There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at 1-800-647-0982. The person g g this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law. X :. 2) The international code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150' from an approved access road. Roads are requi ed to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where such roads connect with c my maintained public road or to another fire apparatus access road which connects to a county maintained public road. X 3) All other necessary permits from Mason County, Washington State and/or Federal Agencies that are required t proposed development and construction must be obtained PRIOR TO SAME DEVELOPMENT AND CONSTRUCTION. X 4) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and 4aare not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building Department prior to any fu he * spections being performed or approvals granted. X 5) Owner/Agent is responsible t the assigned address and/or purchase and post private road signs in accordance with Mason County Title 14.28. X 6) The plan review check list and corrections are part of the approved plans and must remain thereto. It is the responsibility of the applicant to make the corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or removal of approved documents sult in failure of required building inspections. X /W 7) The"approved" site plan is required to be on-site for inspection purposes. If an inspection is requested and the"approved" site plan is not on site, then approval will not be granted. X 8) This structure is limited to U-occupancy use my(private garages, carports, sheds, and agricultural buildings.) Any other use will be in violation of the international codes and Mason County lations unless a"Change of Use" permit is applied for, reviewed and approved. X BLD2007-01883 Please referto the following pages for conditions of this permit. 2 of 5 9) Any changes in proposed construction shall be reviewed by the engineer or architect of record and submitted in writing to the Mason County Building Department prior to construction. All engineering and/or architectural documents are a part of the approved set of plans and shall remain attached thereto. If documents are removed, appy.9val will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and shall be collected by ding Department prior to any further inspections being performed or approvals granted. X 10) International Building Code. 4ddition to the inspections required by Chapter 1 of the International Building Code, the owner or the engineer or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of work listed under IBC, section 1704. The special inspectors' duties & responsibilities shall be as specified in Chapter 17. Required if pile height is over 10 feet. Engineer to verify piles are driven to of refusal. X 11) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the State of Washington. Occupancy limit to the approved and permitted classification. Any non-approved change of use or occupancy would result in permit revocation. X 12) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access connecting from a Mason County Road, Con ct the Mason County Public Works Department prior to construction at Ext 450. For any construction which is proposed to be located within 25'of M n County road right of way, it is suggested to contact that office to review future planned work which may affect your project. X 13) All changes to"approved" buildin plans that effect compliance with the international codes as amended and adopted, or any other Mason County ordinance or regulation, must b ed and approved by Mason County prior to construction. X 14) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as mended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building Inspector shall be made pri uesting additional inspections. X 15) All property lines shall be clearly identified at the time of foundation inspection. X 16) All building permits shall have a final i pection performed and approved by the Mason /ounty Building Department prior to permit expiration. The failure to request a final inspection or to approval will be documented in the legal property records on file with Mason County as being non-compliant with Mason County ordinances and g regulations. The Building Official may extend the time for action for a period not ex Fed' g 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit holder haveprevented i i om being taken. No more than one extension may be ranted. 9 Y 9 X 18) Pressure treated w d manufac re after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, connectors, and flashing. Inst al connectors approved for contact with the new types of pressure treated material. X 19) Debris or deleterious material resulting from the construction sha moved from the beach area and project site and shall not be allowed to enter waters of the state. (Use Reg#22, Res Ch. of SMP). X 20) Water quality is not to e d raded to the detriment of the aquatic environment as a result of this project. (Use Reg#21 of Res Ch of SMP). X 21) Floatation for the structure shall be entirely enclosed and contained to per ently prevent the breakup or loss of the floatation material into the water. Orient the floatation so that it does not obstruct the grating. X 22) The proposed project must be consistent with all appl' I policies and of(r provisions of the Shoreline Management Act, its rules, and the Mason County Shoreline Master Program.X 23) Pressure treated wood must be completely dry prior to placement in the water. IJ aosote or pentachlorophenol is prohibited. Install plastic wear strips between the float and the pilings to reduce treated wood chafe. X 24) Any discharge of sediment-laden runo r other pollutants to waters of the s ate is in violation of Chapter 90.48, Water Pollution Control, and WAC 173-201A, Water Quality Standa Surface Waters of the State of Washington and is subject to enforcement action (per DOE SEPA comments).X 25) X 26) During construction, all release of oils, hydraulic fluids, fuels, other petroleu products, paints, solvents, and other deleterious materials must be contained and removed in a manner that will prevent their discharge sand soils of the state. The cleanup of spills should take precedence over other work on the site. (per DOE SEPA comments). X 27) All trenches, depressions or holes created in the intertidal area hall be backfilled p innundation by tidal waters. All stormwater dsicharges must be constructed in a manner that will not cause scouring on the beach. X 28) All proposed best management practices within the Biological Evaluation are to be implemented. X BLD2007-01883 Please refer to the following pages for conditions of t permit. 4 of 5 31) Deed Restriction: An Agreement regarding joint-use was submitted by applicants for the proposed joint-use structure Both joint-use owners have agreed to build no additional Pier, Ramp and Float structures on their property. Both Deeds have statements provided to the Planning Department. Statements refer to the joint-use for both parcels. All facilities must be constructed and maintained in a safe ou d condition an a done�r unsafe docks and piers be removed or repaired promptly by the owners per the Hearing Examiner Decision. X e 32) In order to minimize any direct effects on the listed species, pile driving should take place between and January 14 subject to Washington State Dept of Fish &Wildlife Hydraulic Project Approval and Bald Eagle Management Plan. X 33) The one mitigation measure offered by the applicants and their agent consists of the removal of three (3) oyster bags from the upper intertidal area on Conrad's property, the parcel to the west side of the PRF facility. Oyster bag size is 2'X 3'or 6 square feet each for a total of 18 square feet to be removed and allowed r rn to natural state. X This permit becomes null and void if work orconstruction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of work is by means of a progress inspection.The owner or the agent on the owners behalf,represents that the information provided is accurate and grants employees of Mason County access to the above described property and structure for review and inspection.
