Alder Street Purpose of checklist Governmental agencies use this checklist to help determine whether the environmental impacts of your proposal are significant. This information is also helpful to determine if available avoidance, minimization, or compensatory mitigation measures will address the probable significant impacts or if an environmental impact statement will be prepared to further analyze the proposal. Instructions for applicants This environmental checklist asks you to describe some basic information about your proposal. Please answer each question accurately and carefully, to the best of your knowledge. You may need to consult with an agency specialist or private consultant for some questions. You may use "not applicable" or "does not apply" only when you can explain why it does not apply and not when the answer is unknown. You may also attach or incorporate by reference additional studies reports. Complete and accurate answers to these questions often avoid delays with the SEPA process as well as later in the decision-making process. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects.The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. Instructions for lead agencies Please adjust the format of this template as needed. Additional information may be necessary to evaluate the existing environment, all interrelated aspects of the proposal and an analysis of adverse impacts. The checklist is considered the first but not necessarily the only source of information needed to make an adequate threshold determination. Once a threshold determination is made, the lead agency is responsible for the completeness and accuracy of the checklist and other supporting documents. Use of checklist for nonproject proposals For nonproject proposals (such as ordinances, regulations, plans and programs), complete the applicable parts of sections A and B, plus the Supplemental Sheet for Nonproiect Actions (Part D). Please completely answer all questions that apply and note that the words "project," "applicant," and "property or site" should be read as "proposal," "proponent," and "affected geographic area," respectively.The lead agency may exclude (for non-projects) questions in "Part B: Environmental Elements" that do not contribute meaningfully to the analysis of the proposal. Agency requesting checklist: Mason County, WA (Department of Community Development); Washington DOE, Fish & Wildlife; U.S. Army Corps of Engineers; other agencies or indigenous tribes with interests in the project site. 6. Proposed timing or schedule (including phasing, if applicable): Begin construction (landward of existing, failing bulkhead) in late fall 2024, or early 2025. 7. Do you have any plans for future additions, expansion, or further activity related to or connected with this proposal? If yes, explain. Plans note potential future location for davit,or retractable stairs,or ladder to the beach, near the NW corner of the rebuilt bulkhead.This would provide a form of beach access for small watercraft(kayak/canoe)and pedestrian access for owners and guests. 8. List any environmental information you know about that has been prepared, or will be prepared, directly related to this proposal. Do you know whether applications are pending for governmental approvals of other proposals directly affecting the property covered by your proposal? If yes, explain. No other approvals pending. 10. List any government approvals or permits that will be needed for your proposal, if known. There are several questions later in this checklist that ask you to describe certain aspects of your proposal. You do not need to repeat those answers on this page. (Lead agencies may modify this form to include additional specific information on project description.) Owner will replace approx.70'linear ft.of failing 48 yr old,vertical concrete bulkhead,with collapsed stairs for a single family, primary residence. Linear ft measure includes returns to upland wing-wall and south property Iine.Existing bulkhead serves a total of 4 lots, but permit review is for work that will only take place on owner's parcel. Existing bulkhead is over-topped by storm waves and King tides;owner requests a 24"wave cap(within footprint of existing bulkhead)for protection from future sea-level rise.All work to be landward of existing bulkhead, except demolition of collapsed concrete stairs and old bulkhead once replacement is in place. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. Environmental Elements 1. Earth Find help answering earth questions a. General description of the site: The site is located on a west-facing shoreline of a small inlet within Hood Canal at a location where the inlet of the Tahuya River is approximately.2 miles wide in the northwest to southeast direction.The project is near the mouth of the Tahuya River,and within the Tahuya Waterfront Tracts platted tidelands.The site receives heavy storm surge(driven across the 5 mile fetch from Potlatch),with large woody debris during winter storms and at King tides.Significant overtopping and scouring of the existing bulkhead during winter storm events has greatly accelerated the bulkhead failure. Circle or highlight one: Flat, rolling, hilly, steep slopes, mountainous, other: b. What is the steepest slope on the site (approximate percent slope)? The slope west of the residence is estimated at 60%per the Aspect Consulting geotechnical assessment and report of conditions, at page 4;such geotechnical assessment and report of conditions,dated February 7, 2024 is attached hereto and incorporated by reference. c. What general types of soils are found on the site(for example, clay, sand,gravel, peat, muck)? If you know the classification of agricultural soils, specify them,and note any agricultural land of long-term commercial significance and whether the proposal results in removing any of these soils. Site specific soils are listed at pp 7&8 of the Aspect Consulting geotechnical assessment