Gentlemen:
A representative of this department has reviewed your request for a
renewal of the referenced Hydraulics Project Approval , and the Time
Limitations shall be changed to:
Immediately - December 31, 1989
All other provisions of the original approval and amendments shall
remain as written, and this letter shall be attached and on the job
site at all times.

THIS APPROVAL IS TO BE AVAR ABLE ON THE JOB SITE AT ALL TIMES AND ITS PROVISIONS FOL OWED BY THE PERMIT-
TEE AND OPERATOR PERFORMING THE WORK.
The person(s) to whom this approval is issued may be held liable for any loss or damage to fish life or habitat which results from
failure to comply with the provisions of this approval.
Failure to comply with the provisions of this approval is a gross misdemeanor, possibly punishable by fine and/or imprisonment.
The Departments reserve the right subject to the holders opportunity to a hearing to contest agency actions as provided by the Ad-
ministrative Procedure Act, chapter 34.04 RCW, to make additional restrictions or conditions or revoke the approval when new informa-
tion shows such action is necessary by the departments for the protection of fish lite.
These departments cannot be held liable for any property damage which might occur as a result of this project, except where
damages are proximately caused by actions of the departments. r
This approval pertains only to the provisions of the Fisheries and Game Codes. Additional authorization from other public agencies
may be necessary for this project.
1. There is to be no destruction, removal , uprooting of any aquatic vegetation
(lilies, pondweeds, etc. ) as a result of movement of heavy equipment into
the lake, into the work site, between work sites, or out of the lake.
2. The only permissible removal of aquatic vegetation shall be that incidental
to the physical movement of the piles into the substrate.
3. Where dock construction is for a new, previously non-existing dccF , crwhE_X(i
z dock is to be replaced and moved to a new location, the siting of the dock
shall be coordinated with the property owner such that the incidental removal
of shoreline vegetation is minimized or prevented.
4. Any debris resulting from this construction project shall be removed from
the water and disposed of or placed in such a manner to prevent its being
washed back into the water by high water or wave action.
5. There is to be absolutely no removal of logs or large woody debris from
the lake bottom without the express approval of a representative of the
Department of Game (telephone: Mark Grandstaff, 357-4120) on a case by
case basis.

PROVISIONS:
6. In the event the material used is treated with penta, creosote, or other
preservatives, it shall be completely dry before use in or near the water
7. Extreme care shall be taken to assure that no petroleum products, fresh
cement, concrete or other deleterious material is allowed to fall , be wasted
into or otherwise enter surface waters.
8. This approval covers the repair and replacement of existing docks. A repre-
sentative of the Department of Game shall be notified for a field review
prior to the installation of any new docks.
SEPA: Exempt
NOTE: This approval relates to protection of aquatic habitat only. Where
equipment ingress and egress requires use of developed State access
areas, a separate approval shall be negotiated with the Department
of Game's Olympia office.
