The Washington State Shoreline Management Act(RCW 90.58) requires that substantial developments within designated
shorelines of the state comply with its administrative procedures (WAC 173-14) and the provisions of the Mason County
Shoreline Management Master Program. The purpose of this Act and local program is to protect the state's shoreline
resources. The program requires that substantial development (any development of which the total cost or fair market
value exceeds $7,047.00 or materially interferes with the normal public use of the water or shorelines of the State be
reviewed with the goals, polices, and performance standards established in the Master Program.

Install a replacement pier,ramp,float,and mooring buoy alternative on the Sullivan's Lake Cushman shoreline property.This will be a reduction in
137.33 SF of over-water coverage and development in the shoreline buffer.Note:Include ATF permitting for previous float repairs to this application.
The 4'x29'pier will be supported by(8)6"diameter Schedule 80 piles,driven to refusal depth.The pier will be fully grated with Eco-grate 62 to achieve
60%open area.A 3'x6'fully grated aluminum pre-fab ramp will span between the pier and float.The 8'x20'float will be attached to(4)6"diameter
Schedule 80 guide piles,driven to refusal depth.This float will be fully grated with Eco-grate 62 to achieve 60%open area.A float cradle will be installed
to prevent the float from resting on the bottom which includes 4 stub piles(6"diameter Schedule 80 piles,driven to refusal depth)and 6x8 AYC beams.
The mooring buoy alternative will meet Tacoma Power requirements.
Existing Use (identify current use of property with exist improvements):
❑This shoreline property is used recreationally and seasonally. Existing improvements include
8'x11'8" fixed pier with wood decking installed on (4) posts encased in concrete, 3'x6' wooden
ramp, and (2) 8'x20' floats with wood decking on (8) piles and concrete footings.
Reason for requesting development:
❑To replace existing dock structure while bringing it up to compliance with current regulatory code
requirements.
Note:The float was repaired under expedited HPA permits but float is now being fully replaced under current project so
proponent is not proceeding with ATF permits on the float.

The Applicant shall provide, at a minimum, the following information:
a. SITE PLAN -drawn to scale and including:
i. The boundary of the parcel(s) of land upon which the development is proposed,
ii. The ordinary high water mark (OHWM). This may be an approximate location provided, that for any development
where a determination of consistency with the applicable regulations requires a precise location of the OHWM the
mark shall be located precisely and the biological and hydrological basis for the location as indicated on the plans
shall be included in the development plan. Where the OHWM is neither adjacent to or within the boundary of the
project, the plan shall indicate the distance and direction to the nearest OHWM of a shoreline;
iii. Where appropriate, the proposed land contours using five-foot intervals in water area and ten-foot intervals on
areas landward of OHWM, if development involves grading, cutting, filling, or other alteration of land contours;
iv. The dimensions and location of existing structures which will be maintained:
v. The dimensions and locations of proposed structures; parking and landscaping;
vi. The location of proposed utilities, such as sewer, septic tanks and drain fields, water, gas and electricity;
vii.The location, source, composition, and volume of fill material,
viii. The location, composition and volume of any extracted materials, and proposed disposal area,
b. CROSS SECTION, drawn to scale including:
i. The existing ground elevations,
ii. The proposed ground elevations,
iii. The location and height of existing structures;
iv. The location and height of proposed structures;
V. The OHWM.
c. VICINITY MAP, including:
i. The location of subject parcel using natural points of reference (roads, state highways, prominent landmarks,
etc.).
ii. If the development involves the removal of any soils by dredging or otherwise, identify the proposed disposal
site on the map. If disposal site is beyond the confines of the vicinity map, provide another vicinity map
showing the precise location of the disposal site and its distance to nearest city or town.
iii. On the map, or separately, give a brief narrative description of the vicinity of the proposed project including
identification of the adjacent uses, structures and improvements, intensity of development and physical
characteristics.
d. ADJACENT LANDOWNERS. Provide names and mailing addresses of all real property owners within 300
feet of property line boundaries where development is proposed as mailing labels or pre-addressed
envelopes.

Dear Mr. and Mrs. Sullivan:
You recently contacted the City of Tacoma, Department of Public Utilities, Light Division
(Tacoma Power) to review the enclosed plans to demolish and remove existing shoreline and
dock structures, and lakebed debris, and to construct a new dock and convert your mooring
buoy to meet Tacoma Power's Shoreline Use Specifications and Permitting Guidelines
(SUSPG). This work is proposed on Tacoma Power property fronting your leased development
lot at 90 N Sundown Dr. at Lake Cushman (Figure 1). We have reviewed the following materials
in conjunction with your proposed project:
• Tacoma Power shoreline use permit (SUP) application, special conditions, and scope of
work, 5 pages total, by contractor, Integrated Northwest Construction (enclosed)
• Engineering drawings, 5 pages, by MET Engineering, with redline details from contractor
(enclosed)
• Tacoma Power records
Your proposal is approved with the following conditions:
1. Your proposal will require a cultural resources survey (CRS) performed by a licensed
archaeological firm. To protect inspection result confidentiality Tacoma Power will
employ a consulting archaeologist directly. However, the Applicant pays Tacoma Power
a deposit ($2,500) to secure these services. You have provided the required deposit to
Tacoma Power. Your CRS will be scheduled during the next reservoir drawdown as the
lake level, weather and consulting archaeologist availability allow. After the CRS is
complete, you will receive a summary of the findings.
Results of the CRS are used to determine if Tacoma Power can authorize the
proposed/existing uses and if any special conditions are necessary to protect cultural
resources. If the CRS findings show the proposed project or existing uses may have
affected cultural resources, Tacoma Power may deny the Permit or work with the
Applicant to revise the design to avoid an effect to the resource, if possible.
2. Your proposal requires regulatory permits from Mason County, the Washington State
Department of Fish and Wildlife, and the US Army Corps of Engineers. You may
proceed with regulatory permitting for the project as described in your attached plans.

Please provide a copy of this letter to the regulatory agencies when you submit your
application and plans.
It is your obligation to apply for, and obtain, all applicable regulatory permits. Tacoma
Power makes no assertions that your project can obtain regulatory permits as proposed.
If your project must be redesigned, you must re-submit your plans for Tacoma Power
review and approval. Once the required regulatory permits are obtained, you must
provide copies to Tacoma Power.
3. A construction license is required before any work occurs on Tacoma Power property.
Once the CRS is completed and Tacoma Power has verified that you have secured all
relevant regulatory permits, Tacoma Power will authorize the work through a
construction license.
4. A SUP is required for your use of Tacoma Power property. After installation is completed
and approved, then a SUP will be issued for your allowable uses and structures located
on Tacoma Power property.
