Box 186 Shelton, Washington 98584 Subject: Collins Lake Community Club, Water System Improvement Project,Reservoir and Pumphouse Dear Mr. Waters: Please find the following inspection reports and testing results related to the construction of the reservoir and pumphouse for well# 3 for the Collins Lake Community Club. This information is provided at your request. Included are soil test results, reinforcing steel inspection, concrete test results and weld inspection including x-ray examination. I personally inspected the reinforcing steel when it was tied in place prior to the concrete pour. The materials and the installation were satisfactory and consistent with the plans, specifications and the approved shop drawings. The enclosed report presents the results of this inspection on the three areas of the tank examined. After minor rework, all of the welds in all three areas were determined to be satisfactory. The subgrade soils exposed in the tank footing excavation will support the designed 4500 PSF loading. Also, the rebar was in-place and consisted of 418 bars on two foot centers each way. Date: Tuesday, October 24, 1995 Subject: Collins Lake Water System, GTL Project No.: B8501 On 10/19/95 1 visited the site at 9:00 AM to observe subgrade conditions at the proposed water tank pad, found gravelly silty sand which, with proper preparation will support the design loadings of 4500 PSF. Also obtained a proctor sample to be run in lab for use during backfill testing at the site. Backfill testing is to be on an as-requested basis
