Instructions for applicants: This environmental checklist asks you to describe some basic information about your proposal. Please answer each question accurately and carefully,to the best of your knowledge. You may need to consult with an agency specialist or private consultant for some questions. You may use "not applicable" or "does not apply"only when you can explain why it does not apply and not when the answer is unknown. You may also attach or incorporate by reference additional studies reports. Complete and accurate answers to these questions often avoid delays with the SEPA process as well as later in the decision- making process. The checklist questions apply to all parts of your proposal, even if you plan to do them over a period of time or on different parcels of land. Attach any additional information that will help describe your proposal or its environmental effects. The agency to which you submit this checklist may ask you to explain your answers or provide additional information reasonably related to determining if there may be significant adverse impact. VB BTS II proposes the construction of a 150-ft AGL (160-ft AGL with appurtenances) monopole communications tower, an associated 100-ft x 100-ft tower compound lease area and proposed 15-ft wide by approximately 485-ft long accesslutility easement to be located at 110 E. Tintagel Place, in the City of Shelton, within Mason County, Washington.The project is located within a portion of NE '/4 SE '/4 of Section 28, Township 21N, Range 3W, Willamette P.M. Shelton, Washington. This project is located on Tax Lot# 321280000000 identified by the Mason County Tax Assessor's Office. 12. Location of the proposal. Give sufficient information for a person to understand the precise location of your proposed project, including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any permit applications related to this checklist. Shelton, Washington. B. ENVIRONMENTAL ELEMENTS 1. Earth a. General description of the site (Circle one): Flat, rolling, hilly, steep slopes, mountainous, other: Proposed tower compound area is generally flat sloping to the northwest b. What is the steepest slope on the site (approximate percent slope)? -2% slope within the leasehold area. c. What general types of soils are found on the site(for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any agricultural land of long-term commercial significance and whether the proposal results in removing any of these soils. According to the USDA Web Soil Survey of Mason County, WA, the soil of the subject property is identified as Alderwood gravelly sandy loam, 8 to 15% slopes. Alderwood gravelly sandy loam is described as moderately well drained, found on hills or ridges, and is formed from glacial drift and/or glacial outwash over dense glaciomarine deposits. A typical soil profile consists of gravelly sandy loam from 0 to 7-inches and very gravelly sandy loam from 7 to 59-inches below ground surface (BGS). d. Are there surface indications or history of unstable soils in the immediate vicinity? If so, describe. None known or observed. e. Describe the purpose,type,total area, and approximate quantities and total affected area of any filling, excavation, and grading proposed. Indicate source of fill. Proposed project requires minimal amount of cut, and spoils will be deposited on-site. A proposed 150-ft AGL (160-ft AGL with appurtenances) monopole communication tower, within a 100-ft x 100-ft fenced tower compound and an associated 20-ft x 12-ft gravel turnaround area. The compound will be accessed by a proposed approximately 15-ft x -435-ft access & utility easement. These areas are herein referred to as the direct area of potential effect(APE) and total approximately-16,765-ft' (0.38-acres). Weed-free gravel will be brought in from a local landscape company or gravel supplier (TBD). f. Could erosion occur as a result of clearing,construction,or use? If so, generally describe. Yes, but erosion will be controlled with proper best management practices (BMPs), including but not limited to silt fencing and straw wattles. About what percent of the site will be covered with impervious surfaces after project construction (for example, asphalt or buildings)? Proposed measures to reduce or control erosion, or other impacts to the earth, if any: Construction BMPs including but not limited to sift fencing and straw wattles. 2. Air a. What types of emissions to the air would result from the proposal during construction,operation, and maintenance when the project is completed? If any, generally describe and give approximate quantities if known. During construction, equipment/vehicle emissions and dust are expected. Construction should occur over the course of approximately 8-weeks. Upon completion, the facility will be accessed approximately 1 to 2 times every 3 months. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. No. c. Proposed measures to reduce or control emissions or other impacts to air, if any: None. 3. Water a. Surface Water: 1) Is there any surface water body on or in the immediate vicinity of the site (including year- round and seasonal streams, saltwater, lakes, ponds,wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. None known within the immediate vicinity of the proposed action areas. The nearest identified surface water feature is Cranberry Lake located approximately 1,600-ft to the north, and Lake Limerick located approximately 2,500-ft to the northeast. 