Subject: Documents required for Griffith permits
Thank you for responding to Mr. Griffith to let him know what documents our office will require
with the new permit applications needed to perform a building department plan review in accordance
with the hearing examiner decision dated May 22,2012, Additional documents may be required as
determined by the 3rd party review agent.
1) Completed building permit and land modification permit applications.
2) Construction plans for the land modification and retaining wall system demonstrating compliance
to Mason County Code, Chapter 14.
3) Supporting design calculations for land modification work and the retaining wall system
4) Geotechnical report along with written confirmation that the geotechnical engineer has reviewed
land modification and construction documents and finds them to be consistent with the geotechnical
report.
5) Survey documents along with written confirmation that the survey professional has reviewed
land modification and construction documents and finds them to be consistent with the survey.
All document shall be sealed and signed by the design professional who prepared them. When the
documents are submitted Mason County will collect plan review fees and a deposit to cover the
estimated costs for peer review.

Hello Allan and Barbara,
As a result of the plans submitted by Allan Tahja on behalf of Mr. Griffith on July 25, 2014 I have reviewed the Hearing Examiner
findings of the Administrative Appeal, HEX 2009-00035, dated May 22, 2012. The referenced document is attached to this email.
The hearing examiner found that the building and grading permits were denied and the applicant was instructed to re-apply.
I suspect that Mr. Griffith submitted the plans on 7.25.2014 without a building permit application so that we would begin the
process to obtain bids for a third party peer review. I have some questions and thoughts....
Barb....can we discuss 1 & 2 for immediate consideration?
1) Can we discuss the process for obtaining the 3rd party peer review bids?
2) The documents did not come in with a building permit application. Based upon my review of the HEX findings a new permit is
required for both permits.This belief is consistent with the email prepared by A Borden in June 1, 2012. See document attached.
3)The plans submitted are similar to the original plans submitted with revision changes dated 7.22.2014. See document
attached. In regards to the plans submitted I have the following comments/thoughts:
A) The second to the last page of the plan documents submitted specifies total yardage is 976 CYI. The calculation needs to
include cuts and fill. Based upon the information provided it appears that material quantities may be adjusted and may not
represent material quantities that are cut and then moved to another location. Need to verify that the calculation was prepared
correctly.
B) Comments in the hearing examiner findings include concerns that the design did not align with the topo survey prepared by
Agate Survey. I believe that the surveyor should review the plans and verify that the design is consistent with the survey in order
to verify heights and excavation work.
C) I cannot find specific information that identifies height of the retaining walls. Plans should identify height and length in
relation to the specific location on the plan.
D) Based upon discussions on Friday when the plans were received the applicant will need to submit a narrative that identifies the
scope of work including specific excavation and fill information, ht and length of the retaining wall system, etc.
E) The third party review will perform work to verify that the retaining wall system is designed in accordance with the 2012
International Building Code relating to calculations and final design. In addition the third party will review to verify that
quantities specified and grading and excavation is performed in accordance with Mason County Grading and Excavation code
14.44 and clearly represented on the plans.
F)The survey prepared by Agate Land Survey was not included but requested by Allan Borden in the email dated 6.1.2012.

Hello Mr. Griffith,
On July 25th Allan Tahja dropped off plans related to your project with permit numbers BLD2007-00906 and GRD2007-
00012 written on a note. The plans were delivered at approx. 3pm.
Based upon the hearing examiner findings, case number HEX2009-00035, the old permits were denied and included
information if you choose to re-apply. Permit applications for land modification activities and construction of the retaining wall
were not included when the plans were received. Please complete and submit a land modification permit and building permit
application as soon as possible. New permit numbers will be assigned. This is consistent with the information provided by Allan
Borden in the email dated 6.1.2012. See document attached. Based upon a conversation with the Department of Community
Development Director, Mrs.Adkins, permit fees that were paid under the 2007 land modification and building permits may be
attributed to the new building permits since the fees paid were never used to perform the work.The total permit fee paid for the
land modification permit (GRD2007-00012) is $301.76. The fee paid for construction of the retaining wall (BLD2007-00906) is
$411.45.
In addition to requiring separate permit applications our office will need the following information:
A) Please provide a written document that identifies the scope of work. Include specific excavation and fill information, height,
and length of the retaining wall system,work schedule, etc.
B)The second to the last page of the plan documents submitted specifies total yardage is 976 CY. The calculation needs to
include cuts and fill. Based upon the information provided it appears that material quantities may be adjusted and may not
represent material quantities that are cut and then moved to another location. Please verify that the calculation includes total CY
for cuts and total CY for fill.
C) Comments in the hearing examiner findings include concerns that the design did not align with the topo survey prepared by
Agate Survey. The surveyor should review the plans and verify that the design is consistent with the survey in order to verify
heights and excavation work. Please provide a written statement from Agate Land Survey acknowledging that they have
reviewed the plan documents and agree with the design conditions. In addition provide a copy of the survey prepared by Agate
Land Survey was not included but requested by Allan Borden in the email dated 6.1.2012. See email attached to this email.
D) Please identify the height of retaining walls. Plans should identify maximum height and maximum length in relation to the
specific location on the plan.
E) The third party review will perform work to verify that the retaining wall system is designed in accordance with the 2012
International Building Code relating to calculations and final design. In addition the third parry will review to verify that
quantities specified and grading and excavation is performed in accordance with Mason County Grading and Excavation code
14.44 and clearly represented on the plans.
In order to proceed with a third party plan review a deposit of $1500 shall be required. Please submit the deposit with requested
documents.

