An Environmental Impact Statement (EIS) is not required under RCW 43 .21C. 030 (2) (c) . This decision was made after review of a completed Environmental Checklist and other information on file with the Lead Agency. This information is available to the public upon request . A to RR r w / Conditions of this Case are listed below: Case No.:SEP96-0101 1 ) MITIGATED CONDITION #1 : Per Mason County Interim Resource Ordinance #77-93 a Geo-tech assessment is required for this site. The reason for the Geo-tech assessment is that the slopes on site exceed the standards in Ordinance #77-93. The geo-tech assessment can be done by either a licensed engineer in the State of Washington or a geologist with expertise in land slide hazards. The short plat will not be recorded until the geo-tech assessment has been reviewed and approved by Mason County Public Works. 2) MITIGATED CONDITION #2: Per Mason County Interim Resource Ordinance #77-93, the type 4 stream that biscests lot "B" will have the following buffers placed upon it. The buffers are as follows: There is a twenty-five(25') foot no-cut buffer on each side of the stream and a forty(40') foot building set-back on both sides of the stream. Both of these buffer are part of a fifty(50') foot management area on both sides of the stream. 3) MITIGATED CONDITION #3: A drainage/siltation control/stormwater plan will be created and implemented prior to development, clearing, grading, or construction of any portion of the proposed segregation. X----------------------- 4) MITIGATED CONDITION #4: Fire apparatus access roads serving more than two (2) building lots shall be a minimum of 20' wide from the existing county road to within 150' of the building sites. Maximum grade is 14% up to nine (9) lots; 12% for ten (10) lots or more. 5) MITIGATED CONDITION #5: If the fire apparatus access road is longer than 1 50', pullouts for fire apparatus may be required. Contact the Mason County Fire Marshal and/or the local fire protection district. 6) INFORMATIONAL ONLY: The applicant/agent will create a pump out plan. This plan will set up the tank(s) of the septic system(s) will be pumped and inspected every 3 years. The report generated from the pump out and inspection will be submitted to and acted upon by Mason County Heath Services, Environmental Health Department. If the segregation becomes part of an established district or is hooked up to a Wastewater Treatment Facility, the above statement will be voided. X 7) A drainage/siltation control/stormwater plan will be created and implemented prior to development, clearing, grading, or construction of any portion of the proposed segregation.
