The numbers shall also be plainly visible and legible from the street or road fronting the property and shall contrast with their background. Mason County Building Department requires that this be completed prior to calling for any site inspections. A re-inspection fee based on rates as adopted by the jurisdiction and the international codes will be assessed if the owner and/or contractor fail to post the address on site prior to requesting insnyX 2) The internatioanl code requires a fire apparatus access road for every facility, building, or portion of a building that is more than 150'from an approved access road. Roads are required to meet the minimum Mason County Fire Marshal standards for Fire Apparatus Access Roads up to the point where such rods�onnith a county maintained public road or to another fire apparatus access road which connects to a county maintained public road. X 3) All approved plans are required to be on-site for inspection purposes. If an inspection is called for and plans are not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule, minimum 1 hour)will be charged and must be collected by the Building Departmg_rrt for y further inspections being performed or approvals granted. The permit holder is responsible to retain the complete approved set of plans on site for the duration of the project. Failure to comply and/or removal of op ve locuments will result in failure of required building inspections. If an inspection is requested and the "approved" site plan is not on site, then approval will not be granted. All installers hired to do installation work shall be certified manufactured home installers and shall be present to supervise the installation of all on-site work. An Installer Tag shall be posted on site giving the certification number and signature of the certified installer responsible for each major part of the installation. RCW43-63B.090 An approved Installer cerification tag shall be placed on the end of the manufactured home directly above or below the HUD certification tag or temporarily located in plain site within three of the home's front entry. There shall be one certification tag for each certified installer accounting for the work that each installer performed or' stalle ertification number and signature of the certified installer responsible for each major part of the installation. WAC365-210 X 8) If you are installing a manufacturing home and no longer have the installation manual for the home, you must use the instructions of the American National Standards Institute (ANSI). To order the ANSI instructions you may either get an order form from the Mason County Building Department or you can contact the Offfice of Manufacturing Housing (360) 725-2800. 9) All construction must meet or exceed all local ordinances and the international codes requirements as adopted and amended by Mason County and the State of Washington. Occupancy is limited to the approved and permitted classification. Any non-approved change of use or occupancy would result in permit re7M� X - 10) REQUIRED INSPECTIONS (Footing Inspection-prior to pour, Set-up Inspection-prior to skirting, Final Inspection-prior to occupancy). I hereby assume all responsibility for the scheduling of my required inspections. If the required inspections are not requested, inspected and signed off(approved) by the inspector in the prescribed order, I understand that reinspection fees and an hourly investigation fee pursuant to the current fees adopted by the Mason County Building Dept., and will be assessed in addition to my original permit fees to resolve any questionable practices or problems that have been discovered. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason County Building InspegWr sh I be made prior to requesting additional inspections. X T 15) The installation permit shall be displayed in clear view of the site access road. The approved site plan and other applicable instructions, including installation instructions, shall be available in this location OR placed in the location specified by WAC 296-150M-655. Support configuration shall be clearly marked in the installation instructions. 16) All property lines shall be clearly identified at the time of foundation inspection. X 17) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with Mason C inty-�ances and building regulations. X 18) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit holder har a ted action from being taken. No more than one extension may be granted. The report must state that the hazards of the landslide area can be overcome in such a manner as to prevent harm to property and public health and safety, and must also assure the project will cause no significant environmental impact. See Mason County Code 17.01.100, E7. The Landslide Hazard covenant attached to the back of the enclosed ordinance will also need to be completed and recorded with the Mason County Auditor prior to building permit issuance. Thank you. 2/11/2005 2 of 2 BLD2005-00136 -10 NC MEMORANDUM Date: September 2, 2005 TO BE KEPT IN THE PARCEL FILE TO: Emmett Dobey, Community Development Director FROM: Barbara Robinson, Permit Assistance Center Manager � SUBJECT: Permit # BLD2005-00136/Tax Parcel No. 12330-54-00008 I reviewed the information you provided from Rita Matson regarding her request to change or revise a permit that was