We will complete a completeness review next week and come to the meeting with a list of items that are
needed to complete the submittal package.

GRADING PLAN ADDENDUM
As indicated in the attached email correspondence from Mason County Department of
Community Development dated 7/29/14 prepared by Debbra Coker, additional
information is requested pertaining to Mason County case no. GRD2007-00012 and
plans received by Mason County on July 25`h, 2014. Mrs. Coker requested the following
information;
A) Please provide a written document that identifies the scope of work. Include specific
excavation and fill information, height, and length of the retaining wall system, work schedule,
etc.
Mr. Griffith wishes to install a residential driveway to access his property located on the
ease shore of Hood Canal at the south end of Dewatto Beach Drive South, a Mason
County public road. The County road ends at the south end of the Griffith property. Mr.
Griffith's property measures approximately 125 feet along the Hood Canal Shoreline. Mr.
Griffith owns both the tidelands and the uplands. The upland(easterly end of the
property) side of the property includes a near vertical bank which rises from the road
elevation of roughly 14.00 sea level to an elevation of approximately 50 feet above sea
level where the slope continues to climb at slopes measuring—20% over a distance of
roughly 80 feet where the slope increases to as much as 45% near the back (south end) of
the property. Stations used to develop cross sections for excavation and embankment
quantities have been developed into a plan sheet format and are appended to this
document. The cross section sheets include wall heights at each cross section as well as
the cross section areas of cut and fill areas. The driveway length shown in the plans is
165 feet where the driveway essentially daylights out. The driveway is designed to be
supported by an ecology block retaining wall initiating at the west end of the property
and climbing to the east at a design grade of 32%. The ecology block wall begins at
approximate plan station 0+45 with a wall height of roughly 11-feet including a concrete
footing-foundation beneath the ecology blocks. At the westerly end of the property, the
ecology block wall is planned to be constructed to a height of approximately 25-feet
where the wall turns into the bank and the wall and footings are installed in steps.

Geosynthetic fabric reinforcement has been designed into the plans and a drainage
envelope is to be installed behind the ecology block wall sections. The grade finished
driveway will be finished with a 4"+thickness Portland Cement Concrete driving surface
crowned 3" in the middle with 6-inch curbs installed at both outboard sides of the
driveway. The driveway finish is specified as "ROUGH"and is planned to be a"rodded"
finish. Stormwater received by the driveway surface will be conveyed to a catch basin
and drain pipe specified in the plans for discharge to a Hood Canal dispersion area.
Mr. Griffith will be in control of planning a work schedule, but main issues will be
avoiding bird nesting periods until the end of August. Dry weather would be favorable,
but with adequate Erosion and Sediment Control activity, the project could be planned at
Mr. Griffith and his contractor's convenience.

appears that material quantities may be adjusted and may not represent material quantities that
are cut and then moved to another location. Please verify that the calculation includes total CY

Cross section sheets have been prepared from the survey information provided by
AGATE LAND SURVEYS. In the process of developing the cross section sheets for
printing minor editing occurred and the quantity claimed is now 967 cubic yards
combined total cut& fill. The method used to calculate the volumes is the average end
area method, the same method used in the original quantity calculations.
C) Comments in the hearing examiner findings include concerns that the design did not align with
the topo survey prepared by Agate Survey. The surveyor should review the plans and verify that
the design is consistent with the survey in order to verify heights and excavation work. Please
provide a written statement from Agate Land Survey acknowledging that they have reviewed the
plan documents and agree with the design conditions. In addition provide a copy of the survey
prepared by Agate Land Survey was not included but requested by Allan Borden in the email
dated 6.1.2012. See email attached to this email.
I have met with Mr. Sidney Bechtold, P.L.S., the licensed surveyor for AGATE LAND
SURVEYS, PLLC to discuss the Griffith project and expect Mr. Bechtold will prepare
the response requested.
D) Please identify the height of retaining walls. Plans should identify maximum height and
maximum length in relation to the specific location on the plan.
Plan sheets have been developed for each station identified in the project plans which
include the wall heights at each of the stations.
E) The third party review will perform work to verify that the retaining wall system is designed in
accordance with the 2012 International Building Code relating to calculations and final design. In
addition the third party will review to verify that quantities specified and grading and
