The loss occurred during the December 07 storm. This permit, with conditions, is granted pursuant to the Mason County Resource Ordinance (Chapter 17.01 MCC.) Nothing in this permit shall excuse the applicant from compliance with any other federal, state, or local statutes, ordinances, or regulations applicable to this project. This permit may be rescinded if the permitee fails to complete the project as proposed or fails to comply with the standards of this ordinance. Authorized Local Government Official Date Mason Environmental Permit 7/24/2008 Case No.: MEP2008-00057 Conditions: 1.) Equipment used for this project shall be free of external petroleum-based products while working around the stream. Accumulation of soils or debris shall be removed from the drive mechanisms (wheels, tires, tracks, etc.)and undercarriage of equipment prior to its working below the ordinary high water mark. Equipment shall be checked daily for leaks and any necessary,rep i shall be completed prior to commencing work activities along the stream. X ' (f 2.) All construction and demolition debris must be removed from the beach after project completion. Proper disposal of construction debris must be on land in such a manner that debris c t enter or cause water quality degredation of State waters. X ' ' n 3.) Appr va r di ensions and setbacks on submitted site plan. X 4.) If at an time, as a result of project activities, fish are observed in distress, a fish kill occurs, or water quality problems develop (including equipment leaks or spills), immediate notification shall be made to the Washin too partment of Ecology at 1-800-258-5990, and to the Area Habitat Biologist. X 5.) Erosion control methods shall be used to prevent silt-laden water from entering the stream during all phases of the project. These may include, but are not limited to, straw bales, filter fabric, temporary sediment ponds, check dams of pea gravel-filled burlap bags or other material, and/or immediate mulching of exposed areas. If high flow conditions that may cause silta 'on ap enFountered during this project, work shall stop until the flow subsides. X ,7 6.) Applicant acknowle ges that permit conditions may be amended based on the outcome of the re-evaluation by Washington Department of Fish &Wildlife and on the recommendations of the Habitat Management Plan that will be submitted. The applicant will nq/�oled prior to any proposed amended or changed conditions. X ( 7.) The app icant acknowledges that the project will be completed as proposed, so that the project will be consistent with all applicable standard hg Mgson County Resource Ordinance and the Shoreline Master Program. W 9.) As required by the Mason County Resource Ordinancd 17.01.110 Fish and Wildlife Habitat Conservation Areas G(1)(a)a Habitat Management Plan will be required. The Habitat Management Plan requirements are outlined in section J. This report, prepared by a qualified fish and wildlife professional, is to be submitted to the County for review by September 24, 2008.The report shall identify how the impacts from the proposed activity will be mitigated through i mw'tigation. A SEPA Environmental Checklist will also be required at that time. X IIJJ 10.) As required by the Mason Co'urlty Resource Ordinance 17.01.100 Landslide Hazard Areas (E)a special report will be required for development within a Landslide Hazard Area or its buffer or within 250 feet of the buffer. The Special Report requirements are outlined in section E. This report, prepared by a licensed civil engineer or a licensed geologist, is to be sCITY the County for review by September 24, 2008.