2) Will the project require any work over, in, or adjacent to(within 200 feet)the described waters? If yes, please describe and attach available plans. No. Runoff will be controlled with BMPs during construction. 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. N/A 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities if known. No. b. Ground Water: 1) Will groundwater be withdrawn from a well for drinking water or other purposes? If so,give a general description of the well, proposed uses and approximate quantities withdrawn from the well.Will water be discharged to groundwater? Will this water flow into other waters? If so, describe. Stormwater runoff will flow off-site and presumably flow local topography to the northwest. Runoff will eventually infiltrate the soil and/or will flow into Cranberry Lake. 2) Could waste materials enter ground or surface waters? If so, generally describe. The proposed tower compound lease area and associated easement areas were observed to be occupied by a clearcut area with minimal regrowth occurring. c. List threatened and endangered species known to be on or near the site. Review of the USFWS — Mason County — Official Species List indicated the possible presence of one (1) federally endangered terrestrial species: Taylor's checkerspot (Euphydryas Editha taylori); three (3) federally threatened terrestrial species: marbled murrelet (Brachyramphus marmoratus), streaked horned lark (Eremophila alpestris strigata), and yellow-billed cuckoo (Coccyzus americanus); and one (1) federal candidate terrestrial species: monarch butterfly (Danaus plexippus). Additionally, the list also indicated the possible presence of one (1) federally threatened aquatic species: bull trout (Salvelinus confluentus); however, due to the distance from aquatic resources, it is the opinion of TEP that the proposed action will have no effect on the aforementioned aquatic species. The site inspection performed on August 17, 2022 consisted of a pedestrian inspection of the proposed action area and immediate vicinity for indications of the presence of federally listed species and/or their critical habitats and preferred/suitable habitats, as described by the USFWS. According to the USFWS —Trust Resources List, there are no officially designated critical habitats on, or in the vicinity of the action area. TEP also utilized the Washington Natural Heritage Program's Element Occurrences online map and determined that no (0) state-listed species occur within 1-mile of the proposed action area. Further, TEP utilized the WA DFW's Priority Habitats and Species web mapper and found no (0) occurrences for state-listed species within 0.5-miles of the proposed project area. d. Proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on the site, if any: None proposed. e. List all noxious weeds and invasive species known to be on or near the site. Animals a. List any birds and other animals which have been observed on or near the site or are known to be on or near the site. Examples include: None were observed. birds: hawk, heron, eagle, songbirds, other: mammals: deer, bear, elk, beaver, other: fish: bass, salmon, trout, herring, shellfish, other: b. List any threatened and endangered species known to be on or near the site. None were observed on or within the immediate vicinity of the proposed action area.The USFWS Mason County Official Species List for the proposed project indicated the possible presence of one (1)federally endangered terrestrial species: Taylor's checkerspot (Euphydryas Editha taylori); three (3)federally threatened terrestrial species: marbled murrelet (Brachyramphus marmoratus), streaked horned lark (Eremophila alpestris strigata), and yellow-billed cuckoo (Coccyzus americanus); and one (1) federal candidate terrestrial species: monarch butterfly (Danaus plexippus). Proposed measures to preserve or enhance wildlife, if any: The proposed tower will be under 200-ft tall,will be unlit,will not utilize guy-wires,and will not be located within a wetland,on a mountaintop,or in any other area known to have frequent foggy conditions. e. List any invasive animal species known to be on or near the site. None were noted within the vicinity of the proposed action areas during the site inspection completed by TEP. 6. Energy and natural resources a. What kinds of energy (electric, natural gas, oil, wood stove, solar) will be used to meet the completed project's energy needs? Describe whether it will be used for heating, manufacturing, etc. Electric. b. Would your project affect the potential use of solar energy by adjacent properties? If so, generally describe. List other proposed measures to reduce or control energy impacts, if any: None. 7. Environmental health a. Are there any environmental health hazards, including exposure to toxic chemicals, risk of fire and explosion, spill, or hazardous waste,that could occur as a result of this proposal. If so, describe. None known. 