Helical pile leads and extensions were verified as 5.0 ft long and 2.5 inches in diameter, consisting of 2.875 inch diameter helical piles manufactured by Ideal Manufacturing,Inc. Operations were continued from the previous day (MTC report #F312340) Upon arrival, MTC checked in with the contractor (Foundation Specialist) and reviewed the available plan set, which directed the installation of 2.875" diameter helical piles at pre- selected locations. Piles were driven using a Dingo wide-track TX-1000 loader with the appropriately sized pile driving attachment and a gauge capable of reading 3000 psi with a resolution of 50 psi. MTC observed most piles driven to the minimum installation torque (i.e. refusal) in the selected locations, with the exceptions as noted below in red. Specified refusal criteria varied depending on the location of the pile, per the provided plan set. See Pile Installation log below for final installation depths of all piles. On July 7,2023,seventeen(17)helical piles were installed by R&R Foundation Specialists(R&R)to lift and stabilize the residence located at 23430 Highway 101. Piles were installed with a DIGGA 10k drive head to a maximum depth of 10ft.To the knowledge of L120,these piles were installed by R&R certified installers in accordance with the cited structural package and ESR-37S0. Please refer to the attached drive log provided by R&R. Concrete samples taken have been reviewed and deemed to have reached acceptable strength prior to 28 days. New foundation elements appear to have been installed according to the drawings. With pile spacing at a maximum 6'-0"on-center,the minimum required vertical capacity as determined by L120 was allowable loadings of 15 kips per pile.These demand requirements are as determined and specified in L120's structural calculation package cited above. As required by the referenced structural package, proof testing was to be performed to 200 percent of the specified design load. It is L120's understanding that the installers utilized the dead weight of the existing structure to achieve the required reaction of a test pile. Pile capacities for helical piles have been calculated utilizing the torque correlation factors provided in ESR-3750.A torque/capacity chart detailing this calculation has been attached to this letter. Per the attached document,an Ideal Foundation Systems 2-7/8in diameter 10k helical pile installed to minimum pressures of 1,000psi results in an installation torque of 3,480 ft-lbs,and an allowable capacity of 15.7 kips including a safety factor of 2. Upon arrival, MTC checked in with the contractor (Foundation Specialist) and reviewed the available plan set, which directed the installation of 2.875" diameter helical piles at pre- selected locations. Piles were driven using a Dingo wide-track TX-1000 loader with the appropriately sized pile driving attachment and a gauge capable of reading 3000 psi with a resolution of 50 psi. We have been asked to verify that these updates/changes are approved and sufficient as built. • Please verify the use of a single helix 2 7H8" diameter helical pile in lieu of the double helix pile specified in detail 4/SD-1.2. • Per the Level Survey,the existing foundation was anticipated to be CMU, which would require an Angle Iron to span between the piles to support the stemwall and footing. However, at the site, it was discovered that the foundation wall was in fact concrete, but too shallow to incorporate an angle iron support. Therefore it was requested that 4x4 tube steel be used to span between the piles in lieu of the angle iron at the locations indicated on plan. • Per the structural drawings, a new continuous strip footing with enlarged and deepened pile caps have been specified from piles #4-11. In lieu of providing varying forming for deepened pile cap sections, R&R has requested to just form and pour the pile cap section as a full length grade beam in lieu of the continuous footing, thus providing widened footing, with increased strength/capacity. Solution: Based on the information provided to us from R&R Foundations from the site conditions, we recommend the following. L120 is in agreement with the requests and the alternate solutions listed above for the on site field adjustments. Please see below for further information. It shall be noted that this foundation repair is for stabilization only. • Single helix 2 7/8" diameter helical piles are acceptable to be used. On July 7,2023,seventeen(17)helical piles were installed by R&R Foundation Specialists(R&R)to lift and stabilize the residence located at 23430 Highway 101. Piles were installed with a DIGGA 10k drive head to a maximum depth of 10ft.To the knowledge of L120,these piles were installed by R&R certified installers in accordance with the cited structural package and ESR-3750. Please refer to the attached drive log provided by R&R. Concrete samples taken have been reviewed and deemed to have reached acceptable strength prior to 28 days. New foundation elements appear to have been installed according to the drawings. With pile spacing at a maximum 6'-0"on-center,the minimum required vertical capacity as determined by L120 was allowable loadings of 15 kips per pile.These demand requirements are as determined and specified in L120's structural calculation package cited above. As required by the referenced structural package,proof testing was to be performed to 200 percent of the specified design load. It is L120's understanding that the installers utilized the dead weight of the existing structure to achieve the required reaction of a test pile. Pile capacities for helical piles have been calculated utilizing the torque correlation factors provided in ESR-3750.A torque/capacity chart detailing this calculation has been attached to this letter. Per the attached document,an Ideal Foundation Systems 2-7/8in diameter 10k helical pile installed to minimum pressures of 1,000psi results in an installation torque of 3,480 ft-lbs,and an allowable capacity of 15.7 kips including a safety factor of 2. The results are summarized below: Maximum Demand(allowable)=15.0 kips/pile Available Capacity(allowable)=15.7 kips/pile 2 of 6 By inspection,the pile capacities of the installed helical piles exceed the maximum loading demand or reached refusal as determined by L120's analysis.Therefore,the installed piles comply with the structural design/demand. MTC inspector on site,as requested,for visual inspection of reinforcing steel(RS)into the stem-wall at lower rear of the house.The RS was observed to be of proper size,grade,lap,spacing,location,clearance and secured prior to concrete placement other than the 1 item noted below. 1.Reinforcing steel in corners have corner bars per detail 3 and General Notes in"Reinforcing Steel/Note 3" .Contractor spliced 2 bars(top and bottom)at corners without any additional corner bars. CONCRETE PLACEMENT:Continuous Inspection The inspector also observed the placement of approximately 5 CY of concrete with Mix Design ID#0360A40 supplied by Miles into to at the location noted above. One set of five(4"x 8")cylinders was cast for compressive strength test specimens.Refer to Report#C312977 for field test results and for more information. Concrete was placed via Brundage Bone concrete pump truck with tremmie,was mechanically consolidated with-cycle vibrator,and finished by hand. Noteworthy Inspection Item: Concrete was sampled approximately 3 hours after the batch time printed on ticket.This needs EOR's review and approval. To the best of MTC inspector's knowledge,the above-described work was performed in general accordance with project specifications and approved plans with the exception of the noteworthy inspection item described above.
