IT IS MUTUALLY AGREED THEREFORE: That the Original Contract is hereby amended as follows:
1. PURPOSE: Provide additional funding to support additional rent for the CHG Eviction Prevention
Program to serve at least 15 households.
2. FUND SOURCE: Consolidated Homeless Grant
3. AMENDMENT TERM: April 1, 2025 through June 30, 2025
4. TOTAL ADDITIONAL AWARD: $70,000

5. SCOPE OF WORK:
• Households must be connected to the Dispute Resolution Center before rent is paid to attempt
conciliation and reconciliation to prevent evictions.
• Crossroads Housing will enhance relationships with the courts to reduce the number of unlawful
detainer cases.
ALL OTHER TERMS AND CONDITIONS of the original Contract and any subsequent amendments hereto
remain in full force and effect.