1) Describe any known or possible contamination at the site from present or past uses. None. 2) Describe existing hazardous chemicals/conditions that might affect project development and design. This includes underground hazardous liquid and gas transmission pipelines located within the project area and in the vicinity. None. 3) Describe any toxic or hazardous chemicals that might be stored, used, or produced during the project's development or construction, or at any time during the operating life of the project. Lead acid batteries may be utilized within antenna equipment shelters within fence compound. 4) Describe special emergency services that might be required. None. 5) Proposed measures to reduce or control environmental health hazards, if any: None. b. Noise 1) What types of noise exist in the area which may affect your project(for example: traffic, equipment, operation, other)? None. 2) What types and levels of noise would be created by or associated with the project on a short-term or a long-term basis(for example: traffic,construction, operation, other)? Indi- cate what hours noise would come from the site. Short term noise during construction phase (during regular business hours). Land and shoreline use a. What is the current use of the site and adjacent properties?Will the proposal affect current land uses on nearby or adjacent properties? If so, describe. The parent property was primarily occupied by forested and cleared undeveloped land. The adjoining properties to the north, south, and west were primarily occupied by undeveloped forested land and the adjoining properties to the east primarily occupied by low-density residential land use. b. Has the project site been used as working farmlands or working forest lands? If so, describe. No. How much agricultural or forest land of long-term commercial significance will be converted to other uses as a result of the proposal, if any? None. If resource lands have not been designated, how many acres in farmland or forest land tax status will be converted to nonfarm or nonforest use? None. 1) Will the proposal affect or be affected by surrounding working farm or forest land normal business operations, such as oversize equipment access,the application of pesticides, tilling, and harvesting? If so, how: No. c. Describe any structures on the site. None existing within the proposed tower lease area or easement. There is one residential structure east of the proposed action area. d. Will any structures be demolished? If so,what? No. e. What is the current zoning classification of the site? Rural Residential 20 Acres f. What is the current comprehensive plan designation of the site? Rural Residential 20 Acres - Residential zoning districts in rural areas of the County allow for housing to acreage ratios varying from one home for every two and a half acres to one home for every twenty acres. These districts also permit small hobby farms and cottage industries enabling people to work from home, as well as group homes, churches and community centers to meet the needs of rural lifestyles. With the approval of special permits, schools and day care centers can also be developed, further SEPA Environmental checklist(WAC 197-11-960)(MC version-non project portion omitted): 2016 Page 9 of 14 accommodating people living outside the urban areas and alleviating the need for commuting for those families without the resources to do so. There are also multi-family housing districts in the rural areas that provide for duplexes, multifamily housing complexes and mobile home parks. This type of housing can be a more affordable option for low-income families. g. If applicable,what is the current shoreline master program designation of the site? None. h. Has any part of the site been classified as a critical area by the city or county? If so, specify. No. i. Approximately how many people would reside or work in the completed project? None. Unmanned facility with 1-2 visits every three months. j. Approximately how many people would the completed project displace? None. k. Proposed measures to ensure the proposal is compatible with existing and projected land uses and plans, if any: Project will be submitted to Mason County Planning &Zoning for applicable review and permits. I. Proposed measures to ensure the proposal is compatible with nearby agricultural and forest lands of long-term commercial significance, if any: Project will be submitted to Mason County Planning &Zoning for applicable review and permits. At the time of inspection, the proposed area was observed to occupied by cleared undeveloped land. Proposed telecommunications tower facility is anticipated to have no impact to agricultural and/or forested lands of long-term commercial significance. 