We reviewed the Integris Arch/Hargis Engineers detail 6/S3.01 Fuel Tank Foundation Slab.The contractor has#4 @ 12"OC EW,on 3"concrete dobies.The concrete cover at bottom and sides of the monolithic footing conform to the requirements of structural drawings.There is an expansion joint material surrounding the tank slabs.The control joints at the tank slab comers have every other bar cut. Shelton, WA 98584 Subject: Final Special Inspection Letter Mason County Permit No.: COM2021-00001 Dear Mr. Luck, In response to the request and authorization of Young Kim representing WA State Department of Enterprise Services, Krazan &Associates has provided special inspection and related construction materials testing services for the 18-318 WCC Xers and Switches project located at 2321 West Dayton Airport Rd., in Shelton, WA 98584. Our services were performed by our staff from February 2, 2022 through under the supervision of a State of Washington registered engineer. Our observations were documented in our inspector's daily field reports which have been submitted the owner, structural engineer of record, contractor, and the Mason County building department. Krazan provided special inspection for the following types of construction at the project site: 1705.2-Steel Construction 1705.3-Concrete Construction 1705.6-Soils In accordance with Chapter 17 of the 2015 International Building Code,the work requiring special inspection was performed in conformance with approved plans and applicable standards of workmanship at the time of completion. Limitations We have performed our services in a manner consistent with the standard of care and skill ordinarily exercised by forms of our type practicing under similar conditions at the time and locality our inspection was performed. Please recognize that construction monitoring is a technique employed to reduce the risk of problems arising during construction. Provision of construction monitoring by Krazan &Associates personnel is not insurance, nor does it guarantee the construction of any type. In all cases the contractor shall retain sole responsibility for the quality of their work, adhering to the approved project plans and specifications and for repairing any defects, deficiencies or omissions, regardless of when they are found. Our services were limited to the work documented in our reports. We do not undertake the guarantee of construction or completion of a completed project conforming to the plans and specifications. PROJECT#: 10622055 PERMIT#: PROJECT: 18-318 WCC Xers&Switches Northeast INSPECTOR: Geoffrey Fisher LOCATION: 2321 West Dayton Airport Rd.Shelton JURISDICTION: Mason County KA P.M.: Wes Mahan WEATHER: No precip TEMP:461 F As dispatched,I arrived on site and met with project superintendent,Kent,we discussed today's required inspection activities, which involved a rebar inspection of the first-floor foundation and concrete pour/sampling for the Nitrogen Tank Enclosure slab,compaction testing of backfill in the electrical trench,and a concrete pour for the electrical trench. I reviewed the plans and inspected the rebar placement in the generator pad forms.Based upon my observations,I determined that the installation of the steel reinforcement was performed in substantial conformance with the plans and specifications. Approximately 10.75 CY of 4000psi mix ID 1240AF was supplied by Miles Sand and Gravel,and delivered by revolving drum mixer truck. The truck discharged the concrete directly into the slab forms via truck chute. The concrete was tested for slump,temperature,air content and five sample cylinders were cast on site. The samples were placed in a Krazan company curing cooler and left on site for the initial cure. Refer to Concrete Report 10622055CR4272022GF-1 for test result data. Approximately 10.5 CY of 3000psi mix ID 01450C duct bank mix was supplied by Miles Sand and Gravel and delivered by revolving drum mixer truck,which discharged the concrete directly into the duct bank(electrical)trench.The concrete was tested for slump and temperature,and five sample cylinders were cast on site.The samples were placed in a Krazan company curing cooler and left on site for the initial cure. Refer to Concrete Report 10622055CR4272022GF-2 for test result data. Several soil compaction tests were completed with a Troxler Model 3430 Moisture Density gauge on the native backfill and crushed surface top course for the electrical conduit trench.The tests revealed that the compaction of the soil materials met the 95%of maximum dry density requirement. Refer to Soil Compaction Report 10622055SCR4272022GF for the test result data. We met with the contractor who provided the Mason Co permit and the Hargis Engineering structural drawings.We inspected the(2)generator pads at lines 1.5 to 2,and D to E,and E to F.These are in conformance with detail 5/S3.01. We also inspected the nitrogen tank enclosure pad at 8'south of line A,at 1.2 to 1.8 lines approximately.The reinforcement for this pad conforms to detail 8/S3.01 David � PK. Shelton,WA JURISDICTION: Mason County KA P.M.: Wes Mahan WEATHER: Partly Cloudy TEMP:55 