1) The internatioanl code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150'from an approved
access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where
such road� onne t with a county maintained public road or to another fire apparatus access road which connects to a county maintained public road.

2) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will
not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building
Department prior to any further inspections being performed or approvals granted.

3) In accordance with international codes and Title 14, Mason County Building Code, "Standards for Fire Apparatus Access Roads,"all new structures that
require an address shall have approved numbers or addresses located at the beginning of long driveways when the address is not clearly visible from the
access road. The numbers shall also be plainly visible and legible from the street or road fronting the property and shall contrast with their background.
Mason County Building Department requires that this be completed prior to calling for any site inspections. A re-inspection fee based on rates as adopted
by the jurisdiction and the international codes will be assessed if the owner and/or contractor fail to post the address on site prior to requesting
inspections.n

4) The plan review check list and corrections are part of the approved plans and must remain thereto. It is the responsibility of the applicant to make the
corrections indicated on the plans. Once the plans are marked "APPROVED", they shall not be changed or altered without authorization from the Building
Official. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or
rr(emoval of approve documents will result in failure of required building inspections.

5) THE FOUNDATION SYSTEM SHALL BE PLACED ON UNDISTURBED, NATIVE SOIL. X
6) The"approved"site plan is required to be on-site for inspection purposes. If an inspection is requested and the"approved"site plan is not on site, then

7) Concrete used for basement walls, foundation walls, exterior walls, porches, carport slabs, steps exposed to the weather, garage floor slabs and other
vertical concrete work exposed to the weather shall have a minimum compressive strength of 3000 psi (IRC Table R402.2).

8) This structure is limited to U-occupancy use only(private garages, carports, sheds, and agricultural buildings.) Any other use will be in violation of the
internati al codes and Mason County Regulations unless a"Change of Use" permit is applied for, reviewed and approved.

9) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the
State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in
permit revocation.

10) All changes to"approved" building plans that effect compliance with the international codes as amended and adopted, or any other Mason County
ordinance or r ulation, must be reviewed and approved by Mason County prior to construction.

11) The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance
with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building
Inspector shall be�made prior to requesting additional inspections.

12) All property lines shall be clearly identified at the time of foundation inspection. X (�
13) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Mason County ordinances and building regulations.

14) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit

15) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners,
connectors, and ashing. Install metal connectors approved for contact with the new types of pressure treated material.

16) This permit does not release the unit for occupancy. Separate occupancy approval must be obtained prior to any occupancy by Mason County Building
Department. The unit must remain posted prohibiting occupancy. This permit is for the deck construction only.

Therefore, review of your application will not proceed until the necessary information

Comments: Due to slopes immediately adjacent to the project a Geological
Assessment will need to be done to address slope stability and the
other requirements of the enclosed landslide hazard chapter 17.01.100
section E. 4. The report must state that the hazards of the landslide
area can be overcome in such a manner as to prevent harm to property
and public health and safety, and must also assure the project will
cause no significant environmental impact. See Mason County Code
17.01.100, E7.
Please review your Geological Assessment prior to submission to
ensure that it contains the information required in section E. 4. of the
enclosed ordinance. Missing information will result in further delays in
the review process.
If any grading / excavation is proposed in or near slopes a grading plan
will be needed to show any proposed topographical changes, cuts,
retaining walls etc.

The receipt of this notice shall constitute service regarding notification of violations of
the Mason County Code, Title 14, Building and Construction. The violation(s)are
occurring at the following property:

❑ The Mason County Building Department investigated on 09/03/04 a complaint
regarding a Dangerous Building at the above location. The inspector noted that the
complaint appeared valid and posted the building with a Do Not Occupy and
Dangerous Building Notice4.
According to the Mason County Assessor records,parties named above hold legal
interest in the parcel and are therefore subject to enforcement action for failure to
comply.
In order to bring your site into compliance you must: 1)Immediately upon receipt of
this notice, secure the building and structures against entry. Apply for and obtain
the required permits from Washington State Department of Labor and Industries
and Mason County to remodel/repair the mobile home unit and the attached deck.
Please make the necessary arrangements to submit for the required building permits prior
to October 14,2004. If the permit is not received by October 14,2004,we will pursue
additional enforcement action against you, including but not limited to civil infractions,
liens against the property through the Mason County Hearings Examiner process and/or
criminal prosecution as allowed under Title 14 and Title 15 of the Mason County Code.

Returned a call to Johnnie Carey and got Gary. He said they just bought both properties at 191 and 181 and it is in escrow. They have L&I permit for deck to show good faith. Told him needed building
Permits for both decks on both sites and to check with L&I on the other MH repairs(doors,windows,roofs etc). Gave them until 1/l/05 to submit as escrow is 60 days. They will continually check to make
