SEE IMPORTANT GENERAL PROVISIONS ON REVERSE SIDE OF APPROVAL NOTE: This Hydraulic Project ,Approval (HPA) is for replacement of an existing dock. Plans inaicate the new dock shall be no larger or longerhan the existing dock. New pilings will be added to replace existing pilings . 1 . Work shall be accomplished per plans submitted to the Washington Department of Fish and Wildlife (WDFW) , except as modified by this HPA. These plans reflect design criteria per chapter 220-110 WAC. A copy of , these plans shall be available on site during construction. 2 . This HPA is for repair/replacement of the existing structure or piling only and shall not result in expansion of the structure . P g Y P 3 . Excavation for and placement of the footin s and foundation shall be landward of the ordinary high water line (9HWL) . 4 . Removal of existing structures shall be accomplished so the structure and associated material does not reenter the lake . 5 . All treated wood shall be professionallyy treated and completely cured prior to installation below the OHWL to minimize leaching into the water or substrate . The use of wood treated with creosote or pentachlorophenol is not authorized. 6 . Skirting or other structures shall not be constructed around the pier, dock, or float . No floatation material is authorized. 8 . All work operations shall be conducted in a manner that causes little or no siltation to adjacent areas . 9 . Removal of aquatic vegetation shall be limited to that necessary to gain access to construct the project . 10 . If at any time, as a result of project activities, fish are observed in distress, a fish kill occurs, or water quality problems (includingg equi ment leaks or spills) , o erations shall cease and WDFW at (360) 753-2600 and DOE at (360) &07-6300 shall be contacted immediately. Work shall not resume until further approval is given by WDFW.
