The above mentioned building permit has been reviewed by Environmental Health and found more information is
required.
The building site plan does not match up with regard to septic system location/reserve drainfield designation on 2008
asbuilt.The reserve was designated to be 20x30ft area just north of the house but it no longer looks like there is room
available there according to GIS map and building site plan. Reserve drainfiekls need to be 5ft back from property line
and 10ft back from foundation/foolings.We will require a septic designer submit a site plan showing there is adequate
room for a reserve dreinfeld on this lot in the event the primary drainfeld fails.We cannot approve an expansion of
footprint unfit we are sure there is adequate reserve drainfeld area.
Sepfic system is overdue for maintenance.Aerobic Treatment Unit systems require annual Operation and Maintenance
inspections. Please let us know when this is complete and satisfactory.

Finally,we will require a Certifica esidential Use: Limitation on Number of Bedrooms.This septic system is
designed for 3 bedrooms/6 occupants.The new Floor plan shows two additional rooms,both of which have closets.
Please see attached form, have notarized and recorded with Auditors office. Please submit a stamped copy to our office
once recorded.