and report of conditions. Soils are described as generally loose and unstratified, cobbles, pebbles, sand, silt, clay, boulders and diamicton. d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. The underlying geomorphology of the site is consistent with reported studies of rotational landslides attributed to the advancement and retreat of the Cordilleran Glacier icesheet at the end of the Vashon Stade of the Fraser Glaciation, approximately 13,000 years ago.See discussion at pp 7&8 of the referenced geotechnical assessment and report of conditions. e. Describe the purpose, type,total area, and approximate quantities and total affected area of any filling, excavation, and grading proposed. Indicate source of fill. Total area of project will be upland of existing 5'tall bulkhead;comprised of 56'linear feet of soldier pile wall,with an approximate 10'return landward,where the piles will interface with the existing upland wing-wall,which protects the residence at the top of the slope.The wave cap portion of the design will add 2'height.Total volume of fill behind the soldier pile wall will be(10'x 56'x T) approximately 3,920 cubic feet(145.185 cubic yards).Source of fill will be finalized by contractor and meet the following specifications:Clean gravel back-fill landward and behind the proposed soldier pileflag-block walls,geotextile fabric to control silt transport,and hydraulically applied erosion control products(HECPs)per Soldier Pile Wall Notes 1.4,1.5,and 1.6 on plan sheet C-01 through C-03. f. Could erosion occur because of clearing, construction, or use? If so,generally describe. No additional upland slope movement towards the beach is expected with the rebuilt bulkhead.Beach material is seasonally eroded and accreted to the beach at this location due to the tidal action,winter storm events and ordinary river flows within the Tahuya River estuary.Naturally occurring littoral drift of beach sediments at this location is compromised/interrupted by a boat ramp,(owned by an upland HOA,that lies south of the 4-lot bulkhead),which was raised without finalized permits per Mason County records. g. About what percent of the site will be covered with impervious surfaces after project construction (for example,asphalt or buildings)? The project will not add any additional impervious surface to the site.Owner's entire site is 56'x 1,200',or 67,200 sq.ft.minus the 60'ROW for North Shore Road;or 63,840 sq.ft.(inclusive of the 25,545 sq.'of tidelands).The total developed area of the site(West of North Shore Road)is approximately 38,295 sq.ft.as there are no improvements east of North Shore Road. The existing on-site impervious surfaces are:house(37SK40'=1,480 sq.ft.);garage(26'x28.5'=741 sq.ft.);picnic shelter(8'x12'=96 sq.ft.),asphalt paved drive from North Shore Road (151x250'=3,750 sq.ft.);total of impervious surfaces on site is approximately 6,067 sq.ft.,approximately 9.5%of the total lot area.(6067 sq.ft./63"0 sq.ft.). h. Proposed measures to reduce or control erosion,or other impacts to the earth, if any. Geotextile fabric and HECPs will be used on the upland side of the replacement soldier pile wall to prevent silt transport into the open waters of the Tahuya River channel and Hood Canal. Air Find help answering air questions a.What types of emissions to the air would result from the proposal during construction operation, and maintenance when the project is completed? If any, generally describe and give approximate quantities if known. Pile driving equipment uses diesel fuels,exhaust from which will be emitted during the construction phase of the project.These emissions are transient in nature and will not be present after the project.A tug-driven barge, gas or diesel,will be utilized to transport all construction materials and remove all concrete spoils following demolition of the failing bulkhead wall and stairs. b.Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. No. c. Proposed measures to reduce or control emissions or other impacts to air, if any. None. 3. Water Find help answering water questions a. Surface Water: Find help answering surface water questions 1. Is there any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds,wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. The site is located on Hood Canal,off of NE North Shore Road.This is on the east side of the lower Tahuya River embayment,approximately .25 miles south of Hogan's Spit/Caldervin Creek,where the river enters Hood Canal.Ordinary low water(OLW)at the project site is the eastern meander line of the river channel.Mean high water(MHW)fills the embayment from site to the Hogan's Spit/Caldervin Creek complex. 2. Will the project require any work over, in, or adjacent to (within 200 feet)the described waters? If yes, please describe and attach available plans. The project will not involve work over or in the described waters; however, the project footprint is immediately adjacent to such waters. All work is expected to take place landward of the existing vertical bulkhead. 3. Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. None. 4. Will the proposal require surface water withdrawals or diversions?Give a general description, purpose, and approximate quantities if known. No. 5. Does the proposal lie within a 100-year floodplain? If so, note location on the site plan. The project area is located in a FEMA"AE" designated area along the shoreline. Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. No. b. Ground Water: Find help answering ground water questions 1. Will groundwater be withdrawn from a well for drinking water or other purposes? If so, give a general description of the well, proposed uses and approximate quantities withdrawn from the well. Will water be discharged to groundwater? Give a general description, purpose, and approximate quantities if known. No additional volume of well water will be drawn down or is expected to be caused by this proposal. Existing shared well is used for potable residential use and irrigation of under-story plantings. 