9. Housing a. Approximately how many units would be provided, if any? Indicate whether high, middle, or low- income housing. None. b. Approximately how many units, if any,would be eliminated?Indicate whether high, middle, or low-income housing. None. c. What is the tallest height of any proposed structure(s), not including antennas;what is the principal exterior building material(s) proposed? A proposed 150-ft AGL (160-ft AGL with appurtenances) monopole communication tower. Tower will be comprised of galvanized steel. b. What views in the immediate vicinity would be altered or obstructed? The assessment concluded that the proposed 150-ft AGL (160-ft AGL with appurtenances) monopole communications tower will be visible from those areas located inside the assessed 0.5-mile area; however, the proposed tower will not be visible from those areas located outside and/or just within the assessed 0.5-mile radius that are obstructed by the existing topography surrounding the parent property to the north, south, east, and west. c. Proposed measures to reduce or control aesthetic impacts, if any: The proposed monopole tower will be less than 200-ft AGL in height and will be an unlit structure. Therefore, the proposed structure is anticipated to be visibly subordinate to the landscape from the majority of the public accessible areas within the aforementioned visual APE and will not adversely impact surrounding public viewsheds. 11. Light and glare a. What type of light or glare will the proposal produce? What time of day would it mainly occur? The tower will be unlit. The galvanized steel tower will naturally "weather" and is not anticipated to be a source of glare. b. Could light or glare from the finished project be a safety hazard or interfere with views? No. c. What existing off-site sources of light or glare may affect your proposal? N/A d. Proposed measures to reduce or control light and glare impacts, if any The proposed tower is an unlit structure. 12. Recreation a. What designated and informal recreational opportunities are in the immediate vicinity? None within the immediate vicinity of the proposed action area. The closest informal recreational opportunity is the Lake Limerick Country Club approximately 0.65-miles northeast from the proposed action area. b. Are there any buildings, structures, or sites, located on or near the site that are over 45 years old listed in or eligible for listing in national, state, or local preservation registers located on or near the site? If so, specifically describe. According to the Washington's State Historic Preservation Office's online database no (0) previously recorded historic properties were identified within the direct APE. b. Are there any landmarks, features, or other evidence of Indian or historic use or occupation? This may include human burials or old cemeteries.Are there any material evidence, artifacts, or areas of cultural importance on or near the site? Please list any professional studies conducted at the site to identify such resources. According to the Washington's State Historic Preservation Office's online database no (0) archaeological sites determined listed or eligible for the NRHP were identified within the direct or visual APEs. c. Describe the methods used to assess the potential impacts to cultural and historic resources on or near the project site. Examples include consultation with tribes and the department of archeology and historic preservation, archaeological surveys, historic maps, GIS data, etc. TEP performed a search on the Washington's State Historic Preservation Office's online database on August 12, 2022 to determine whether any historic properties were located within a 0.5-mile radius. No (0) previously recorded historic properties were identified within the direct APE. No (0) archaeological sites determined listed or eligible for the NRHP were identified within the direct or visual APEs. No (0) artifacts, features, or other signs of historic or prehistoric cultural activities were observed during the pedestrian inspection or shovel testing. TEP has sent correspondence to all applicable tribes with known ancestral and/or aboriginal rights to Mason County, WA; as per FCC Tower Construction Notification System (TCNS). d. Proposed measures to avoid, minimize,or compensate for loss, changes to, and disturbance to resources. Please include plans for the above and any permits that may be required. None known. No (0) previously recorded historic properties were identified within the direct APE. No (0) archaeological sites determined listed or eligible for the NRHP were identified within the direct or visual APEs. TEP has sent correspondence to all applicable tribes with known ancestral and/or aboriginal rights to Mason County,WA; as per FCC TCNS. 14. Transportation a. Identify public streets and highways serving the site or affected geographic area and describe proposed access to the existing street system. Show on site plans, if any. Is the site or affected geographic area currently served by public transit? If so, generally describe. If not, what is the approximate distance to the nearest transit stop? None known. c. How many additional parking spaces would the completed project or non-project proposal have? How many would the project or proposal eliminate? None. A proposed 100-ft x 100-ft fenced tower compound and an associated 20-ft x 12-ft gravel turnaround area. d. Will the proposal require any new or improvements to existing roads, streets, pedestrian, bicycle or state transportation facilities, not including driveways? If so, generally describe (indicate whether public or private). The proposal will require an access and utility easement with an associated 20-ft x 12-ft gravel turnaround area. e.
