1) By definition, propane tanks and heatpumps are structures, which must meet setback conditions. Please check your"Approved Site Plan"to ensure
these structures meet the setback conditions listed.

2) Owner/@ Agent is responsible to post the assigned address and/or purchase and post private road signs in accordance with Mason County Title 14.28.

3) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the
State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in
per revocation.

4) All changes to"approved" building plans that effect compliance with the international codes as amended and adopted, or any other Mason County
ordi nce or regulation, must be reviewed and approved by Mason County prior to construction.

5) All construction and demolition debris must be removed from the beach after project completion. roper disposal of construction debris must be on land
in such a manner that debris cannot enter or cause water quality degredation of State waters. X -.-
6) A Road Access Permit or Approval must be granted by the Mason County Department of Public Works. For more information contact Public Works, at
(360)427-9670, ext. 450. The building permit will not be"finaled" until the permit holder can show proof that the access permit from Public Works has
been Vnaled"and approved.

7) The demolition and disposal of debris must meet the regulations of Mason County and Olympic Region Clean Air Agency (ORCAA).
It is unlawful for any person to cause or allow the demolition (or major renovation) of any structure unless all asbestos containing materials have been
identified and removed from the area to be demolished. Work shall not commence on an asbestos project or demolition project unless the owner or

8) Proper erosion and sediment control practices must be used on the construction site and adjacent areas to prevent upland sediments from entering
shoreline waters. Erosion control measures must be in place prior to any clearing, grading, or construction. These control measures must be effective to
prevent soil from being carried into surface water by stormwater runoff. Sand, silt, and soil will damage aquatic habitat and are considered pollutants.
Any discharge of sediment-laden runoff or other pollutapollutants to waters of the state is in violation of Chapter 90.48 RCW, Water Pollution Control, and
WAC 173-201A, Water Quality Standards for Surface Waters of the State of Washington, and is subject to enforcement action. Any work in or adjacent
to waterways that will adversely affect water quality must receive specific prior authorization from the Department of Ecology pursuant to WAC
173-201A-110. A short-term water quality standards modification maybe issued if the proponent agrees to a number of specific construction practices
and techniques designed to minimize water quality impacts. All areas disturbed or newly created by construction activities must be revegetated using
bioenai,ineering techniques, clean durable riprap, or some other equivalent type of protection against erosion when other measures are not practical.

9) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building
Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached
thereto. If documents are removed, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be
charg d and shall be collected by the Building Department prior to any further inspections being performed or approvals granted.

10) Prior to final approval, all upland areas disturbed or n ly created by construction activities shall be seeded, vegetated or given an equivalent type of
erosion protection (silt fencing or straw matting). X
11) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED
BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance
with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building
Inspe.qtor shall be made prior to requesting additional inspections.

12) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Maso ounty ordinances and building regulations.

13) The international code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150' from an approved
access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where
such ro s connect with a county maintained public road or to another fire apparatus access road which connects to a county maintained public road.

14) The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical report. All applicable
recommendations and specifications shall be applied to the development on this site. Any deviation requires stamped written approval from the registered
design professional responsible for the report/assessment, and may require special inspection by same. Structures and/or land modifications (grading,
cuts, fills, etc.) required in the geotechnical report/assessment, may require a separate permit. The geotechnical report shall remain attached to the
apprSved building plans.

15) Landings and stairs must meet the same setback conditions as any permitted structure; and, must be shown on your site plan. Please check your
"Ap oved Site Plan"to ensure these structures are shown and meet the setback conditions listed.
16) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact
adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the
stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further
information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access
connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction which
is proposed to be located within 25'of a Mason County road right of way, it is suggested to contact that office to review future planned work which may
affect your proj

17) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit
holde ave prevented action from being taken. No more than one extension may be granted.

18) The plan review check list and corrections are part of the approved plans and must remain thereto. It is the responsibility of the applicant to make the
corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building
Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or
remov f approved documents will result in failure of required building inspections.

19) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will
not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building
Depa,`41nent prior to any further inspections being performed or approvals granted.

20) All propane tanks must be installed in accordance with the International Fire Code and all applicable Mason County ordinances. All propane tanks must
meet Oie,installation requirements and minimum setbacks as listed in the Mason County Fire Marshal's Standards for the Installation of Propane Tanks.

22) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division.
There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at
1-800- 7-09B2. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law.

23) Retaining walls needed to support a surc rge such as structures, roads, or to support slopes, shall require a separate building permit and approval prior

24) REQUIREMENTS FOR ROOF COVERINGS. Roof coverings shall be applied in accordance with the applicable provisions of the current code and the
manufacturer's installation instructions.

25) WIND LOADS - Roof coverings shall be designed and tested to withstand the maximum basic wind speed. The basic wind speed for Mason County is 85
MPH.

26) The proposed project must be consistent with all applicable policies and other provisions of the Shoreline Management Act, its rules, and the Mason
Coun horeline Master Program.

27) App ed per dimensions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the structure.

28) The"approved" site plan is required to be on-site for inspection purposes. If an inspection is requested and the"approved" site plan is not on site, then
approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall be collected by the
Builds Department prior to any further inspections being performed or approvals granted.
29) An inspection performed by a representative of the geologist, their authorized representative, or engineer of record, shall be completed on-site prior to the
footing pour to verify that earthwork and foundation installation activities comply with the Geotechnical report or assessment recommendations.
In addition a final inspection report, prepared by a representative of the geologist, their authorized representative, or engineer of record shall be required
to verify that erosion control, drainage and final work is completed in accordance to the Geotechnical report or assessment recommendations.
Reports shall be submitted to the Mason County Building Dept., PO Box 186, Shelton, WA 98584, prior to the footing and final inspections and available
for ins ction.