2. Describe waste material that will be discharged into the ground from septic tanks or other sources, if any (domestic sewage; industrial, containing the following chemicals...; agricultural; etc.). Describe the general size of the system, the number of such systems, the number of houses to be served (if applicable), or the number of animals or humans the system(s) are expected to serve. There will be no changes to the Mason County approved on-site septic system, which undergoes periodic inspections for regulatory compliance. c. Water Runoff(including stormwater): a) Describe the source of runoff(including storm water) and method of collection and disposal, if any (include quantities, if known). Where will this water flow?Will this water flow into other waters? If so, describe. Rain gutter downspouts on the residence, garage and picnic shelter discharge to open gravel adjacent to the structures. This water percolates rapidly. Seasonally heavy rainfall percolates directly into the septic field which is covered by pavers-on- sand. b) Could waste materials enter ground or surface waters? If so,generally describe. No. c) Does the proposal alter or otherwise affect drainage patterns in the vicinity of the site? If so, describe. Hood Canal summer chum, steelhead, and Chinook are ESA listed species found in the South Hood Canal and the Tahuya River Watershed. ESA aquatic mammals, specifically Southern Resident Orcas, are not present in Hood Canal. d. Proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on the site, if any. Wild huckleberry will be moved upland from the immediate construction area and reused for bank stability. Owner's will supplement with other native species found on site, such as salaal, Oregon grape and thimbleberry. e. List all noxious weeds and invasive species known to be on or near the site. Scotch broom, English ivy, and seepweed (on beach). 5. AnimalS Find help answering animal questions a. List any birds and other animals that have been observed on or near the site or are known to be on or near the site. List any threatened and endangered species known to be on or near the site. The biological site assessment(BSA by DCG Watershed,Seattle)calls out the Southern Resident Circa populations as being within the Puget Sound,generally.However, such species are not observed in the waters of Hood Canal,where the Bigg's orcas(T-pods)often visit.The BSA also notes other ESA listed animals,none of which have been observed along the subject property. c. Is the site part of a migration route? If so, explain. Owner's participate in the Hood Canal Dark Sky project to eliminate night time lights that interfere with migratory birds. d. Proposed measures to preserve or enhance wildlife, if any. Use of local well to keep on-site understory well watered and fruit bearing, as forage for all species found on-site or nearby. e. Energy and Natural Resources Find help answering energy and natural resource questions 1. What kinds of energy(electric, natural gas, oil,wood stove, solar) will be used to meet the completed project's energy needs? Describe whether it will be used for heating, manufacturing, etc. None. 2. Would your project affect the potential use of solar energy by adjacent properties? If so, generally describe. No. 3. What kinds of energy conservation features are included in the plans of this proposal? List other proposed measures to reduce or control energy impacts, if any. None. 7. Environmental Health Find help with answering environmental health questions a.Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spill, or hazardous waste,that could occur because of this proposal? If so, describe. No. 1. Describe any known or possible contamination at the site from present or past uses. Failing concrete bulkhead and collapsed stairs are on-site. All old concrete from the bulkhead, its footings, and the stairs will be removed from the site. Failing bulkhead was originally built in the mid-1970's of concrete and rebar. 2. Describe existing hazardous chemicals/conditions that might affect project development and design.This includes underground hazardous liquid and gas transmission pipelines located within the project area and in the vicinity. None. 3. Describe any toxic or hazardous chemicals that might be stored, used, or produced during the project's development or construction, or at any time during the operating life of the project. None. 4. Describe special emergency services that might be required. None. 5. Proposed measures to reduce or control environmental health hazards, if any. Contractor will use OSHA/WISHA approved hearing and dust protection for worker safety on this project. SEPA Environmental checklist(WAC 197-11-960) January 2023 Page 8 of 15 b. Noise 1. What types of noise exist in the area which may affect your project (for example: traffic, equipment, operation, other)? Rural traffic along North Shore Road; boats and other small watercraft during the summer months. 2. What types and levels of noise would be created by or associated with the project on a short-term or a long-term basis (for example:traffic, construction, operation, other)? Indicate what hours noise would come from the site)? Construction noise from the project will follow Mason County rules for construction noise; no noise before 7:00 am and none after 7:00 pm. During construction, piles will be drilled/driven into the beach behind the existing old bulkhead. After piles are driven and lag blocks installed, the old bulkhead and stairs within the project area will be demolished. 3. Proposed measures to reduce or control noise impacts, if any. Stay within the allowed Mason County rural construction noise limits. 