III 426 West Cedar Street P.O.Box 186 Shelton, WA 98584 Subject: Lilliwaup Falls Power Project— Landslide,Conveyance Pipe and Intake Repairs Final Inspection Report for Permit#GRD2012-00006 This final inspection report presents a summary of professional engineering inspection provided by Jacobs Associates(JA) for the Lilliwaup Falls Power Project. The Lilliwaup Falls Power Project construction documents consist of the following: • "Landslide,Conveyance Pipe and Intake Repairs"-25 sheets of plans, Jacobs Associates, June 13,2014 • "MSE Retaining Wall, Specifications, Plans & Stability Calculations", Ontiveros Cesaretti & Prebor,June 12,2014. • "Geotechnical Report for Landslide and Conveyance Pipe Repairs",Jacobs Associates,April 12, 2012 Project construction inspection was performed.for the following work: 1) Grading operations including line, grade,and surface drainage of the developed area; 2) Welded wire retaining wall system including compaction testing of wall backfill; 3) Land modification and stormwater controls; 4) Conveyance pipeline installation and backfilling. A discussion of construction observations for each of these project elements is presented below. Jacobs Associates has provided Mason County thirteen weekly construction summary reports prepared by the engineer of record, Frank Pita. These reports comply with the requirements noted in Chapter 14.44 of the Mason County Code, Section 14.44.220 through 14.44.230. The weekly reports include a weekly summary of the project construction, and a copy of the daily Construction Inspection Reports for each day JA was onsite(30 days). Grading Operations The project site grades were generally constructed per plan. Areas above and below the retaining wall are graded at a maximum 2H:1 V slope, except where slopes transition into steeper existing slope areas. Fill slopes were constructed and compacted in accordance with the project plans and 1109 First Avenue, Suite 501, Seattle,Washington 98101 Phone: (206)588.8200 Fax:(206)588.8201 r 2 specifications, under the observation of a JA engineer. Slopes have generally remained stable since the completion of grading in early October. See also Land Modification section below for stormwater pollution prevention measures. All material cut or excavated during construction was placed as fill on the project site. Material was imported to the site for the following uses: quarry spalls for erosion control, gravel for pipe bedding/backfill, washed drain rock for the underdrain system, and crushed surfacing for access roads. Welded Wire Retaining Wall A 52-foot high Hilfiker Welded Wire Retaining Wall was constructed on the site to stabilize the landslide area and provide support for the conveyance pipeline. The wall was designed by Hilfiker Retaining Walls. The wall consists of non-galvanized black wire reinforcing mats and backing mats, and Miramesh GR, a geosynthetic fabric to retain soil at the wall face. The wall reinforcing mats extend up to 32' deep from the face of the wall. The existing on-site soils were suitable for use as wall backfill. The wall was generally constructed per the design plans. However a slight modification in wall alignment was made due to site conditions encountered during construction. This change was reviewed and approved by the Hilfiker on-site representative. The as-built wall alignment is shown on the attached Figure 1. Wall backfill operations were observed by JA engineer, and a %"hand probe was used to verify fill was firm and unyielding. In addition, an independent testing service, Materials Testing & Consulting, Inc. (MTC), performed regular compaction testing with in place nuclear density gage. These test reports were included in the weekly construction summaries previously provided to the County. The wall backfill met the compaction requirement for 95% of Standard Proctor density. The foundation drain system was generally constructed per the design plans. However, some of the perforated pipe lengths were modified to fit site conditions. All changes to the plan were made under the observation of a JA engineer. All installed slotted drain pipe was 12-inch, Schedule 80 PVC. Pipe was bedded in washed drain rock. The as-built foundation drain system is shown on the attached Figure 2. Land Modification and Stormwater Controls Per the project construction documents, and in response to site conditions, the following erosion control measures were installed: • Hydroseed with bonded fiber matrix (BFM) was applied on the graded upslope area, above the retaining wall. • Areas steeper than 2H:1 V were staked with jute matting and hand seeded. • All other disturbed areas were covered with straw and hand seeding. • Trees, shrubs and live stakes were planted on the slope areas above and below the wall. • Silt fencing installed per original plans. 