X 12) All Material Safety Data Sheets (MSDS) for all hazardous materials used within the facility shall bbe jallaced in a notebook and permanently located in a conspicuous location inside the building near the main entrance. (IFC Chapter 50)X rr� 13) Lever hardware is required at doors. The unlatching of any door shall not require more than ONE operation. Hardware with locks must open with a single action from the egress side of the door. Door hardware shall allow egress doors to be readily open able from the egress side without the use of a key or special knowledge or effort. Handles, pulls, latches, locks and other operable parts on accessible doors shall have a shape that is easy to grasp with one hand and does not require tight-grasping, pinching, or twisting of the wrist to operate. X & 14) All doors shall be pivoted or sides hinged and swing in the direction of exit travel. Means of egress doors shall be readily distinguishable from the adjacent construction and finishes such that the doors are easily recognizable as doors. Egress doors shall be side- i ed unless specifically exempted in the Section 1010.1.2.1 & 10.10.1.2 of the International Building Code& International Fire Code. X &Z 15) ALL exits and exit access doors shall be marked by an approved exit sign readily visible from any direction of egress travel. All exit signs shall be internally ternally illuminated at all times and equipped with to ensure continued illumination for duration of at least 90-minutes. X 16) In an Assembly Occupancy group with less than 300 occupant load, occupancy groups B, F, M, and S you may only use a lock that prevents operation of the door at the MAIN door ONLY. X //�'k1J 18) In addition to the inspections required in IBC, Section 109, the owner, the engineer or architect of record acting as the owner's agent shall employ one or more special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by the design professional. The special inspectors duties & responsibilities shall be as specified in Chapter 17. Special inspection reports shall be submitted to the Mason County Building Department, 615 W Alder St, Shelton WA 98584 and available for inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and final occ�y inspections. X 19) Any changes in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are removed, approval will not be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and collected ason County Building Department prior to any further inspections being performed or approvals granted. b t X J COM2018-00037 Page 6 of 13 20) ALL CONSTRUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF JUSE OR OCCUPANCY WOULD RESULT IN PERMIT REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x 21) Changes to approved building plans that affect compliance to the current Washington State Energy Code (WSEC), ventilation requirements), Building/PI F ing/Mechanical Codes and/or Mason County Regulations shall be approved prior to construction. 22) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason Coun X � ilding Inspector shall be made prior to requesting additional inspections. 23) All property lines shall be clearly identified at the time of foundation inspection. X /r'F/ 24) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant wit pl ason County ordinances and building regulations. X 25) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit r have prevented action from being taken. No more than one extension may be granted. X 26) The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical report or assessment. Structures and/or land modifications (grading, cuts, fills, etc.) required in the geotechnical report/ass ssment, may require a seperate permit. The geotechincal report/assessment shall remain attached to the approved building plans. X 610 2 An inspection performed by a representative of the geologist, their authorized representative, or engineer of record, shall be completed on-site prior to the footing pour to verify that earthwork and foundation installation activities comply with the Geotechnical report or assessment recommendations. In addition a final inspection report, prepared by a representative of the geologist, their authorized representative, or engineer of record shall be required to verify that erosion control, drainage and final work is completed in accordance to the Geotechnical report or assessment recommendations. Reports shall be submitted to the Mason County Building Dept., PO Box 186, Shelton, WA 98584, prior to the footing and final inspections and X available inspection. 28) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fastener nnectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. X 29) Recyclable materials & Solid Waste Storage: Space shall be provided for the storage of recycled materials and solid waste. The storage area shall be designed , eet the needs of the occupancy, efficiency of pick-up, and shall be available to occupants and haulers.X COM2018-00037 Page 7 of 13 30) Prior to obtaining final occupancy approval the registered design professional shall provide evidence of systems commissioning and completion in accordance with the 2012 International Energy Conservation/Washington State Energy Code (IECC/WSEC) Section C408. ` Copies of all documentation shall be given to Mason County Department of Community Development prior requesting the final occupancy inspection and shall include commissioning of the building mech cal system (C403), service water heating systems (C404), electrical power and lighting systems (C405), and energy metering (C409). X �J 31) By definition, propane tanks and heatpumps are structures, which must meet setback conditions. Please check your"Approved Site Plan"to ensure these structures meet the setback conditions listed. X _ 32) Landings and stairs must meet the same setback conditions as any permitted structure; and, must be shown on your site plan. Please check your "Approved:Plan"to ensure these structures are shown and meet the setback conditions listed. X 33) ALL SURFACE WATER AND POTENTIAL RUNOFF WILL BE CONTROLLED ON SITE AND SHALL NOT ADVERSLY AFFECT ANY ADJACENT PROPERTIES NOR INCREASE THE VELOCITY FLOW ENTERING OR ABUTTING TO ANY STATE OR COUNTY CULVERTING/DITCHING SYSTEM OR ROAD WAY. X , 34) WINDOWS AND GLASS DOOR INSPECTION Windows and glass doors shall be installed and flashed in accordance with the manufacturer's written installation instructions. Written instructions shall be provided by the manufacturer for each window and available for inspection. In addition all exterior windows and doors shall be designed to resist the design wind loads of the project. X �' 35) Accessible parking spaces shall be provided in accordance the 2015 International Building Code Section 1106 and constructed in accordance with