1) Holding tank must have audio/visual alarm. Pump line must be cased. Installation permit for tank placement required. Tank must be installed by a
Mason County Certified Installer.
2) All conditions set forth in DDR2007-00009 and SHR2007-00003 are conditions of this permit.
3) The proposed project must be consistent with all applicable o�cies and other provisions of the Shoreline Management Act, its rules, and the
Mason CountyShoreline Master Pro ram.X 2'jl �

4) Excavated materials cannot be used as Ian�f hin 200 feet of the shoreline without prior approval from the shoreline division of the Mason
County Planning Department. X ����
5) Parking shall be sufficient for normal parking stalls (9 feet by 20 feet) and handicap parking stalls (12.5 feet by 20 feet)with sufficient
maneuvering aisles. Handicap stalls shall be of a smooth surface at level or ramped to entry, located closest to the byfldi g;1entry, and shall be
signed with the International Symbol of Access. Screening from adjacent residential properties is required. X �V
6) All approved plans are required to be on-site for inspection purposes. If inspection is called for and plans are not on site, Approval WILL NOT be
granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected by the Mason County
Building Department prior to any further inspections being performed or approvals granted. X

7) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely
impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements
of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose.
For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a
driveway or access connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450.
For any construction which is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that office to review
future planned work hich may affect your project.

8) All construction on site associated with KLEINFELDER Geotechnical Engineering Report No. 77183 must be completed per stated
recommendations contained within the report. Changes can be made per recommendations by the Consulting Engineer and approved by the
Planning Department/Mason County as necessary.
9) Temporary erosion control measures must be implemented as necessary t pr vent water quality degradation of adjacent water bodies of the
state. Silt fencing must be installed per BMP C233 as necessary. X
10) A fuel spill response plan needs to be implemented and kept on site for inspection. An appropriately sized spill kit(industry standard) for land and
water needs to be kept on s2L-
11) tocked at all times to keep any fuel spills from infiltrating into soils and reaching state waters or dispersing
within the water. X Construction stormwater runoff that discharges to water bodies of the state must meet state water quality standards. See Washington State

construction must be obtained PRIOR TO SAME DEVELOPMENT AND CONSTRUCTION. X
14) PER TITLE 14 MASON COUNTY BUILDING CODE-CHAPTER 14.17, STANDARDS FOR FIRE APPARATUS ACCESS ROADS - 14.17.110:
A fire apparatus access road in excess of 14% grade and more than 150'to new residential or commercial structures will require an automatic fire
sprinkler system installed. Contact the Mason County Fire Marshal at(360)427-9670, extension 273, for further information.

15) Provide an emergency power shutoff switch within 75 feet, but no closer than 25 feet from the dispensing station. Provide a NO SMOKING sign on
tank. Provide a 20 BC rated fire extinguisher located within 75 feet of dispensing station. Provide protective bollards as follows: Guard posts
constructed if steel not less than 4 inches in diameter and concrete filled, Spaced not more than 4 feet between posts on center, Set not less than 3
feet feep in a concrete footing of not less than 15 inches in diameter, Top of posts not less than 3 feet above ground, Located not less than 5 feet
from the tank,,4n inspection by the Mason County Fire Marshall is required before the tank ios put into service.

16) The fuel storage tank and all equipement piping and appurtenances are reuired to be UL approved and installed per the manufactures installation
specifications a dpeA 30 ,30A and 2006 IFC. The specifications are required to be on site during the inspection.

17) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance
Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be
obtained at 1-800-647-0982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to

This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at anytime after work is
commenoed. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Proof of continuation of
work is by means of a progress inspection.The owner or the agent on the owners behalf, represents that the information provided is accurate and grants employees of Mason County access to
the above described property and structure for review and inspection.
