1, The Grantee has applied for a grant in the amount of $5,000 towards the purchase and
installation of portraits, historical prints, history plaques, and mounting fixtures, as well
as the creation of promotional materials.
2. The County is granting up to the $4,000 requested for this project on a reimbursement
basis,. based on receipts. All persons performing the work associated with this grant shall
act as an independent eontraotor(s), and not as an employee(s), on the terms and
conditions set forth herein, The money for this grant comes from fees collected by the
Mason County Auditor as authorized by RCW 36.22.170, which established a dedicated
fund for supporting local historic preservation.
In consideration of the foregoing and of the mutual promises set forth herein, and intending
to be legally bound, the parties herein agree as follows:

(a) The County hereby agrees to reimburse the Grantee up to the full 'amount
of the grant based on receipts and a final report documenting the completion
of purchasing and installing portraits, historical prints, history plaques, and
mounting fixtures, as well as the creation of promotional materials, as
described in the scope -of -work (Exhibit "13"), which shall be performed by
independent contractor(s).
(b) The Grantee hereby accepts the engagement to have said work performed,
to present receipts and a final report for roimbursement and agrees to an
inspection of the final product prior to receiving payment under the terms and
conditions set forth herein,
(a) This Contract will commence on the date written above, and unless
modified by the mutual written agreement of the parties, shall continue until
six months from that date.
March 28, 2025, to September 28, 2025
The Grantee may request a one-time six-month extension. Extension requests
must be in writing and be submitted to the Mason County Historic

Preservation Commission for approval no later than 5:00 p.m. four months
from the date of the award (July 28, 2025).
3. Grantee's Obligations
(a)
The Grantee shall provide a scope -of -work, consistent with the grant
application, which shall be attached to this contract as Exhibit B.
(b) Upon completion of the project the Grantee shall provide the County
with receipts or other proof of payment (ie. bank statements) for
reimbursement for the work approved by this grant award. At a minimum
the receipts or other proof of payment shall include the organization's
name, billing address, date of invoice (if applicable), date of payment,
and details of expenses. Invoices without proof of payment or the
aforementioned details will not be eligible for reimbursement. Grantees
will allow at least 60-days for reimbursement after the invoice has been
received by the County.

Upon completion of the granted project, a final report shall .be submitted
by the Grantee to the Mason County Historic Preservation Commission.
The final report shall summarize the project, its completion and
outcomes. The final report shall also include before and after
photographs of the completed project (if applicable). The final report
shall be submitted to the Mason County Historic Preservation
Commission, 615 West Alder St, Shelton, WA 98584 within 10 calendar
days of the authorized grant completion date (October 8, 2025).
(d) Grant recipients are required to comply with all local, state, and federal
permit requirements, ordinances and laws. Non-compliance can result in
the revocation of approved funding.

Mason General Hospital Foundation's Arts Commission Chapter / Mason
Health Foundation $4,000.00
The Mason General Hospital Foundation's Arts Commission Chapter /
Mason Health Foundation will use the funds to purchase and install
portraits, historical prints, history plaques, and mounting fixtures for a
mixed media art display at the main entrance to Mason Health's Mason
Clinic, as well as create brochures and flyers in both English and
Spanish. They will partner with local artists, installers, and tribal
representatives to showcase artwork that connects our history through
stories, myths, and legends. Completion of this project will promote
health and healing through integrating art and history in a public display
for Mason Heath's patients, visitors, and community members to view.
