Dear Mr. Nichols:
On February 17, 1995 an investigation was conducted of your mobile home location
on lot 107, Division 5 of Lake Limerick, near the intersection of Pebbles Ct and
Comarty Ct., in Shelton. This investigation was in response to a complaint received
by the Department from the Architectural Committee of the Lake Limerick Country
Club. The complaint revolved around alterations being performed on a mobile
home.
Alterations of mobile homes are the jurisdiction of the Department of Labor and
Industries as stipulated in WAC 296-15OB-115 (1) which states in part:
"No person may alter a mobile home...unless the person has first applied for
and obtained the Department's approval of the alteration." A search of our records
indicated that no permit was currently open foi- S/N Z2930 under your name or at
that location. Those areas that were observed in your mobile home for which an
alteration permit is required, include:
1) The new floor decking in the mobile home.
2) The window headers have been removed in the living room.
3) Some portions of exterior wall studs and exterior wall bottom plates
have been removed in the living room.
4) New interior wall finish surfaces.

This letter will serve as a notification of non-compliance in accordance with
WAC 296-150B-135. As such, you will need to obtain an alteration permit at a cost
of$75.00 within ten days. Failure to conform to this course of action may require
the Department to post a prohibited occupancy and sales notice and/or remove the
insignia from the structure.
For your convenience, I have enclosed an alteration permit for you. This permit
would be valid for six months and may, under some circumstances be extended an
additional six months. I look forward to your cooperation in resolving this matter.
