Permittee: John Rosendahl Contractor: Advance Environmental Location: 5660 Grapeview Loop LIC: ADVANE1972MH Tank Description: Conditions/Decommissioning Procedure: 1) Expose the top of the tank enough to allow the tank to be purged and filled as identified in later steps. 2) Remove the fill (drop) tube, gauge and product lines. Cap or plug all but the fill and vent lines. • 3) Remove all product in the tank by pumping out. Retain in approved containers to be disposed of in accordance with State Law including any residual water (water may not be discharged to the ground, waterways or sewer). 4) Fill the tank with water until liquid can be seen at the fill opening and stop the filling, taking care to avoid the release of any water into the soil. 5) With water still present in the tank, cut a hole in the tank large enough to fill later with an approved material (see step 8). 6) Pump out the water and retain in accordance with step 3. 7) Wipe out any remaining liquid or built-up residual sludge. 8) Fill the tank with a suitable solid inert material. Options include free flowing sand, concrete slurry (also known as control density fill), or a dirt-sand mix. Material must completely fill the tank eliminating any voids. An inspection of the tank by this office must occur before proceeding. 9) Once filling is complete, remove and cap/plug the vent & fill lines and any outside plumbing. Secure the tank opening(s) and backfill the excavated area to grade and compact. If tank is in the crawl space of the residence, care shall be taken to avoid disturbing the foundation. If excessive disturbance of the foundation is observed, an engineering analysis may be required. 10) Within 30 days after the tank closure is complete, the tank decommissioner shall complete a Permanent Closure Notice and submit to the Department of Ecology The closure activities identified in step 8 above shall be witnessed by a member of this office. A minimum 48 hours' notice (business days) are required prior to the date of the inspection request. Such properties do not include dormitories, convents, mobile parks, apartments, hotels and similar facilities, unless the UST system is used by the owner solely for his or her own personal use, rather than to maintain the overall UNDERGROUND STORAGE TANK ABANDONMENT INSTRUCTIONS: 1. Drain and flush the piping into the tank. 2. Remove all liquids that can be pumped out. It may be necessary to use a hand pump to remove the bottom few inches of product. Contaminated water may not be discharged to the ground or sewers and must be retained. 3. Dig down to the top of the tank. 4. Remove the fill (drop) tube. Disconnect the fill, gauge, and product lines. Cap or plug open ends of lines, which are not to be used further. The vent line is to remain connected until the tank is filled as outlined in step 7. 5. Fill the tank to overflowing with water to purge off all products. As the level of liquid rises, the remaining product should flow to the top of the water. When the floating product reaches the fill opening, suspend the filling, remove the floating product and place it in a suitable container for disposal. 6. After the water has overflowed the tank, cut one or more large holes in the tank top. This can be accomplished with several blows from a backhoe. Pump out the water and dispose of it in a suitable manner. 7. Install a suitable solid, inert material through the top of the tank. Options include: concrete slurry, dirt and sand mix, of free flowing sand. 8. Disconnect and cap the vent line. 9. Once the process is achieved, complete the UST removal checklist, and return to the Mason County Permit Assistance Center. If there is evidence of product in the ground contact the Department of Ecology at (360) 407-6264 for further guidance.