1109 First Avenue,Suite 501,Seattle,Washington 98101 Phone: (206)588.8200 Fax:(206)588.8201 r 3 • Straw wattles were installed on slopes per plans. • Quarry spall check dams installed along new access road per plans. • Water bars were graded into all existing site access roads at regular intervals. • An interceptor swale was installed at the top of upslope area to divert runoff away from the newly graded slope. • During construction a portion of the 2H:1 V slope above the wall experienced seepage and sloughing. A 2-foot thick layer of quarry spalls was installed to stabilize the slope. This area now appears to be stable. The site experienced significant rainfall events in October and November and has remained stable with very little erosion. No off-site discharge of sediment has been observed. The site has stabilized and all erosion control measures are complete. Conveyance Pipeline The conveyance pipeline consists of approximately 450 if of Advanced Drainage System's (ADS) 48-inch Sanitite HP pipe. The conveyance pipe connects the concrete flume at the gate house to the screening reservoir,just upstream of the steel penstock. The pipe was installed per the plan alignment, with granular pipe bedding surrounding the pipe. The final pipe slope is approximately 0.1%due to existing elevations at the connection points. The pipe bedding material was reviewed and approved by the JA engineer. The pipe interior was also inspected by JA after installation. The connections between the pipe and the steel transition structure and the concrete screening reservoir were made with flexible watertight couplings fastened with pipe clamps. A low pressure air test was performed on all the pipe joints (3.5 psi for 5 seconds),per the manufacturer's recommendations.All joints passed the air testing. Closure This report was authorized by and has been prepared for the exclusive use of Lilliwaup Falls Generating Company for Mason County permitting requirements. The site grading and drainage construction is complete and ready for final inspection by Mason County. If you should have any questions or comments, please do not hesitate to contact us. Lilliwaup Street, Lilliwaup WA Report #1 By: Thomas Pallua, Bill Gates Arrive: 8:30am Depart:4pm Contractor: Granite Construction, 3 employees working Weather: Partly sunny, 60's F Summary: 8:30 arrive on site 9:00 safety briefing with contractor 9:15 start installing ladder access 15:30 complete ladder access 16:00 left site — Site conditions and safety issues: The gate house is currently not accessible because the existing access way collapsed after a recent land slide. The gate house is located 62 feet below the top of the slope. The slope shows an inclination of 62 - 82°. A temporary ladder access with fall protection to the gate house is required for safe access for the contractor. Other safety issues are rock fall and the presence of poison oak on the slope face. — Work done: Because of the limited access, JA engineering geologists had to rappel to the flume next to the gate house. To clear an access, first brush and branches were cut with the aid of a machete and obvious loose rock was manually removed along the slope. Next, a 20-foot-long aluminum extension ladder was lowered down to the gate house by one engineering geologist on rappel, while the other one, secured on a second line, anchored the ladder to the slope using tree trunks as anchor points.The ladder was secured to the slope using 6mm, 10ft and 30ft anchor ties secured to nearby trees and stumps. A second 20-foot-long ladder was installed adjacent and above the first using a similar anchor method. A 15-foot ladder was installed on top of the slope with the aid of 10-feet anchor lines and two 1 inch flat webbing anchors. Next a 100-foot-long 5/8" life line rope with a snap hook was anchored on top of the slope to a large tree trunk and lowered to the gate house.The life line rope was equipped with a steel rope grab to use as additional safety device for descending and ascending the ladders. Granite workers were instructed by JA engineering geologists on how to safely use the rope grab or use an alternative sling with a prussic knot on the life line for descending and ascending. The ladder anchorage system is intended to be strictly temporary for the period of the construction. If for any reason the ladder access needs to be used during winter months, the Lilliwaup Falls Power Project anchorage system needs to be inspected or the anchors should be replaced with cables and rock bolts. Observations and Comments • Beware of loose and raveling rock above the flume and gate house. • Consider safety scaling above the flume and gate house; • Use fall protection at all times when descending to gate house; • Control poison oak along the slope face with herbicide and pruning. Supplies used ladder access: • 150 feet of 6 mm anchor line • 60 feet of 1 inch flat webbing anchors • 2 steel locking carabiners • 1 steel 5/8 inch rope grab device • 1 life line, 5/8 inch diameter 100 feet rope with snap hook Gate house: The existing gate house was inspected and the following was noted: — Gate on SE aspect is electric winch operated; — Gate on E aspect is lever and gear operated; — Both gates leak at the base and must be cleaned of debris to get a proper seal. Lilliwaup Street, Lilliwaup WA Report #2 By: Frank Pita, PE/LHG Arrive: 9:45am Depart:4pm Contractor: Granite Construction, 6 employees