Gentlemen:
On October 11, 1985 , you were sent three copies of General
Permit No. M85-214 for a road approach to your clinic at the
above-referenced location. Included were instructions for
signature , bond preparation and return of two permit copies .
To date, we have not received the bond and permit copies in
this office . Please complete the steps outlined on the in-
structional letter and return the necessary items to this
office for final execution by the Department.
No work on or use of your approach is authorized until you
have received the validated permit back from us and given the
required notice to the State ' s representive .
