BLD2003-01256
1) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance Division.
There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be obtained at
1-800-647-9982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to WA state law.

2) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will
not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building
Department prior to any further inspections being performed or approvals granted.

3) In accordance with the Uniform Building Code, all sites shall have approved numbers or addresses located in such a position as to be plainly visible and
legible from the street or road fronting the property. Mason County Building Department requires that this be completed prior to calling for any site
inspections. A re-inspection fee based on rates as adopted by the jurisdiction and the Uniform Building Code will be assessed if the owner and/or
contractor fail to post the address on site prior to requesting inspections.

4) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact
adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the
stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further
information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access
connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction which
is proposed to be located within 25'of a Mason County road right of way, it is suggested to contact that office to review future planned work which may
affect your project. ,

5) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Mason County ordinances and building regulations.

6) The plan review check list and corrections, along with the Energy Compliance Worksheet(when applicable)are part of the approved plans and must
remain thereto. It is the responsibility of the applicant to make the corrections indicated on the plans. Once the plans are marked "APPROVED", they
shall not be changed or altered without authorization from the Building Official. The permit holder is responsible to retain the complete approved set of
plans on site for the duration of the project. Failure to comply and/or removal of approved documents will result in failure of required building inspections.

9) The Uniform Fire Code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150'from an approved
access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where
such roads connect with a county maintained public road or to another fire apparatus access road which connects to a county maintained public road. X
10) All construction must meet or exceed all local ordinances and the 1997 Uniform Building Code requirements as adopted and amended by Mason County
and the State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would
result in permit revocation.

11) Placement of structure must comply with standards setforth per 1997 UBC Chapter 18 regarding descending and/or ascending slopes.

12) All changes to"approved" building plans that effect compliance with the Uniform Codes as amended and adopted, or any other Mason County ordinance
or regulation, must be reviewed and approved by Mason County prior to construction.

13) The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance
with the Uniform Codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building
Inspector shall be made prior to requesting additional inspections.

14) All propane tanks must be installed in accordance with the Uniform Fire Code and all applicable Mason County ordinances. All propane tanks must meet
the install tion requirements and minimum setbacks as listed in the Mason County Fire Marshal's Standards for the Installation of Propane Tanks.

15) Fuel piping shall be inspected after the installation of gas piping is complete, and before the attachment of fixtures, appliances, or shut-off valves. At the
time of inspection the test pressure shall be no less than 10 psi held for no less than 15 minutes. Appliances to be attached to the fuel piping system shall
not be use until the final inspection has been performed and approved by a Mason County building inspector.

16) All property lines shall be clearly identified at the time of foundation inspection. X

17) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit
holder have prevented action from being taken. No more than one extension may be granted.

18) The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical report or assessment. Structures
and/or land modifications (grading, cuts,fills, etc.)required in the geotechnical report/assessment, may require a seperate permit. The geotechincal
report/assessment shall remain attached to the approved building plans. X
19) All upland areas disturbed or newly created by construction activities shall be seeded, vegetated or given an equivalent type of erosion protection (silt
fencing or straw matting). X ;,;j
20) All construction and debris must be removed from the area after project completion. Proper disposal of construction debris must be on land in such a
manner that debris cannot enter the ravine or cause water quality degredation of State waters. X -,191-
21) Temporary erosion control measures must be implemented to prevent water quality degradation of adjacent waters or wetlands. Silt fencing must be
installed and maintained until upland vegetation has become established. X _ '?-Z
22) Approved per dimensions and setbacks on submitted site plan. No allowable buildable area shall be allowed along the south and west side of the
proposed residence due to location of Type 4 stream.

23) The Mason County Resource Lands and Critical Areas Ordinance requires vegetative management of the site. There shall be a minimum disturbance of
trees and vegetation in order to minimize erosion and stabilize Landslide Hazard Areas. Limbing trees for view purposes is preferred over tree removal.
Vegetation shall remain undisturbed along top of ravine and steep slopes to prevent erosion and siltation from entering stream

24) Regulations which apply for GRADING of the site (per Mason County Resource Lands and Critical Areas Resource Ordinance):
A. No grading shall be performed in Landslide Hazard Areas or slide prone areas prior to obtaining a grading permit subject to approval by the Director
and based on recommendations contained in the geotechnical report with slope stability analysis and grading recommendations.
B. Clearing, grading, and other construction activities shall not aggravate or result in slope instability or surface sloughing.
C. Undergrowth shall be retained to the extent feasible.
D. Clearing methods which minimize soil disturbance shall be used.
E. No fill, dead vegetation (slash/stumps), or other foreign material shall be placed within a Landslide Hazard Area.

26) Proposed structure(s) must maintain a minimum of a 20'setback from all property line , easements and 25'from all County and State Road right of ways.

27) All changes to"approved" building plans that effect compliance with the Uniform Codes as amended and adopted, or any other Mason County ordinance
or
regulation, must be reviewed and approved by Mason County prior to construction.
28) Applicant acknowledges that a type 4 stream flows in a southeasterly direction, estimated at a range of 115'to 125'from the south and west portions of
the proposed residence. The type 4 stream eventually intersects with Cranberry Creek a Type 1 stream near the north property line. Per the Mason
County Resource Ordinance Fish and Wildlife Chapter 17.01.110, a vegetative buffer is required to protect the Fish and wildlife Conservation area of 100
feet of undisturbed vegetative buffer and 15'building setback. Activities contemplated in the buffer are subject to additional planning review and permits.
No structure is allowed in buffer without further planning review.

This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended fora period of 180 days at any time after work is
commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied.

Comments: The proposed building site is located adjacent/contain steep slopes. A
steep slope trends downward at an angle in excess of 21 .8 degrees,
approximately 75 to 100 feet away from the building site. This area
meets the criteria for a Landslide Hazard Area per the provisions of the
Mason County Resource Ordinance No. 77-93. Applications for
development within 300 feet of a Landslide Hazard Area require a
geotechnical report to assess slope stability and address specific
efforts to remediate the hazard. Enclosed is a copy of the Landslide
Hazard Areas chapter of the Resource Ordinance. Please note the
distinction between a geologic assessment and a geotechnical report.
For informational purposes only, the ravine on the subject property
contains a drainage that is identified as a Type 4 stream. The drainage
flows in a southeasterly direction and eventually intersects with
Cranberry Creek to the north. Cranberry Creek flows along the
northern boundary of the subject property. A type 4 stream per the
Mason County Resource Ordinance Fish and Wildlife Chapter provides
for a 100' vegetative buffer and 15' building setback from the Ordinary
High Water Mark of the stream.

comments: It is determined, per County Planning and Public Works review of the
geotechnical assessment for a single family residence located at 681
Greenview Lane, Mason County, that additional information is
necessitated in order to have their geotechnical assessment completed
to meet geotechnical reporting requirements. As noted, even though
the assessment adequately addresses slope stability issues, due to the
ravine which contains slopes in excess of 100%, located approximately
80 feet from the proposed building site, a geotechnical report is the
necessary appropriate level of reporting that applies to this site.