working Weather: Partly sunny, 60's F Additional Staff onsite: Ron Gold,Wayne Marshall, caretaker. Site Observations&Comment • Arrived site with Craig Norsen, Seneca CM, who had pre-arranged meetings with Granite's management and two electrical contractors. The first meeting took place at 10AM. • During that meeting, Granite's Superintendent Dave & myself went to the work area and observed the access road to the bottom, the excavation around the bridge and tree falling work being performed around the edges. (see photos below) • Dave& I looked at the slopes and the material types. • We drove over the top where the staging area has been made & drove the access road down to the stockpile area where drain gravel is being placed. It will be transfer into an offroad dump truck & brought to the site. I inspected & approved the material (see photo below). • We continued back to the house& picked up Craig& Ron and went to the site again. • Craig had another meeting with an electrician at 2PM. • Then we went to the intake& measured the sizes of the mechanical screens that are on site. • The tree falling was complete at that time. Future Tasks • Granite's staff asked for wall layout instructions, which Craig & I provided in the field & we will get them a drawing. • Granite needs classification information of the material composing the steep hillsides from a stability standpoint & they need direction on grading the over-steepened area above the screen house site. • Granite &JA decided that on Monday we would obtain samples for compaction testing and bring them to the lab on Monday PM. • 1 sent an email to MTC lab telling that & they may be needed toward the middle of the week. • Also, we checked the cleanout work of the return flume (see photos). A future discussion is needed with Mr. Reed about what condition the sides of flume are to left and whether the dead trees should be removed. The'jumping jack' is used against the rock and at the face. • We are building the wall in 4 foot high vertical face sections and then we will step in. This will promote more vegetation on the face at the benches according to Len. The end product is the same. • Changes have been made to the initial design because of the location of rock, which Len has no problem with. The angle point will not exactly at the place as drawn on the plans, but close. Total wall height will be 52'. • Per plans, the wall is stepped back 1' per each 4' of height. This pattern will continue up the wall face. • The wall layout was modified due to field conditions. The convex angle point at the front of the wall was shifted 8'west of the original location;otherwise the wall is the same. • Native material is granular and meets the specification for wall backfill. Sieve tests of the material were performed during the design phase. Backfill is being placed and compacted with steel vibratory roller in 1' lifts to within 3' of wall face. Adjacent to face of wall a jumping jack and plate compactor is being used. • Toe of MSE wall was backfilled to 5'depth per plans. • Slope below wall was compacted and stabilized with quarry spalls, loose straw and straw wattles. • Personnel from Materials Testing & Consulting, Inc. (MTC) performed nuclear density tests on Wednesday and Friday at the top of 6' of wall fill, and 10' of wall fill. MTC collected two more samples of wall backfill for proctor testing. We are awaiting the lab proctor test results to calibrate the compaction values. • An additional 40' of slot drain was installed approx. 10' upslope of first drain (which was at the back of the wall). Pipe was bedded in drain gravel. • Grading operations, MSE wall installation, and stormwater controls are all in general conformance with the design documents. Paul indicated he has had no problems with the airstrip being impacted. • GC placing and compacting 4th mat layer(each soil layer is 2'thick separated with wire mat, 26 total layers=52'vertical).At end of day they were installing the top mat to this layer. • A total of two 4' high levels have been installed, 11 remain to be constructed. • Lance Levine, a geotech engineer with Materials Testing & Consulting (MTC) performed 2 nuclear density tests just to the north and to the east of the 41h wire mat (tested the top of the 3rd wire mat soil layer). He could not test within wire mat area due to interference with test equipment. Lance did not have lab proctor results available, so compaction results will be forthcoming. Material moisture was 6%to 8%+. Lance took two additional 4 gal. samples of fill material with him for testing. • GC backfilled and compacted the toe of the Hilfiker wall to a 5' depth per plans, with a 4'+ wide bench along the wall. Quarry spalls were placed on the slope from top to bottom in the vicinity of the outfall pipes, at my direction. (Spalls extend further than indicated on plans, but are recommended due to the possibility of water seepage out of the face of the slope through the drainrock that is bedding the 12" PVC pipes.) • GC installed two lengths of straw wattles on finished slope below wall. (Plans specified only one length.) • Rob Ames and I