This includes replacing approx. 15, 650 linear feet of watermain and installing nine new fire hydrants. A new 60, 000 gallon stand pipe reservoir will be constructed to achieve the system capacity and fire flow requirements. A new 60 gallon per minute well and pump house to be constructed. The improved system will serve 270 residential connections and provide 500 GPM for 30 minutes while maintaing a minimum of 20 PSI of pressure. Construction improvements to an existing water system All improvements to take place within either roadway (where the water main upgrade will occur) or within existing easements. A new 60,000 gallon water storage tank will be constructed within 50 feet of Pxisting pump house and rectangular 1 concrete reservoir. Additional improvements involve replacing water main piping and related appurtenances. 12. What is the location of the proposal? The south half section of Section 31, Township 23 North, Range 2 West, W.M. in Mason County, WA Collins Lake is shown on the Lake Wooten Quadrangle (USGS 7.5 Minute series topographic), and the property is directly contiguous to the Tahuya State Forest Lands. See Appendix of Water Complan for list of maps. B. ENVIRONMENTAL ELEMENT: 1. EARTH: a. General description of the site: Flat, rolling,hilly, steep slopes, mountainous, other. b. What is the steepest slope on the site(approximate slope)? 10%(Existing NE Collins Lake Drive) C. What general types of soils are found on the site? Alderwood, gravely, sandy loam (Soils Survey Series 1951 No. 9; SCS Soil Survey - Mason County, Washington) d. Are there surface indications or history of unstable soils in the immediate vicinity? SEPA. Dear Mr. Bober, We apologize for the initial confusion surrounding the SEPA checklist for the water system upgrade. The Department had received the checklist alone with no accompanying documents such as a building permit or site plan. At.your request we may process the checklist and issue a determination based on the listed alternatives as proposed in the plan. Also be advised that a Temporary Erosion and Sediment Control Plan must be submitted and approved prior to construction. Concurrency with the Mason County Interim Resource Ordinance must be achieved. Additional permitting and/or design modifications may be required. These issues will fleshed out during site inspections conducted by staff in the upcoming weeks. We will keep you posted as needed. Box 578 Shelton,WA 98584 Art: Gary Yando, Director of Community Development Re: Collins Lake Community Club Water System FmHA Preliminary Engineer's Report& SEPA Checklist Dear Gary: I am the Engineer for the Collins Lake Community Club Water Plan. August 23, 1994 I mailed a copy of the FmHA Preliminary Engineer's Report to your office per the advise from Pete Weber, P.E. - FmHA State Engineer. My understanding in doing so was that Mason County Department of Community Development is the clearinghouse for FmHA projects in this section of the state. The Draft Collins Lake Community Club Water Comprehensive Plan in presently being reviewed by the DOH(Lisa Raysby, RE.). On August 23, 1994 I prepared and mailed the SEPA Checklist to the client for their review and remittance of review fee. Per conversation with Barry Sparks (Collins Lake Community Club-President), the required $100 fee for review of the project was attached with the checklist. If our submittal is too premature, please advise us. The design phase of the project will require that all pertinent issues be addressed regarding this project, which shall be clearly stated on the contract drawings and specifications. It is my understanding that Farmers Home Administration has completed their environmental assessment for the project, and if requested we will obtain a copy for your department's review. We have not had any response regarding this submittal from Mason County. Please advise us as follows by checking the appropriate response: 1. The SEPA checklist has been processed and no adverse impacts were determined. 2. The SEPA checklist has been processed and there are adverse environmental impacts associated with its construction. 3. _ The SEPA checklist has not be completed or has been improperly processed. Should there be any further questions or clarifications for this small private water system, please feel free to contact me at your earliest convenience. I have attached a self-addressed envelope for your convenience should your time be limited, and would appreciate a response by no later than November 7, 1994. Shelton,WA 98584 Att: Jason Manassee,Planner Re: Collins Lake Community Club Water System SEPA New Water Tank Location and Well Dear Jason: Barry Sparks, President of Collins Lake Community Club contacted me recently regarding their water project. The Association wishes to amend their plan by including the construction of a new storage tank on Lot 61 of Division 3 and development of a new well and wellhouse on Lot 65 of Division 3. Agency progress to date is as follows: • State DOH is presently reviewing CLCC Water Complan. • WA State Department of Fish and Wildlife-HPA application is presently being reviewed. (Jyll Dunlap,206-902-2576) Upon completion of design, we anticipate the wellhouse facility to require permitting. The structure to house the well, booster pump(s),and related appurtenances will probably be greater than 120 square feet in floor area. The design for the water storage tank will probably be one with a cylindrical geometry;approximately thirty(30)feet in diameter and fifteen(15)feet in height. We anticipate that Best Management Practices (BMP) will be employed for stock piling of trench spoils during water main replacement in the existing roadways, construction of the new water tank, and well facilities.
