THIS APPROVAL IS TO BE AVAILABLE ON THE JOB SITE AT ALL TIMES AND ITS PROVISIONS FOLLOWED BY THE
PERMITTEE AND OPERATOR PERFORMING THE WORK.
SEE IMPORTANT GENERAL PROVISIONS ON REVERSE SIDE THAT ARE ALSO PART
OF THIS APPROVAL.
1. The use of equipment within the wetted perimeter shall be held to
an absolute minimum.
2 . Drive mechanisms of wheeled or tracked equipment shall not enter
the wetted perimeter.
3 . Alteration or disturbance of bank, shoreline, . or bank and shore-
line vegetation shall be held to an extreme minimum, and all dis-
turbed areas shall be protected from erosion and revegetated.
4 . The onl permissible removal of aquatic vegetation is that inci-
dental o the physical movement of piles into the substrate.
5. Extreme care shall be taken to assure no deleterious material
such as fresh cement, concrete, lime, petroleum products,
chemicals, etc. , be allowed to enter the water.
6. Material treated with penta, creosote, or other preservatives
shall be completely dry prior to water encroachment.
,7 . There shall be no concrete poured into the water.
8 . Debris from this pro ect is to be removed and disposed of by
placing beyond high water flows.

9. This ap roval authorizes dock construction only. Any additional
work within the ordinary high water line will require a separate
Hydraulic Project Approval.
10. If styrofoam floats are used, they should be fully covered or
wrapped to prevent damage by otters and muskrats.
11. There is to be absolutely no removal of logs or large woody
debris from the lake bottom without the approval of Department of
Wildlife's habitat biologist on a case-by-case basis.
NOTE: This permit covers only those projects that are exempt from the
SEPA process. Any protects that require SEPA determination from
the county will require a separate Hydraulic Project Approval
from the Department of Wildlife.
NOTE: This approval relates to the protection of aquatic habitat onl .
Where equipment ingress and egress require use of Department o
Wildlife access areas a seperate approval shall be negotiated
with the Department of Wildpife's regional lands agent.
