within 50 feet of a Landslide Hazard Area, you will be required to submit a report or letter prepared by a
professional forester or certified arborist.
If the County determines that one or more of the trees are in a FWHCA buffer or Wetland buffer and do not
meet the definition of danger tree, a Mason Environmental Permit and a Habitat Management Plan will be
required to cut the tree(s). If the County determines that one or more of the trees are in a Landslide Hazard
Area or buffer and do not meet the definition of danger tree, a Mason Environmental Permit and a
Geotechnical Report will be required to cut the tree(s).

1. This tree is a 60-foot hemlock that has died, and I have seen
woodpeckers pecking near the top in the past few months. The
tree is 65 feet from the high-water mark of the creek that's on my
north property line. The tree is also about 40 feet from my picnic
building and RV. The tree is crowded by a fir and cedar tree and
can only fall one way and that is onto my building or RV. I would
like to remove only the dead hemlock before it falls onto my
building or RV.
2. Cedar tree: This cedar tree is on the south property line and is just
150 feet from the creek. There is a PUD power transformer
between this cedar and another one of the same age about 10
inches across the stump. The trees are pushing and heaving the
transformer up. My fear is the tree will cause damage to the
transformer and attached wires causing a power failure. I am
working with PUD 3 about this but thought I would ask to take it
down since I am working on the other trees anyway.
3. This tree is a 40-foot Alder and is on the south property line but
148 feet from the creek, so it meets the rules for your review. The
top branches are dead or dying. The alder next to it fell a year ago
narrowly missing my building that is about 10 feet from the tree.
This tree is also starting to show some signs stump rot. I would
like to take it down before it falls or is blown down.
4. This Alder is right next to tree #2 and has some of the same
issues. Would like to remove it before it falls or is blown over.

When you submit your plans to the Architecture Committee for approval, please remember that the
Committee is comprised of lessees, like yourselves, who have volunteered their time to help maintain
the natural beauty of the Lake Cushman area. The Committee must follow the governing documents of
the Lake Cushman Maintenance Company and any County, State and Federal regulations that may apply
when considering requests. By careful and dedicated effort, and our individual willingness,we can
protect the value of all our properties.Thank you for following the LCMC Covenants, Conditions and
Restrictions when making changes and/or improvements on your property.
This Checksheet must be completed and submitted with vour permit request

100' of a lake? If you checked yes,prior to applying for your LCMC permit, you must submit your
plans to Mason County Permit Center at 360-427-9670 ext 352 to ensure compliance with Mason
County regulations.

Cushman or 482'elevation of Lake Kokanee) If you checked yes, you will need to submit your plans
to Bret Forrester at Tacoma Power-253-502-8782 to ensure compliance with TPU regulations.

prior to applying for your LCMC permit, you must submit your plans to Mason County Permit Center at
360-427-9670 ext 352 to ensure compliance with Mason County regulations.(Note; an easy way to
determine a 40% slope is to use a 10' board. Stick it out level with one end against the ground at the
project site. If the opposite end is 4 ft.or more off the ground that's a 40%or greater slope.)

Lake Cushman Maintenance Co. Tree-Cutting Permit Guidelines
DEVELOPMENT OF 2+ACRE PARCELS
Requirements
By the Lease Agreement imparted to the LCC (the Lessee),by Tacoma Public Utilities(the Lessor), the LCMC
acting for the sub-lessee under item 11 of the Lease has established the following requirements for development
of the parcels containing 2+ acres:
1. The Lake Cushman Architecture Committee, acting under the LCMC can approve development plans that I
allow for a single driveway and for a single building site.
a. Drivewayis limited to 30 feet ' w'm width.
b. Building site is to be no larger than one acre.
c. Stumps and debris must be removed and disposed of in areas designated for material of this nature.
2. Prior to any development or construction, the sub-lessee must make arrangements with the LCMC office

regulations. A County Approved Septic Design is required when requesting a permit to cut trees for septic
installations. Proof of application for a Mason County building permit must be submitted with the tree
cutting permit request when trees are being cut for a structure requiring a county permit. When cutting
trees for construction of a structure, the structure must be shown on the plot plan.