discussed possible alternatives to the transition structure/manhole at the upper end of the pipe. • Decided against extending the ends of the PVC pipes with additional piping. Current pipe end location is adequate. • Maureen flagged the alignment of the penstock with pink flagging, between the access road, and the bottom of the hill near the power plant. I will review this location with Dave Thursday. Site Observations&Comment • GC placed and compacted an additional half width mat at the east end of the 4th mat layer (each soil layer is 2'thick separated with wire mat,26 total layers=52'vertical). • GC placed and compacted the 51h mat layer(a 2' lift). • GC placed additional drainrock and quarry spalls at the west end of the toe of the wall, adjacent to the rock face. • GC installed 40' of slot drain approx. 10' upslope and parallel to the first 20' section of slot drain.Slot drain was bedded and backfilled with washed drain rock. • A total of five 2' high levels (10 vertical feet) have been installed, 42' vertical feet remain to be constructed. • GC had 5 employees onsite, including David Schneller the super. • We are still awaiting proctor test results from MTC lab. These results are taking too long to obtain, and we may need to switch to a more responsive testing service next week.The lab tests are required in order to calibrate the compaction results. As a result, we do not know the actual compaction of the fill material. MTC will be onsite on Friday 10 am to take additional compaction tests. • However, Frank Pita (JA) was onsite in the a.m. and observed the ongoing compaction and tested the fill with a steel probe. He indicated the compaction was acceptable, but is conditional upon reviewing the final compaction testing results. GC has been directed to proceed with fill and compaction of wall lifts, in light of not having compaction test results. • The existing onsite fill material being used for the wall backfill is of good quality, with low fines and minimal oversize rocks (greater than 6"), and with good moisture level for compaction. • Hilfiker's rep, Len Mead, was onsite all morning. Len had some additional suggestions for wall installation. He told David they would expect approximately 4' to 6' of height wall should be install per day. Only 2'was installed today. • I asked Len about alternatives for installing a fence at the top of the Hilfiker wall. He said they recommend use of their'spiralnails' which can be drilled into the wall fill and used for posts, with chain link or cables installed between the posts. The plans currently show an ecology block wall along the top of the wall. The convex angle point of the front of the wall is located approx. 8' (one mat) west of the original location, and the angle is a little flatter. • A total of six 2' high levels (12 vertical feet) have been installed, 40' vertical feet remain to be constructed. • GC had 5 employees onsite, including David Schneller the super. • MTC was onsite and took additional compaction tests. • We are still awaiting proctor test results from MTC lab.The lab tests are required in order to calibrate the compaction results. As a result, we do not know the actual compaction of the fill material. • Maureen& Dave Schneller(GC) accessed the gate house area and took additional measurement and photos. Future Tasks&Schedule • Next week JA will only be onsite Thursday for the construction meeting. Ron Gold will be on site daily to monitor wall construction. • MTC will be scheduled to perform additional compaction testing on Wednesday and Friday (Sept. 3 &5). • JA to obtain information on testing of pipe joints. Owner's rep, Ron Gold, is onsite every day. • MTC, Inc.took in place nuclear density tests on Sept. 3 and 5. They will continue to perform nuclear density tests approximately twice a week. • Results of the compaction testing that has been performed to date are attached herein. Nuclear density tests were performed as follows: o Aug 27 at 6'wall height,two tests fail(less than required 95%compaction at 89.2%, 91.9%) o Aug 29 at 10'wall height,two tests pass o Sept 3 at 15'wall height,two tests pass o Sept 5 at 17'wall height,five tests pass The two readings taken the first day of testing did not meet the specified 95% compaction. However, testing with a %' probe by JA staff indicated the soils were dense and unyielding, and did not warrant replacement or reworking of the soil lift. In our opinion low test results were likely caused by coarse gravels within the fill. (Note the nuclear density test results were not available when the field tests were made. Now that lab proctors have been completed, compaction test results can be read immediately in the field during testing.) • The 7th through 101h of the 2' thick mat layers of MSE wall were installed, for a total completion of 20' of wall height. Eight vertical feet of wall height was completed during this week. Total wall height will be 52'. • An additional 54'of slot drain was installed.Twenty feet of drain was placed along the rock face, and a 34'section of horizontal drain extended across the fill area. Layout is similar to the design