Dear Sir:
This letter is being written as a result of our on-site meeting yesterday. We have
prepared the needed documentation necessary to begin the process of permitting the fill
site referenced above. Enclosed herewith please find the following:

W/Erosion and Sediment Control Requirements
We fully understand that fees are required for the SEPA Checklist as well as the Fill
Permit application still to be submitted. We ask that you proceed with the initial review
as soon as possible. If you will provide the forms for the Fill Permit Application and the
cost for that application as well as the SEPA Checklist fee we will forward a check to
Mason County for those fees.
We believe that yesterdays meeting of the concerned parties was productive and also
hope that it served to reach a common understanding of the fill permit requirements. We
request that consideration be given towards allowing us to proceed with a fill of up to
20,000 cubic yards as the permit process proceeds. We also believe that the public need
and necessity of reopening SR-101 as soon as possible provides ample justification for
the granting of our emergency request. If Mason County concurs the Washington State
Department of Transportation would like to begin almost immediately to haul fill to the
site so that the Ayock Slide Site can be reopened to at least one way traffic while the
larger project is bid.

FILLING AND GRADING PLAN

Quigg Bros. Inc is proposing to construct a landslide debris fill on property owned by the
Hama Hama Company. The quantity of material to be placed is currently unknown, but
the site capacity is estimated to be more than 250,000 Cubic Yards. The fill material
consists of landslide soils to be removed from the Ayock(M.P. 322) and Lilliwaup (M.P.
326) Landslides on SR-101 south of the mouth of the Hama Hama River. No stumps or
wood waste will be placed into the fill constructed under this proposed project.
The site of the proposed landfill is in a forested valley. No storm water or other drainage
from the site enters the surface waters of the State as the fill site terminates at a natural
pothole at its north eastern boundary. The underlying soils in the area are sand and
gravel overlain by a thin layer of topsoil. The fill site currently infiltrates storm water
into the ground when rainfall intensities are such that onsite infiltration on the
surrounding hillsides cannot accept all of the rainfall.. The pothole shows evidence that
the water level sometimes reaches 5 or more feet in depth,however as the soils are free
draining and the duration of this inundation is not sufficient to sustain wetland type
plants. The area was dry during each of two recent visits.
The intent of the proponent and the property owner is that storm water ponds will be
established on each end of the fill. This will allow storm water to infiltrate into the
ground water which duplicates the existing systems function.. The proposed fill will
occur in stages beginning at the lower end of the valley. The fill will be delivered to the
site by dump trucks. The material will then be spread by bulldozer into lifts. Type 3
compaction will be provided. Greater densification will occur as time goes by, but is not
necessary as the site will be returned to forestry.
Erosion and Sediment Control Requirement #1
Ground cover will be reestablished as the fill proceeds to limit the exposed soils to meet
the requirements of Mason County Ordinance# 141-97. Exposed soils beyond the work
area will be covered within 7 days. No sediment will be allowed to leave the site.
Erosion and Sediment Control Requirement #2
Clearing limits will be clearly marked prior to any clearing of the site. No easements,
setbacks, sensitive/critical areas or their buffers are in the vicinity of this project.
Erosion and Sediment Control Requirement #3
No public or private properties exist within %2 mile of the subject site, therefore Erosion
and Sediment Control Requirement #3 does not apply. Silt fence will be installed along
the access road to insure what any mud or other debris from the trucks will not be washed
by rainfall to any surface water.
Erosion and Sediment Control Requirement#4
The nature of the existing topography does not allow sediment to leave the site.
Sediment ponds will be installed and maintained at both the upper and lower extent of the
fill.
Erosion and Sediment Control Requirement #5
No cut slopes will be constructed as part of this project. The fill will be placed in the
bottom of a natural draw. The only fill slopes to be created will be at the upstream and
downstream ends of the fill where it meets the natural ground level. We anticipate that
the fill will be slightly humped in the center to channel storm water to the edge of the fill
where it abuts the existing slope. A grass-lined swale will be established along both
edges of the fill as it proceeds down slope. This swale abutting the undisturbed soils of
the valley sides will allow most of the storm water from the top of the fill to infiltrate into
the ground in the same manner as is currently occurring. The down valley fill slope will
be limited to a 3:1 slope and the swales will be directed to discharge into rock lined