30) In addition to the inspections required in IBC, Section 109, the owner or the engineer of record acting as the owner's agent shall employ one or more
special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by the design
professional. The special inspectors duties& responsibilities shall be as specified in Chapter 17.
Special inspections and reports shall include as listed on sheet 1 in the constructions notes as follows:
Item#4: Periodic inspections performed and inspection reports prepared by the engineer of record shall be provided to the Mason County Building
Department prior to footing, framing and final occupancy inspections.
Item#5: Soil compaction reports.
Item#7: Concrete compressive strength minimum 3500 psi.
Special inspection reports shall be submitted to the Mason County Building Department, PO Box 186, Shelton WA 98584 (e-mail to
DLC@co.mason.wa.us) and available on-site for inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and
shallt;e submitted prior to the framing and final occupancy inspections.

34) Epoxy grout required and specified on the approved building plans shall require a special inspection. The special inspector shall be the engineer of record
or their authorized representative, a WABO Certified special inspector, or certified testing laboratory. In addition a Mason County Building Inspector may
perform the inspection provided holes are prepared in accordance to manufacturer specifications that are available on-site during inspection.
Special inspectors shall inspect the installation of grouted construction anchors as stated in the manufacturers specifications. An inspection report shall
be ppared and submitted to the Mason County Building Department prior to the framing inspection of the project.

This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at anytime after work is
commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of
work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate and grants employees of Mason County access to

Per Mason County Code,Title 14, Chapter 14.48 a stormwater site plan is required whenever a building application is
made for residential development, or redevelopment', with more than 2,000 square feet of impervious surface 2.
'Redevelopment means,on an already developed site,the creation or addition of impervious surfaces,structural development
including construction, installation or expansion of a building or other structure,and/or replacement of impervious surface that is not
part of a routine maintenance activity,and land disturbing activities associated with structural or impervious redevelopment.
ZCommon impervious surfaces include,but are not limited to,rooftops,walkways,patios,driveways,parking lots or storage areas,
concrete or asphalt paving,gravel roads,packed earthen materials,and oiled,macadam or other surfaces which similarly impede the

Regulated activities shall be conducted only after Mason County Public Works approves a stormwater site plan
(Mason County Code Title 14 Chapter 14.48 section 14.48.70). You will receive a copy of the Public Works document
entitled "Managing Storm Drainage on Small Lots,The Small Parcel Stormwater Site Plan".This document will assist
you in preparing the necessary information and plans for Public Works to review and approve. Per Department of
Public Works this document will constitute an approved plan if all of the relevant details* are to be installed in
their entirety AND no part of the stormwater system adversely affects any septic system (see Environmental Health
information below). If an alternative system is to be used a plan will need to be submitted to Public Works for approval.
A design by a registered professional may be required for more complex sites.
*These details are found in the document Managing Storm Drainage on Small Lots, The Small Parcel Stormwater Site Plan
on the pages that begin with"Handout"

While conducting a site inspection of the building permit for the proposed Ninneman home, slopes
in excess of 25 percent were observed at the rear shore areas of the property. Staff noted site
preparation work was done that has caused impacts to the critical areas present. Past excavations
and wall building not subject to prior review have taken place as the new residence and garage are
proposed.
No further work should take place and the submitted building permit review cannot proceed until, in
accordance with Mason County Resource Ordinance Sec. 17.01.100 Landslide Hazard Areas.
Standards include:
Slopes greater than 15 percent adjacent to or within 300 feet of a proposed project were observed
to the south and north. The applicant shall have a qualified geologist or engineering geologist visit
the subject parcel and prepare a site analysis to determine whether a geologic special report is
needed during development review. Your structural engineer can assist in providing this needed
information.
If the geologist confirms the need for a special report when slopes are greater than 40 percent, a
Geotechnical Report would be needed to address slope stability, site conditions, and the other
requirements of the enclosed landslide hazard chapter 17.01.100 section E. 5. as required by the
Mason County Resource Ordinance. The report must include all Resource Ordinance
requirements and a submittal checklist noting where these requirements are found in the study
document.
The prepared special report shall be specific to proposed site development on the subject parcel
by addressing clearing, grading, driveway and building location, retaining and stabilization walls,
and stormwater control. This study shall address the presence of the septic system on site stability
conditions. This special report and submittal checklist would be provided to the County and
evaluated in this building permit review.
I have provided a list of qualified geologists or engineering geologists who have prepared geologic
special studies in the past.
In addition, the new residence proposed in your site plan is 10 feet wider (at 32 feet) than the
existing cabin (at 22 feet). This becomes a development issue as the existing cabin on Lot 115 is
much closer to the Haven Lake shoreline than the cabins on either Lot 116 to the west and Lot 114
to the east. Proposed expansions of your residence will need to be behind the sight line that
connects the corner of the roof eaves of your existing cabin to the corner of the roof eaves of the
cabin to the west and to the cabin to the east. Existing uncovered decks forward of this sight line
cannot become enclosed living areas as shown in the northwest and northeast corners of the
proposed new residence (closer to the lake). This issue often comes up when replacing old
structures with new and expanded structures, and the issue would have been discussed previously
if you had request a site pre-inspection by a shoreline planner early in the process to prepare your
building replacement proposal.