plan. • Approximately 120 If of existing concrete flume, just east of the slide area was demolished and removed. • Grading operations, MSE wall installation, and stormwater controls are all in general conformance with the design documents. GC was advised to use solid pipe on vertical sections, but no solid pipe was available,so perforated pipe will be used for all remaining drain pipe, per original plan. Contractor: Granite Construction Site Observations&Comments • JA Engineers were on site 4 days this week to observe installation of Hilfiker MSE wall and site grading operations. Owner's rep, Ron Gold, is onsite every day. • Hand probing of the fill by JA staff indicates wall backfill is firm and unyielding. • MTC, Inc. took in place nuclear density tests on Sept. 9 and 12. They will continue to perform nuclear density tests approximately twice a week. • Results of the compaction testing performed this week are attached herein. Nuclear density tests were performed as follows: o Sept 9 at 22'of wall height(elev. 281),three tests pass o Sept 12 at 28'of wall height (elev. 287)two tests pass,three tests at 90%-93%(not pass) • Top of soil layer at elev. 287 appears dry and powdery, but was firm under hand probe. Additional passes with vibratory roller were made, but did not improve compaction. JA provided recommendations to GC for moisture conditioning or thinner lifts to achieve the required compaction on future lifts. • The 111h through 15`h of the 2' thick mat layers of MSE wall were installed, for a total of 30' of wall height completed. Total wall height will be 52'.Ten vertical feet of wall height was constructed this week. • Contractor is continuing to grade the top of the landslide scarp, and is pushing material down to be used for the MSE wall backfill and construction the 2H:1V fill slope. • Underdrain installation was completed, with 160 If of 12" diameter slot drain pipe in place at the back of the wall. Two solid 12" pipes extend under the wall and discharge at the front face into a riprap area. No observed flow in pipes at this time. • Grading operations, MSE wall installation, and stormwater controls are all in general conformance with the design documents. Each soil layer is 2'thick separated with wire mat, 26 total layers=52'vertical. • GC had 6 employees onsite, including David Schneller,the super. • Ron stated a lot of buried debris had to be removed from the area behind the wall in order to complete the underdrain installation. Most of the fallen trees, stumps, debris have been removed from the slope area. • Hand probe of MSE wall elev. 279 indicates backfill is firm and unyielding. • Groundwater seepage is occurring just past the angle point of the rock face, where it had been noted in the past. Elevation of seepage is approx. 285. No water observed discharging from 2 outlet pipes.The low volume of flow is likely moving subsurface. • GC installed additional 20' of slot drain pipe along the rock face, and 27' of slot drain pipe laterally across the slope, with drain rock. This completes the underdrain installation. A total of 161 If of slot drain was installed, and is adequate. Pipe layout was generally per the plans. Plans showed 170 If of slot drain, but due to steep site slopes it would be difficult to install more. Three tests approx. 15' from wall face are at 90%-93% compaction (not passing). Two additional tests approx. 25' from wall face pass 95% density specification. Top of soil layer appears dry and powdery, but is firm under hand probe. I recommend contractor make additional passes with vibratory roller.This is done and no significant soft spots are observed under roller. MTC takes 2 additional tests at 15'from wall face, results are still 90%-93% compaction. Moisture values are 5%-6% (low). I notify David compaction is acceptable for this lift only.JA(F. Pita)sends an email to GC 9/13/14 recommending moisture conditioning or thinner lifts to achieve the required compaction on future lifts. • Maureen and David climb down to Gate House to take additional measurements of the flume end and determine where to make sawcut. JA will revise the permit drawings of the steel transition at the gate house to reflect final measurements,and forward these to GC on Monday. • JA is in the process of reviewing and modifying the design for the screening vault and should have revised drawings Monday or Tuesday. Logs are in a separate pile. GC needs direction for disposal. It may be inadvisable and difficult to spread this on the finished slope. Use of chipped material may be feasible. • David asked whether the access road should remain along the top of the cut slope. This area is cleared (see photo),and Ron and Gary thought it was a good idea. • Ron and Ben Alexander reviewed the area below with VISE wall, and area adjacent to Winter Creek for planting restoration. Ben will provide a price for this installation and an estimate later for the slope restoration. We recommend planting proceed ASAP. Nuclear density tests were performed as follows: o Sept. 23 on mid-slope access road,two tests pass • The 22`h through 245` of the 2' thick mat layers of MSE wall were installed, for a total of 48' of wall height completed.Total wall height will be 52',4 vertical feet remain to be completed. • Grading of upper slopes areas is essentially complete. • Site received approximately 5" of rain this week.Contractor made adjustments to erosion control to address changing conditions, adding filter fabric fence, straw, redirecting runoff. System was performing well, and no offsite discharge of sediment was observed. • Wattles were installed across large slope area, per plans. Some erosion and sloughing occurred in one area of the 2H:1V fill slope, approx. 