channels to protect the slopes. A rock check dam will be placed for each 2 feet of
vertical drop on the fill slope.
Erosion and Sediment Control Requirement #6
Not offsite erosion occurs from this site. No discharge of storm water offsite exists
except the discharge to ground water previously described. This plan will maintain that
condition.
Erosion and Sediment Control Requirement#7
The sites ability to infiltrate the 2-year, 24 hour storm will be maintained. Stabilization
as previously described will be provided to prevent erosion of drainage conduits or
outlets, slopes and downstream reaches and to protect the discharges from all onsite
conveyance systems.
Erosion and Sediment Control Requirement#8
The measures described above are intended to protect the ability of the overall site to
continue its function in the discharge of storm water to the ground water. Since this site
does not currently have a manmade storm water control system and since none is
proposed this requirement does not apply.
Erosion and Sediment Control Requirement #9
This requirement does not apply in this instance, as no underground utility construction is
included in this project.
Erosion and Sediment Control Requirement #10
Construction vehicles will access the site via an existing private gravel haul road. The
haul road is approximately 1/2 of a mile long and this will be adequate to limit the
tracking of mud from the fill site back onto SR-101. The loading site at the SR-101 slide
will require some attention. WSDOT will establish those procedures in their contract.
Silt fences will be provided where required along the haul road as previously described.
Erosion and Sediment Control Requirement #11
Any temporary erosion control BMP's will be removed within 30 days of final site
stabilization. Any sediments collected will be removed and stabilized on site. All
disturbed soils will be planted by hydroseeding and covered with mulch as required in
Erosion and Sediment Control Requirement 41. Final site stabilization will be
accomplished on a schedule while will allow the ground cover adequate time to estabilish
itself.
Erosion and Sediment Control Requirement #12
This requirement does not apply for this project.
Erosion and Sediment Control Requirement #13
Litter, fuel or lubricating oils or other possible pollutants will be closely monitored
during construction of this fill. All BMP's will be followed and any reports required by
Mason County or other agencies with jurisdiction will be filed.
Erosion and Sediment Control Requirement #14
All temporary erosion or sediment control features will be maintained during the life of
this project. Permanent erosion or sediment control features will be maintained as
required to maintain their intended functionality.
STATE ENVIRONMENTAL POLICY ACT
ENVIRONMENTAL CHECKLIST
Purpose of Checklist:
The State Environmental Policy Act (SEPA), chapter 43.21C RCW,
requires all governmental agencies to consider the environmental
impacts of a proposal before making decisions. An environmental impact
statement (EIS) must be prepared for all proposals with probable
significant adverse impacts on the quality of the environment. The
purpose of this checklist is to provide information to help you and the
agency identify impacts from your proposal (and to reduce or avoid
impacts from the proposal, if it can be done) and to help the agency
decide whether an EIS is required.
Instructions to Applicants:
This environmental checklist asks you to describe some basic
information about your proposal. Governmental agencies use this
checklist to determine whether the environmental impacts of your
proposal are significant, requiring preparation of an EIS. Answer the
questions briefly, with the most precise information known, or give the
best description you can.
You must answer each question accurately and carefully, to the best of
your knowledge. In most cases, you should be able to answer the
questions from your own observations or project plans without the need
to hire experts. If you really do not know the answer, or if a question
does not apply to your proposal, write "do not know" or "does not apply."
Complete answers to the questions now may avoid unnecessary delays
later.
Some questions ask about governmental regulations, such as zoning,
shoreline, and landmark designations. Answer these questions if you
can. If you have problems, the governmental agencies can assist you.
The checklist questions apply to all parts of your proposal, even if you
plan to do them over a period of time or on difference parcels of land.
Attach any additional information that will help describe your proposal
or its environmental effects. The agency to which you submit this

Proposed fill to begin during late April or early May 1999
weather permitting. The applicant proposes a 2 year term for
the permit due to the uncertainty of weather and difficulty of
working on landslides.

Nothing beyond a secondary desire on the part of the property
owner and the applicant to make this site available for other
projects so long as permitted capacity is available. Ultimate
capacity of the site will be determined as this project proceeds
through the permit process.

