Scrap Metal Addendum
to obtain a license as a
Scrap Metal Processor,Scrap Metal Recycler,and/or Scrap Metal Supplier
Application Procedure
1. Review the Endorsement and Fee Description Sheets 5. Submit the completed Business License Application,

Section A: Endorsements Requested (All applicants)

We have received your application for a Motor Vehicle Wrecker license(s). The
following is needed to finish processing your application:
Complete the enclosed Corporate Information form.
Complete sections 1 & 5 on the enclosed Vehicle Transport/Disposal Addendum.
Completion of the Listing of Vehicles Used to Conduct Business form enclosed. If
you will be requesting motor vehicle wrecker plates on this form, you will need to
enclose a check for the fees. First motor vehicle wrecker license plate is $6 and
each additional wrecker license plate is $3.
Completion of the Environmental Checklist enclosed.
Completion of the Personal/Criminal History Statement form enclosed.
Please contact your bonding company and have them fax (360)586-6703 me a bond
rider adding "dba Acceleration Auto Works" to your corporate name and indicating
the license period commencing date. Most bonding companies can fax a bond rider
the same day that you request it.
Section 6 of the Vehicle Transport/Disposal Addendum needs to be completed by
your Local Zoning Official.
Section 7 of the Vehicle Transport/Disposal Addendum needs to be completed by
an Inspecting Officer.

In reference to building permit 4 COM2006-00070. There seems to be some kind of
misunderstanding. After speaking to Chuck McCoy at the planning dept. and Debbera
Coker at the dept. of community development it is clear to them that this is not a new
project, but rather I am taking over an existing business willed to me at the time of my
fathers death on April 28`h of 2005. Nothing has changed here at Belfair Truck and Auto
Wrecking. Nothing is being built and the structures are the same as they have been since
1975 when my father had taken over. Also I have owned and operated Acceleration Auto
Works on the same property for the last six years. The reason I had to go to the planning
dept. in Shelton was to submit a S.E.P.A. report to the local zoning officials to obtain a
new wrecker number through the state. However Mason County does not even require
such a report.