40' x 120'. It appears this was caused by subsurface seepage.Areas will be stabilized with quarry spalls. Remainder of slope performed well. • Cleared vegetation was shredded on site for reuse. • Most of the 48"Sanitite HP pipe is stockpiled on site. • Grading operations, MSE wall installation, and stormwater controls are all in general conformance with the design documents. Rain interrupted testing. • JA used %' hand probe to check density of the upper slope area (above the access road). Soil is loose to 6" to 8" depth (tracked with dozer), and firm and unyielding at 6" + depth. The upper slope is all in a cut, and did not receive new fill so nuclear densometer testing is not applicable. • GC stated they needed additional fill material to complete the wall, approx. 1700 cy.JA directed them to remove the 4' bench that was recently added at elevation 350. Dozers are working up and down the slope to move material down from this bench & finish grading. Grading and compaction of the slope and MSE wall backfill is ongoing. • JA will review erosion control plan to see if additional erosion control measures are needed due to removal of 4' bench. • MSE wall mat at elevation 303 was ended about 5' short of the rock face, due to lack of enough mats. GC was directed to place quarry spalls to fill the void and extend the next mat layer (el. 307') all the way to the rock face.A mat was deleted from the east end of the wall for this use. • Chipper arrived chip the pile of cleared vegetation. Chipping should be complete this week. • GC requested clarification for a berm at the top of the slope,along the access road.JA will issue. Wall fill is at elev. 308. Soil at top of wall is wet and will need to dry out prior to continuing. The site received approximately 4"—5"of rain this week, based on a site rain gage. Creek flow is up significantly. • GC has installed additional erosion control measures, including straw wattles on large slope, additional filter fences, straw and grass seed. Grass seed on the wall ledges and below the wall is beginning to sprout. • A total of 24 MSE wall layers (48 vertical feet) have been installed, 4 vertical feet remain to be constructed. Each soil layer is 2'thick separated with wire mat, 26 total layers=52'vertical. • Maureen and David (GC) marked the sawcut location for the 24"x24" hole for access into the concrete flume at the gate house. GC was directed to provide a 12" concrete riser and 6"frame and round locking lid on the opening. This will set the top of structure approx. 4" above the overflow weir elevation of the gate house. • Localized surface erosion of the 2:1 slope occurred during recent rains on the left side just below the mid-slope access road. Displaced wattles can be seen in the photo. Affected area is approx. 50'wide at road and extends most of the way down to the top of wall landing. Approx. 10 cy of material was washed to the base of the slope. The cause of erosion appears to be seepage, and flowing water was observed hours after the rain stopped. It is also the approximate location of suspected seeps in the past.This was confirmed by Joy Endicott,who was walking the site late in the day. She said seepage often occurred in this area after heavy rains. • GC has been directed to install excavate the area 2' deep and quarry spalls. Frank Pita has indicated filter fabric is not necessary under the spalls. • Some minor downward settlement of some of the wall face was observed in three of the uppermost four foot high wall-steps. See photo. Contractor indicated this occurred due to surface water flowing over front face of the wall and fines being washed out of face. Testing with steel probe of some of the wall steps also indicated the material at the face is not densely compacted, and this may have contributed to some settlement. JA needs to modify the current grading plan to direct surface water away from the wall face. • No water was observed flowing from 2 underdrain pipes. Water flow was observed approx. 