Fish passage is not a concern at this site . 1 . Work shall be accomplished per plans and specifications submitted to WDFW, except as modified by this HPA. A copy of these plans and this HPA shall be available on site during construction. 2 . ADDITIONAL TIMING !,IMITATIONS: Work below the ordinary high water line (OHWL) shall only occur between June 15 and September 30 of calendar ears 1997 and 1998 . Reveetation per Provision 18 shall g be completed no later than October 30th the year the culvert is installed and shall be monitored through October 30, 2001 . 3 . All work shall be accomplished duriuy a low-flow Pc=_.,.. ••------- the time limitations per Provision 2 . 4 . The temporary culvert or bridge used to move ecruipment . and supplies to the opposite side of the stream, shall only remain in place between Tune 15 and September 30 of calendar years 1997 and 1998 . This structure shall be installed and maintained to pass all anticipated flows with consideration of the debris likely to be encountered. ! The structure shall be installed to maintain structural integrity ` c `he 100-ve r -.e t a � ak i_oc� with consideration of the debr; s - to be encountered. - 6 . If a culvert is used, it shall be installed and maintained to avoid inlet scouring and to prevent erosion of stream banks downstream of the project . 7 . Clean angular rock may be used to armor the inlet and the outlet of the culvert . The rock shall be installed to withstand 100-year peak flows . River gravels, other round cobbles or concrete shall not be used as armor. 8 . Fill associated with the structure installation shall be protected from erosion to the 100-year peak flow. 9 . If a temporary culvert culvert is used, it shall be installed in the dry or in isolation from stream flow by the installation of a bypass flume or by pumping the stream flow around the work area. 10 . A sandbag revetment or similar device shall be installed at the bypass inlet to divert the entire flow through the bypass . 11 . The bypass shall be of sufficient size to pass all flows and debris for the duration of the project . 12 . Prior to releasing the water flow to the project area, all bank protection or armoring shall be completed. 13 . Upon completion of the project, all material used in the temporary :wpass and the temporary crossing snaii be removed from the `site and the site returned to preproject or improved conditions . i4 . Erosion control methods shall be used to prevent silt-laden water From entering the stream. This may recnzire the use of straw bales, filter Fabric, tempora sediment gg filled burlap bags or other material and/orcimmediate mulchinVeof exposed areas . Accumulated silt shad be removed with the filter fabric, straw bales or check dams upon completion of the pro ect and the burlap bags shall be slit to allow the pea gravel to disperse downstream. 15 . Prior to startina work, temporary filter fabric, straw bale, or pea gravel-filled burlap bag check dam (s) shall be installed sownstream. Accumulates sediments shall be removed during the project and prior to removing the check dam (s) after completion of worx . Imported fill which will remain in the stream after temporary crossing removal shall consist of clean, rounded, uniformly-graded gravel ranging in size from 1/4 to three inches in diameter. 17 . Affected streambed and bank areas outside the temporary crossing and associated fill shall be restored to preproject configuration or better following construction. 18 . Disturbance of the streambed and banks shall be limited to that necessary to place and remove the temporary crossing. The temporary crossing and fill shall be removed, and the disturbed bed and bank areas shall be reshaped to preproject configuration. Within 7 calendar days of project completion, all disturbed areas shall be protected from erosion using vegetation or other means . Within one year of project completion, the banks shall be revegetated with native or other approved woody species . Vegetative cuttings shall be planted at a maximum interval or 3 feet (on center) and maintained as necessary for three years to ensure 80 percent survival . 19 . As mitigation for the loss of stream and its function due to the house spanning the stream the applicant, through the authorized agent David Grellier, shad maintain a vegetative, native plant, woody buffer along the remainder of the stream through their ownership.