8. Land and Shoreline Use Find help answering land and shoreline use questions a. What is the current use of the site and adjacent properties?Will the proposal affect current land uses on nearby or adjacent properties? If so, describe. Residences, some full-time, other part-time. The proposal will not affect current land uses on nearby or adjacent parcels. b. Has the project site been used as working farmlands or working forest lands? If so, describe. How much agricultural or forest land of long-term commercial significance will be converted to other uses because of the proposal, if any? If resource lands have not been designated, how many acres in farmland or forest land tax status will be converted to nonfarm or nonforest use? No. 1. Will the proposal affect or be affected by surrounding working farm or forest land normal business operations, such as oversize equipment access, the application of pesticides, tilling, and harvesting? The project site has county mapped geologic hazards related to slope stability; unstable; old slide, DOE Coastal Zone Atlas; and landslides (DNR 2010), Hazard Zone, very high (DNR 2007), and landforms, moderate (DNR 2009). i. Approximately how many people would reside or work in the completed project? Household members and guests; (2-4) j. Approximately how many people would the completed project displace? Zero. k. Multi-agency review of all submittals for conformance to development regulations; Net local ecological benefit expected from removal of old bulkhead, collapsed stars and failed armoring, is removal of 300-350 cuft of old concrete and rebar. m. Proposed rebuilt bulkhead, with 24" wave cap installed, would be approx. 7' above grade at beach level. b. What views in the immediate vicinity would be altered or obstructed? None. c. Proposed measures to reduce or control aesthetic impacts, if any. None. 11. Light and Glare Find help answering light and glare questions a. What type of light or glare will the proposal produce?What time of day would it mainly occur? None. b. Could light or glare from the finished project be a safety hazard or interfere with views? No. c. What existing off-site sources of light or glare may affect your proposal? None. d. Proposed measures to reduce or control light and glare impacts, if any. None. 2. Recreation Find help answering recreation questions a. What designated and informal recreational opportunities are in the immediate vicinity? Public access to walk along the shoreline is not present from the subject parcel, however, other beach entry points are within .25 to .5 miles. The adjacent tidelands are platted from U.S. Gov. lots and are considered private, as to shellfish on the beach. b. Would the proposed project displace any existing recreational uses? If so, describe. No, except during construction window. c. Proposed measures to reduce or control impacts on recreation, including recreation opportunities to be provided by the project or applicant, if any. Fall construction means no disturbance to most of the summer seasonal recreational opportunities in the area. SEPA Environmental checklist(WAC 197-11-960) January 2023 Page 11 of 15 13. Historic and Cultural Preservation Find help answering historic and cultural preservation questions a. Are there any buildings, structures, or sites, located on or near the site that are over 45 years old listed in or eligible for listing in national, state, or local preservation registers? If so, specifically describe. No. b. Are there any landmarks, features, or other evidence of Indian or historic use or occupation?This may include human burials or old cemeteries. Are there any material evidence, artifacts, or areas of cultural importance on or near the site? Please list any professional studies conducted at the site to identify such resources. No; and no artifacts or items of cultural importance have been found on, or near, the site. c. Describe the methods used to assess the potential impacts to cultural and historic resources on or near the project site. Examples include consultation with tribes and the department of archeology and historic preservation, archaeological surveys, historic maps, GIS data, etc. Owner has completed a literature review for historic or cultural resources in the general vicinity of the project.The Hood Canal Salmon Enhancement Group's study of the Tahuya River habitat only studied the area above the Tahuya River bridge(east of NE North Shore Road). Owner does not have the appropriate credentials to receive access to the Washington State Department of Archaeology 8 Historic Preservation(DAHP)WISAARD data base to conduct further analysis at this time.Owner's JARPA application may generate agency reports that may or may not be proximate to the project location. d. Proposed measures to avoid, minimize, or compensate for loss, changes to, and disturbance to resources. Please include plans for the above and any permits that may be required. During construction contractor shall be on alert if cultural or historic resources are uncovered. In such a case work shall stop and a WA state licensed archaeologist consulted for evaluation and preservation. 14. Transportation Find help with answering transportation questions a. Identify public streets and highways serving the site or affected geographic area and describe proposed access to the existing street system. Show on site plans, if any. Access to the site by land vehicle is on NE North Shore Road, onto a paved driveway. b. Is the site or affected geographic area currently served by public transit? If so,generally describe. If not, what is the approximate distance to the nearest transit stop? Yes. Mason Transit does serve this area of North Shore Road. c. Will the proposal require any new or improvements to existing roads, streets, pedestrian, bicycle, or state transportation facilities, not including driveways? If so, generally describe (indicate whether public or private). No. d. Will the project or proposal use(or occur in the immediate vicinity of)water, rail, or air transportation? If so, generally describe. No. e. How many vehicular trips per day would be generated by the completed project or proposal?