10 vertical feet below the pipe and downslope. It appears water is flowing along the bottom of the drain rock trench.This is not a problem. • Quarry spall ditch was installed along the access road leading to Winter Creek. • The MSE wall mat at elevation 303 was ended about 5' short of the rock face, due to lack of enough mats. I told David that he needs to ask owner as it would be an additional cost. • When I arrived onsite in the morning the last MSE mat and top mat had been placed, elev. 311. The 12" minimum cover of fill material hadn't been placed over the mats yet. A total of 26 MSE wall layers (52 vertical feet) have been installed.Towards the end of the day before I left the site a minimum of 12" of cover had been placed over the top mat. The material had been compacted and appeared to be non-yielding,testing of the top lift scheduled for Wednesday. • The seepage area noted in Field Report#20 was reinspected. No seeping water was observed. Rills and sediment deposits were still noticeable in the erosion area. When I arrived at the site, David has staked the limits of erosion. The staking started about 60' east of the'Y' in the access road and ends 130' east of the Y. The top of the eroded area is 70' wide. The erosion tapers down the slope to a width of 30'. David had staked the bottom of the eroded area. The limits that were staked appear to be reasonable to me. After David and I agreed that seeps will continue to affect this area, he asked if they could blade off the top 2' and replace the material with quarry spalls; David anticipated 350 tons of spalls would be needed. I told him this was fine.The excavated material should be used if additional cover is needed for the MSE wall. • The seep/eroded area was bladed down the slope to a depth of 2'. The dozer operator kept within the staking limits and moved to material down to the MSE wall for additional cover. In areas adjacent the road hard pan was noticed. The loose material sitting on the hardpan was observed to be soupy and showed evidence of concentrated moisture. • All the48" pipe with the exception of one bend (by the gate house) was located on site. The remaining bend is on order and would be shipped to the site by next week. • David expects pipe installation to begin on October 101h • The sawcutting contractor was onsite when I arrived. The end of the flume by the gate house had been cut flush in preparation for the gatehouse transition structure. The screen house structure was also sawcut in preparation for the screening facility structure. Lastly, the top of the intake structure was cut to provide a 24"x 24" access opening so crews can cleanout the sediment buildup. • The sawcut opening at the gate house was measured to see if the proposed structure would fit. Excavated area is approx. 65' wide at top, along the access road,and 15'wide at the bottom of the slope • JA is developing a grading plan for final grading of top of wall, and interception of drainage from quarry spall chute. • Wattle installation and other erosion control measures are ongoing. • Access road has 2" crushed surfacing, additional 2" to be added later. Roadside ditch is graded and is culvert installed. Check dams not yet installed. • Water bars should be added to access roads throughout site. Bar openings will be 4", and debris collected at downstream screening reservoir. We expressed concern about debris getting lodged within grate and throughout system, including in the tunnel. Other modifications may be required. • Based on recent survey, final MSE wall location is further south that original plan, by up to 10'. This is not a concern, and provides additional space for maintenance access. • The area beyond the top of slope(staging area)will need to be final graded to direct water away from the edge of top edge of the slope. Welding is expected to take much longer than estimated and GC is looking for advice on ways to speed it up. Specific issues: 1) are 6 passes required to fully weld top frame to top plate?2) how to fill gap in SE vertical corner of reservoir, 3) do bolt tensioners need to be fillet welded since their work is done, and 4) are welds required on both sides of top frame? • First stick of 48" pipe was inserted in reservoir wall through the Cast-a-Seal rubber gasket. This fit was snug (good), but steel band clamps are still required, and were not provided. We are working to locate the bands. • One 4" concrete riser and one 6" steel manhole frame & grate were observed by the gate house.The concrete riser needs to be 12"so the top is above the ordinary expected water level. • Approx. 60 If of Sanitite HP pipe was installed, through the first bend. Pipe is backfilled approximately half way. Jumping jack being used alongside pipe for compaction. Bedding is imported gravelly sand. • A 2' x 2' trench was been excavated alongside the penstock last week for the electrical service installation to the screening reservoir. • Staff from Sound Native plants is planting the area below the MSE wall. Ron Gold is assisting. • Stemar Inc. equipment is onsite for air testing of pipe joints. • Bonded fiber matrix hydroseed has been applied to all upslope areas. • We provided a cross section to David for the connection of the quarry spalls to a gravel drain trench in the top of the wall area. JA needs to check with Hilfiker wall on whether top wire mat can be cut shorter to accommodate the trench. GC is using a Stemar Inc., Lansas No Void Joint Tester. • Current pipe slope of 0.12% was verified with GC pipe layers. GC has indicated they did not receive plugs from ADS. • 48" pipe and bedding is being stockpiled on top of the MSE wall area. • Sound Native