The site plan does not include all septic components. The sewage permit SWG2016-00249 was never given final
approval. Please have the septic designer provide the as-built of the installed system, installation fee will be required with
as-built submittal. Once the as-built is approved, please add the missing tank(s)to a revised site plan for submittal. This
system also requires annual maintenance (regular maintenance is required by state and local code for all septic
systems); please have your system inspected by a certified septic Operations and Maintenance Specialist, any
