CONDITIONS FOR

1) This applicatigry,is subject to Buffer and Landscaping requirements as established under Mason County Ordinance
1.03.036.X'42
2) The use, handling and storage of hazar to materials or flammable and combustible liquids in excess of 10 gallons is not allowed without the approval
of the Mason County Fire Marshal. X
3) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely
impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of
the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For
further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or
access connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any
construction whic is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that office to review future
planned work w may affect your project.

4) Any portion of any structure over 30" in height from surrounding adjacent grade must be set from all utility and drainage easements, a total of 10'
from each property line, or a variance must be obtained from the Building Department. X � .
5) Proposed structure or any portion thereof greater than 30" in height fro de line, must maintain a minimum of 5' setback from all property lines,
easements and 10' from all County and State Road right of ways. X
6) In accordance with the Uniform Building Code, all sites shall have approved numbers or addresses located in such a position as to be plainly visible and
legible from the street or road fronting the property. Mason County Building Department requires that this be completed prior to calling for any site
inspect ns. A re-inspection fee based on rates as adopted by the jurisdiction and the Uniform Building Code will be assessed if the owner and/or
contr t it to post the address on site prior to requesting inspections.

7) THE FOUNDATION SYSTEM SHALL BE PLACED ON UNDISTURBED, NATIVE SOIL. X
8) The "approved" plot plan is required to be on-site for inspection purposes. If an inspection is requested and the "approved" plot plan is not on site, then
approval will not be granted. In addition, a re-inspection fee in the amount of$47.00 per hour(minimum 1 hour) will be charged and shall be collected
by the Buil � Department prior to any further inspections being performed or approvals granted.

9r Ft!QUIRED INSPECTIONS (Footing Inspection-prior to pour, Set-up Inspection-prior to skirting, Final Inspection-prior to occupancy). I hereby assume
t all responsibility for the scheduling of my required inspections. If the required inspections are not requested, inspected and signed off(approved) by the
inspector in the prescribed order, I understand that reinspection fees and an hourly investigation fee pursuant to the 1997 UBC, and will be assessed in
addition to my original permit fees to resolve any questionable prat ices or problems that have been discovered. I further understand that this
investigation will be scheduled as time allows. Until resolution of all problems no occupancy (Final Inspection) will be granted for the residence.

10) All mobile/manufactured home landings or decks must be freestanding (self supporting). The largest landing or deck allowed without drawings or a
building permit MUST be under 30" in height from surrounding grade. NO second story decks, or decks above 30" can be built without a permit. Any
landing or deck that is 30' ore in height from walking surface to finish grade requires a Permit. Any landing or deck that has 4 or more risers
requires a handrail. X
11) The installation permit shall be displayed in clear view of the site access road. The approved site plan and other applicable instructions, including
installation instructions, shall be available in this location OR placed in the location specified by WAC 296-150M-655. Support configuration shall be
clearly marked in the installation instructions.
12) All property lines shall be clearly identified at the time of foundation inspection. X
13) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The"
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
nonV ppliant with Mason County ordinances and building regulations.
14) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time
for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the
per it Ider have prevented action from being taken. No more than one extension may be granted.

15) Owner/builder assumes all responsibility if drainfield/reserve area is encumbered. X

18) Site is located within an unstable Landslide Hazard Area. Adequate drainage/siltation control/stormwater plan must be submitted for review prior to
development, clearing, grading, or construction on the site. Development should be designed to preserve the natural drainage courses in the area.
Surface drainage, including downspouts, shall not be directed onto or within 50 feet above or onto the face of a Landslide Hazard Area or it's associated
buffer if drainage must be discharged from the top of a Landslide Hazard Area to below it's toe, it shall be collected above the top and directed to
belo toe by tight-line drain and provided with an energy dissipating device at the toe.

19) The Mason County Resource Lands and Critical Areas Ordinance requires vegetative management of the site. There shall be a minimum disturbance
of trees and vegetation in order to minimize erosion and stabilize Landslide Hazard Areas. Limbing trees for view purposes is preferred over tree
removal. Vegetation remov tie slopes of banks between the ordinary high water mark and the top of the bank shall be minimized due to the
potential for erosion.X /
20) Regulations which apply for GRADING of the site (per Mason County Resource Lands and Critical Areas Resource Ordinance):
A. No grading shall be performed in Landslide Hazard Areas or slide prone areas prior to obtaining a grading permit subject to approval by the Director
and based on recommendations contained in the geotechnical report with slope stability analysis and grading recommendations.
B. Clearing, grading, and other construction activities shall not aggravate or result in slope instability or surface sloughing.
C. Undergrowth shall be retained to the extent feasible.
D. Clearing methods which minimize soil disturbance shall be used.
E. No fill, dead vegetation (slash/stumps), or other foreign material shall be placed within a Landslide Hazard Area.

21) All upland areas disturbed or n reated by construction activities shall be seeded, vegetated or given an equivalent type of erosion protection (silt
fencing or straw matting). X
This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period
of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection
must be approved before building can occupie .

NOTIFICATION OF INCOMPLETE APPLICATION

Comments A field inspection of the subject property was conducted on
8/16/01 with the following results.
The site inspection identified slopes on the parcel that qualify as
Landslide Hazard Areas. Prior to approving construction, Mason
County requires a geotechnical report to address slope stability
and construction methods. The specific contents of a
geotechnical report are detailed in the Landslide Hazard Areas
chapter of the Mason County Resource Ordinance No. 77-93. A
copy of that chapter is enclosed for your review. Please note that
this report must be stamped by a licensed engineer.
Upon receipt of the geotechnical report, a County engineer with
the Public Works Department will review the document. Upon
completion of a favorable review, the Planning Department will
sign off on your building permit. No other critical areas issues
affect the construction of the residence. If you have any question
or require clarification on the Landslide issues, please contact
me.