11. Give brief, complete description of your proposal, including
proposed uses and the size of the project and site. There are
several questions later in this checklist that ask you to describe

This project will provide for disposal of landslide debris from
the Lilliwaup M.P. 326 and Ayock M.P. 322 landslides, which
have blocked SR-101. The proposed fill site has no natural
discharge except to ground water through infiltration. The site
is very lightly timbered with non-merchantable trees that are
suffering from root rot. The property owner wants to remove
the few small trees and then cover the site with fill material.
The land will be replanted with ground cover and trees
following completion of the filling process. Provision will be
made at both the upslope and downslope ends of the proposed
fill to allow storm water to infiltrate into the pervious sands
and gravels which underlay the site. This method of storm
water control duplicates the existing condition.
12. Location of the proposal. Give sufficient information for a person
to understand the precise location of your proposed project,
including street address, if any, and section, township, and
range, if known. If a proposal would occur over a range of area,
provide the range or boundaries of the site(s). Provide a legal
description, site plan, vicinity map, and topographic map, if
reasonably available. While you should submit any plans
required by the agency, you are not required to duplicate maps or
detailed plans submitted with any permit applications related to
this checklist.
All of the work to be undertaken as part of this project is
located within the South 1/2 of Section 27, Township 24 North,
Range 3 West WM. Parcel # 32427-00-04000
B. ENVIRONMENTAL ELEMENTS
1. EARTH:

Maximum slopes are in the 31% range in the area to be filled.
This project will reduce cross slopes in the area of the fill and
will leave the lower valleyslope at near current grades, but at a

somewhat higher elevation. The down slope end of the
proposed fill will be limited to a 3:1 slope. Provisions will be
made to control erosion from the fill.
C. What general types of soils are found on the site (for example,
clay, gravel, sand, peat, muck)? If you know the classification of
agricultural soils, specify them and note any prime farmland.
The soil within the project area generally consists of sand and
gravel with shallow topsoil or forest duff on or near the surface.
The existing soils can be described as free draining.
d. Are there surface indications or history of unstable soils in the
immediate vicinity? If so, describe.

e. Describe the purpose, type, and approximate quantities of any
filling or grading proposed. Indicate source of fill.
Exact quantities of the proposed fill are not currently known,
but the site has a capacity of more than 250,000 cubic yards.
f. Could erosion occur as a result of clearing, construction, or use?

No. Construction work will include protective measures for the
possible minor erosion problems.
g. About what percent of the site will be covered with impervious
surfaces after project construction (for example, asphalt or
buildings)?
No impervious surfaces will be created as a result of this
proposed project.

to the earth, if any:
Erosion control features will be constructed as the fill proceeds.
The intent is to leave the site in a condition which will allow
storm water to infiltrate into the ground in the same manner as
currently exists. Straw bales, rock check dams and silt control
fences will all be used. Silt fences and straw bails will be used
along the existing haul road from SR-101 to the fill site.
2. AIR:
a. What types of emissions to the air would result from the proposal
(i.e. dust, automobile, odors, and industrial wood smoke) during
construction and when the project is completed? If any,
generally describe and give approximate quantities if known.
Dust and equipment exhaust, during construction. A water
truck will be used if necessary to control dust generated by the
trucks delivering material to the fill site.
b. Are there any off-site sources of emissions or odor that may
affect your proposal? If so, generally describe.
The materials transport to the fill site by truck is also a
potential source to emissions consisting of dust and exhaust
emissions, but these are subject to other environmental
regulation. Dust control on site will be provided as needed to
limit offsite dust.
C. Proposed measures to reduce or control emissions or other
impacts to air, if any:
None. Contractor's equipment is controlled by current
regulations.
3. WATER:
a. Surface:
1) Is there any surface water body on or in the immediate
vicinity of the site (including year-round and seasonal
streams, saltwater, lakes, ponds, wetlands)? If yes,
describe type and provide names. If appropriate, state what
stream or river it flows into.
Yes, the Hama Hama River is approximately 1/2 mile away
and Hood Canal is approximately 5/8 mile away, however
no drainage from the proposed fill site directly enters
either body of water. All stromwater from the site enters
the groundwater. This project will leave that system
intact.
2) Will the project require any work over, in, or adjacent to
(within 200 feet) the described waters? If yes, please
describe and attach available plans.

3) Estimate the amount of fill and dredge material that would
be placed in or removed from surface water or wetlands and
indicate the area of the site that would be affected. Indicate
the source of fill material.

The intent is to leave the existing pothole as the primary storm
water control mechanism. The amount of open unrestored
earth will be kept to a minimum to reduce the potential of
plugging the underlying sands and gravel with fines. Straw
bales, rock check dams and silt control fences will all be used.

process is complete. The existing trees in this area have root
rot and the land owner believes the fill will provide better

It is estimated that as many as 150-200 truck trips per day will
occur during construction. Most of these trips will occur before
the peak summer traffic season and will cease when the
highway is reopened to traffic, no conflicts with traffic are
expected

Silt fences along the private haul road to insure that mud
which may fall off of the trucks is not washed into any surface
water.
