The approved site plan and other applicable instructions, including installation instructions, shall be available in this location OR placed in the location specified by WAC 296-150M. WAC365-210 * The plan review check list and corrections are part of the approved plans and must remain attached. It is the responsibility of the applicant, owner or contractor to make the required corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or removal of approved documents will result in failure of required building inspections. Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, connectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. The stamped approved site plan is required to be on-site for inspection purposes. If an inspection is requested and the approved site plan is not on site, approval will not be granted. A first addendum to that report was dated February 22, 2014 addressed to the client. I have prepared this addendum to the original geotechnical report to fulfill the requirements of Mason County Planning that there be an update to the report in support of the construction of a small home on the client parcel. This addendum should be submitted with all required supporting documents in order to ensure satisfaction of requirements. BACKGROUND A geotechnical report was prepared by Hal Parks of GTL on July 26, 2007 for the above- mentioned parcel, originally P.N. 12018 21 00000. This was a block of waterfront owned by Christine Wells which was to be sub-platted into four residential parcels. The report indicated that the sites were suitable for construction. However, rules changes required further tests be done with a drilling program to test the deeper sub-surface. The site map was upgraded following this test program and the slope-stability models refined. This work, dated January 16, 2008 and signed by Curtis D Cushman, then with GTL, verified the general conclusions of the report. The first addendum was prepared in 2014 to clarify observations for the checklist previously not required by the county. This present addendum is to specifically address the proposed current location for the small home. SITE INVESTIGATION The site was visited again by Curtis D Cushman LEG, LG and Blaise Jelinek EIT of AAG On November 6, 2024. Client, contact, and Jeremy "Jeb" Thornburg, architect of Tieton Cabin Company were on site.