At the request of Kent representing Northeast Electric,I arrived on site at 18-318 WCC Xers&Switches at 10:00 am to perform field concrete testing for the Electrical Duct Bank 60' S.of Vault MHB to 15'N.of MH2 and perform field density testing for the structural fill and CSTC for the Electrical Duct Banks 50' S.of Vault MHB and 70'N.of MHX to 50'N.of Vault MHW(see SCR for exact locations).Upon arrival I notified Kent that I was onsite.I was issued a badge by the DOC to be present on the site.After my review of the provided project documents I made general site observations regarding compaction testing and concrete placement.The crew placed sifted poorly graded sand w/gravel in lifts for the trench back fill prior to my arrival,the crew had placed and compacted—10 CSTC at the top of trench.Each lift was compacted using a large sled compactor making several passes to compact the material.The crew used an excavator with a grading bucket to opened multiple sections of the CSTC to test the structural fill placed below.In-place density tests were performed in accordance with ASTM D6938 at various locations and elevations,the test results indicated that the fill material placed met or exceeded the minimum requirement of 95%of ASTM D 1557.All test results were verbally communicated to Kent while onsite. See compaction report 10622055SCR04252022JH.Concrete was batched by Miles Sand and Gravel,Plant#208.29 yards of Mix Id 01450C were placed today to the Electrical Duct Bank 60' S.of Vault MHB to 15'N.of MH2.Concrete was placed using the revolving drum mixer truck chute.Contractor consolidated the concrete by hand and tool.See concrete report 10622055CR04252022JH.Specimens were left onsite for initial curing. I met with the Rotschy's foreman Jari;we discussed the scope of work for him today.Field reinforcing inspection for the Nitrogen Tank Enclosure at the east edge of the Generator Building.I performed a review of the approved drawings prepared by Robert W.Graper with Hargis Engineering.I then made general site observations to the area to be inspected.During my inspection I observed the#4 bar size at 12"OC EW,3414 continuous at perimeter,lap lengths,bond breaker at Gen Building stem wall and concrete cover.There was no vapor barrier installed at this slab.I notified the contractor that there was inadequate concrete coverage along the north edge of slab,the contractor corrected the issue prior to leaving the site.The reinforcing steel inspected for the Nitrogen Tank Enclosure was in accordance with the provided structural drawings at various locations.HSS bolts were in place w/10"embed. Upon arrival I notified Kent that I was onsite.I was issued a badge by the DOC to be present on the site.After my review of the provided project documents I made general site observations regarding compaction testing and concrete placement.The crew placed sifted poorly graded sand w/gravel in lifts for the trench back fill prior to my arrival,the crew had placed and compacted—10 CSTC at the top of trench.Each lift was compacted using a large sled compactor making several passes to compact the material.The crew used an excavator with a grading bucket to opened a section of the CSTC at MHY to test the structural fill placed below.In-place density tests were performed in accordance with ASTM D6938 at various locations and elevations,the test results indicated that the fill material placed met or exceeded the minimum requirement of 95%of ASTM D1557.All test results were verbally communicated to Kent while onsite.See compaction report 10622055SCR04132022JH1. Concrete was batched by Miles Sand and Gravel,Plant#208.31 yards of Mix Id 01450C were placed today to the Electrical Duct Bank 50'N.of Vault MHX to 150'S.of Vault NM.Concrete was placed using the revolving drum mixer truck chute. Contractor consolidated the concrete by hand and tool.See concrete report 10622055CR04132022JH.Specimens were left onsite for initial curing. I met with the Rotschy's foreman Jari;we discussed the scope of work for him today.The crew had placed and compacted the sifted structural fill for the Generator Building SOG prior to my arrival.The crew utilized and excavator with a sifting bucket to place and level the material.The crew used a large sled compactor to compact the lifts placed.I performed field density testing for the structural fill at grid lines 1.2/F.5, 1.8/F.5, 1.2/C.8 and 1.8/C.8.In-place density tests were performed in accor- dance with ASTM D6938 at various locations,the test results indicated that the fill material placed met or exceeded the mini- mum requirement of 95%of ASTM D 1557.All test results were verbally communicated to Jerry while onsite.See com- paction report 10622055SCR04132022JH2.They crew will place the cap break next. I noted ach of the bolted assemblies had the twist control knob twisted off and the bolted assembly was tight I was un able to remove the nut with out the use of a wrench. Contractor consolidated the concrete using a gas-powered concrete vibrator and by hand.See concrete report 10622055CR03292022JH.Specimens were left onsite for initial curing.