Section 1101.2/ICC A117.1. Accessible parking spaces shall be provided in accordance with Table 1106.1 as follows: 1 to 25 total parking spaces requires not less than one accessible parking space 26 to 50 total parking spaces requires not less than two accessible parking spaces 51 to 75 total parking spaces requires not less than three accessible parking spaces 76 to 100 total parking spaces requires not less than four accessible parking spaces See IBC Section 1106 and table for more than 100 parking spaces. Construction and design shall be completed in accordance with ICC A117.1, Section 502. Accessible parking spaces shall be located on the shortest accessible route of travel from adjacent parking to an accessible building entrance. Car parking shall be not less than 96-inches wide and access aisles not less than 60-inches wide shall be properly marked. Parking surfaces shall be firm, stable, and slip resistant material and shall not be steeper than 1:48. Accessible parking spaces shall be clearly identified by signs containing the International Symbol of Accessibility placed not less than 60-inches above ground surface, measured from the bottom of the sign. X COM2018-00037 Page 8 of 13 36) Storage racks greater than 59" and up to 8'0" in height shall be installed in accordance to manufacturer specifications for seismic attachment. When seismic attachment is not provided by the manufacturer an engineered design and attachment details shall be required. Installation specifications or engineer's design shall be available during inspections performed by the Mason County Building Department. Storage racks greater than 8-ft in height shall require an engineer's design addressing seismic conditions and attachment. In addition, storage racks 8-ft tall or more shall require special inspection performed by the design professional or an approved representative. The special inspection report and engineered design shall be provided to the Mason County Building Department for approval prior to the final occupancy inspection. X P 37) Accessible routes within the site shall be provided from public transportation stops, accessible parking, and accessible passenger loading zones and public streets or sidewalks to the accessible building entrance served. Provisions for the accessible route shall be designed in accordance to ICC A117.1-2003. Slopes shall not exceed 1:12 and shall be continuous and unobstructed that consist of walking surfaces and marked crossings at vehicular ways. The route shall be at least 36-inches wide, except at doorways. When a route turns around an obstruction the width shall be at least 48-inches wide. A clear space of at least 60-inches by 60-inches shall be provided at intervals of 200-feet or less for passing. Curb ramps shall be at least 36-inches wide with a maximum slope of 1:12 horizontal with flared sides in accordance to ICC A117.1-2003. X //►lY; 38) Commercial Hoods All kitchen hood systems shall be installed and maintained as required by the International Fire Code, Building Code and Mechanical Code. Inspection and maintenance records shall be available for inspectors during all inspections. X � 39) Rooms and enclosed spaces shall have Class C interior wall and ceiling finishes or better. Class C finishes shall have a smoke developed index of 0-450 and a flame spread index of 76-200. Provide classification information during inspection. Solar heat gain coefficient(SHGC) 0.40 or better except skylights SHGC 0.35 or better. OTHER DOORS: Other than entry doors U-0.37 or less and roll-up or sliding doors min. R-4.75. LIGHTING: Total interior lighting shall not exceed 5050 watts, or provide a design calculation demonstrating compliance to IECC/WSEC C405. Lamp wattage shall be calculated using the lamp and ballast or for incandescent fixtures, using the maximum labeled wattage allowed and is approved as follows: A lighting plan has been provided showing the layout of all lights and controls along with a schedule that identifies the wattage for ballast and lamps. All daylight zones shall be provided separate control in accordance with IECCWSEC. Automatic daylight sensing controls shall be capable of reducing light out-put, that control luminaries only within the day lighted area, shall be clearly identified. Switches shall be equipped with separate automatic controls to shut off the lighting in all spaces during unoccupied hours. C observed the placement of(33)threaded dowels.Dowels were held in place with nut&washer to prevent over insertion so that 11 inches will remain above concrete to clear the 1"bracket and achieve required 10"embed in log columns. C observed the contractor install the epoxy and all thread bars.Contractor installed reinforcing bars with a clockwise twisting motion to ensure no air bubbles would pop epoxy out of the holes insuring full coverage.Holes were filled to full capacity to ensure full coverage and adhesion to the reinforcing bar. MTC observed the placement of(22)12"30"length#4 bars for new Rolled edge footing within approximate gridlines A- I.9/2-0.1. Epoxy used was a Simpson Set XP as required. MTC observed the contractor install the epoxy and all thread bars.Contractor installed reinforcing bars with a clockwise twisting motion to ensure no air bubbles would pop epoxy out of the holes insuring full coverage. Reinforcing dowels were Bated to the base of the hole with at least a bead of 1/8"protruding out of the hole and around the reinforcing bar to show that the holes were filled to full capacity to ensure full coverage and adhesion to the reinforcing bar. All materials used and placed were done so in general conformance to the approved site plans,specifications,and per the manufacturers recommendations. To the best of MTC inspector's knowledge,the above-described work was performed in general accordance with project specifications and approved plans. All outside lights are controlled with photo cell and time schedule, the schedule is enabled between 5:00am and 22:30 and the photo cell will turn the lights On or OFF based on ambient lighting during that time. 2. Security lighting outside the building are ON by photo cell when ambient lighting are low. 3. Indoor hardware portion of the store the lights are controlled via time schedule from 6:00am to 22:00. 4. Main store indoor lighting are controlled with time schedule ON at 7:00 and OFF at 22:00. Outside air thru economizer cycle is set during occupied times to deliver 435 CFM's and during unoccupied to be off. The ERV unit that is mounted above the restroom area has also been air balance to provide fresh air in the CFM's total 285. Stall air from restrooms, main building areas and storage areas will be exhausted out the building areas and storage will be exhausted out the building on the north end.
