/or architectural documents are a part of
the approved set of plans and shall remain attached thereto. If documents are removed, approval will not be granted. In
addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall be collected by the
Building Department prior to any further inspections being performed or approvals granted.
* Concrete used for basement walls, foundation walls, exterior walls, porches, carport slabs, steps exposed to the weather,
garage floor slabs and other vertical concrete work exposed to the weather shall have a minimum compressive strength of
3000 psi (IRC Table R402.2).
* Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries,
Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an
unregistered contractor. Further information can be obtained at 1-800-647-0982. The person signing this condition is either
the homeowner, agent for the owner or a registered contractor according to WA state law.
* An inspection performed by a representative of the geologist,their authorized representative, or engineer of record, shall
be completed on-site prior to the footing pour to verify that earthwork and foundation installation activities comply with the
Geotechnical report or assessment recommendations.
In addition a final inspection report, prepared by a representative of the geologist, their authorized representative, or
engineer of record shall be required to verify that erosion control, drainage and final work is completed in accordance to the
Geotechnical report or assessment recommendations.
Reports shall be submitted to the Mason County Building Dept., 615 W Alder St, Shelton,WA 98584, prior to the footing and
final inspections and available for inspection.
* Prior to inspections the shoreline setback, and entire building envelope (including roof eaves, landings, and decks) must
be identified and staked.
Failure to do so will result in a failed inspection and require re-inspection at a later date.
* Mitigation and monitoring conditions of SHX2024-00025 shall be reviewed in detail by the property owner and other
relevant parties prior to development.