Plant staff has nearly completed installation of 1 gal potted plants and live stakes (willow) in area below wall. They have installed additional jute matting on the steeper(eastern) portion of the slope,where it is about 1H:1V.JA provided them an area estimate of the upslope areas(50,000 sf). • Maintenance and repairs of the intake area is underway. It appears water is also flowing through the gravel backfill that extends under the wall. • The french drain trench on top of the wall area has been extended east than shown on the plan. This does not appear to be an issue. • A swale across the staging area directs runoff to the east,away from the top of wall. The verbal instructions included the following- 1.The connections shown in detail A of figure 7 were omitted at all locations. 2.At locations which exhibit an excessive gap between steel members to be welded it was o.k.to use standard re-steel as a spacer or to fill the space with weld metal(see AWS D1.1-2010,subsection 5.22.1). 3.The continuous flare-bevel groove weld shown on detail I of figure 4 was changed to an intermittent fillet weld on both sides with a spacing of 4"every 12". 4.The various weld quality requirements of clause 5&clause 6 in AWS DI.1-2010 was lowered. The excessive gaps existed at a few of the wall to wall welded connections at the comers.These connections was shown on detail I on figure 2.At one location(the SE comer)as shown on figure 2 was a#5 piece of reinforcing steel used as a spacer for the entire height of the structure. Need Engineer of Record(E.O.R.)acceptance of in-field conditions. The observed work was found to have been performed as shown in the contract drawings dated 2-OCT-2014&the above mentioned verbal instructions. There are 4—2.5"dia.steel posts in the center of the flume. The opening at the head of the flume (closed by steel gate) is 24" high x 86" wide.There are 2 vertical rectangular steel supports in the opening. See photos. (No water was leaking from through the steel gate since the water elevation in the gate house was low.) • I inspected the interior of the remainder of the 48" pipe at the upstream end (the portion not previously inspected). All 48"pipe joints have 1"or less deflection. • The grouting between the steel transition structure and the existing concrete flume is well done, and provides a smooth connection between the surfaces. The 48" HP Sanitite pipe overlaps the steel transition structure approx. 16"where the steel structure slips inside the 48" pipe.The gap between the steel structure and 48" pipe is approx. 1.5". A Fernco coupling overlaps the joint, per plan details. It does not appear that the steel structure was coated with coal tar,per details. • A 67" long section of 48" HP Sanitite pipe was cut out and replaced with a new section where pipe was damaged by an excavator backing into it. Location was approx. Sta. 2+65. Repair was made with new pipe section, cut to fit,and two Mission Flex-Seal MR72 Series Repair Couplings. The new joints have a maximum gap of approx. 1/2".David Schneller (GC) stated the vent plugs were installed in all the cut ends of the HP Sanitite pipe,per manufacturer's directions. • There are some small holes in the concrete wall of the gate house, where it connects to the concrete flume,which should be grouted tight(they leak when gate house is full of water). • I walked approx. halfway through the 200' long tunnel from the gate house. The tunnel enters the gate house at a 45 deg angle, but otherwise is a straight shot between the angle point and the intake box. The tunnel has a formed concrete wall and ceiling extending approx. 10' upstream of the gate house, but otherwise is rough basalt rock. It transitions from 6'wide x 8' high at the gate house to approx. 5'w x 4'h at about the midpoint. Measurements at the intake indicate the tunnel is also approx. 5'w x 4 h at the west(intake)end. • The floor of the tunnel and bottom of the gate house appear to be on rock, and are not poured concrete. The gate house invert is approximately 3'below the invert of the steel flume gate.The invert of the tunnel appears to have a reasonable slope and was covered with gravel and cobbles. • A 12-inch ADS smooth wall corrugated pipe was installed downslope of the new access road, past the east end of the wall.The upstream pipe end is covered with quarry spalls at the ecology block wall.This pipe is intended to drain the access road area. • An area about 10' wide had eroded along the left (west) side of the upslope.Jute matting and seed has been installed as recommended. • Installation of control equipment at screening reservoir, gate house and intake area are ongoing. • One of the two 12-inch pipes from the underdrain system continues to flow water. Cromwell equipment operators were working on the installation of SSMH No. 3 and completing the pipe installation from SSMH No. 4. It was observed that pea gravel bedding was being used as called out in the contract documents. Footing drains were being installed on the West side of the building. The South sediment pond side slopes are showing some erosion. These need to be kept stabilized.