issued for placement of a manufactured home on a lot in Beard's Cove (Belfair). The file on this project includes documentation of permit fees paid for a mobile home submittal, planning review, mobile home issuance, address fee, EH plan review, grading, geotech report review, variance, MEP and enforcement for work done without permits. The applicant now wishes to revise her permit, however, it is a complete change in the type of structure, location, size, etc. It is also not clear where the new building would be placed, and whether the geo tech information we have is adequate to address construction of a building on the site with a daylight basement. Given the changes proposed it would be difficult to just revise the existing permit for a change in construction type from a manufactured home to stick built and a revised building footprint. The most appropriate way to handle this change would be to cancel the manufactured home permit and issue a new permit for a stick built home. This will require a new submittal, including a new site plan with setbacks, location of septic and drainfield, new construction drawings of the proposed structure, and geotech report addressing placement on the site. Once specific site and building information is provided we can then determine if additional information will be needed. In a phone conversation with Debbera Coker on August 9, 2005, the applicant was advised of the requirement to submit new plans for review. Based on the past activity and staff time associated with this project so far, the fees paid previously cannot be refunded, with the exception of 80% of the mobile home issuance fee. This individual fee was $214.50, so 80% would be $171.60. The remainder of the fees paid by the applicant are associated with permits and enforcement actions already processed and reviewed, therefore not eligible for refund. There is some question as to whether a $610.00 enforcement fee was paid, as it does not show as paid on Tidemark. This will need to be confirmed with Kristin. This project had a considerable amount of enforcement action based on activities associated with fill placed without permits, an MEP and variance. The permit number is BLD2003-01011 deck repair. Attached are the four pages approved by your planning department with full instructions on what was required to properly repair my deck. These are labeled Exhibit"A". Unfortunately after this repair permit was issued I was diagnosed with serious problems due to Agent Orange from my service in Vietnam during 1968, which unfortunately will eventually result in my death. Naturally I had to attend to this problem and delay my deck repair. Since then.I have been granted 2 permit extensions. I am sorry that this simple repair has taken so long. When I was granted each extension I made a special trip to Mason County for each extension and specifically went over Exhibit"A"with the desk person to make sure that I would be doing everything as required and they assured me that what I had been originally been given was correct. Repairs on the deck were finally started during the past few months. On March 2, 2005 a Mason County Inspector was able to make an inspection on my deck repair. His findings labeled Exhibit`B" are attached and a different county instruction drawing was enclosed. This different set of instructions now includes items#'s 3 and 4 (which I had never seen before). Item number 4 now requires me to raise the front posts, pour concrete and the lower posts to set on concrete. My original instructions never included this and I was specifically instructed, by the building department, not to move or touch any blocks because it would constitute a design change and would require engineering. gq Thus complicating a simple deck repair and was not needed. Page 2 Not understanding this new change I called your building department. On March 11, 2005 the inspector that inspected my deck called me. I asked about this change and I have never been treated so rudely in all my life. I was told that I was a liar and that this was the way the deck is supposed to be repaired,the desk was wrong and it is now my problem not yours. Please compare Exhibits"A"and`B", I am not lying. I was repeatedly asked what my job was before he would continue any further conversation about the deck. Why is this necessary? I cannot ask any person that I deal with what race, color,creed or job they hold,why is this required? Also this person used profanity during this phone conversation, is this normal procedure? I am so disgusted with his actions and the way he talks to people I will never speak to this person again or allow him on my property. Is this the way employees of Mason County are directed to treat the property owners of Mason County? All I have tried to do is repair the deck correctly from the beginning and now I am belittled, sworn at and called a liar. Am I supposed to complete the deck as per Exhibit"A' or`B"? If"B"why was I told to do "A" and now have to redo part of this deck? I still find it hard to believe that this is the way Mason County conducts business. I was always taught that government is there to help people. I now feel like I am under a dictatorship that changes it's requirements when it wants to. I am sorry to bother you with this letter. All I wish to do is repair this little cabin so it does not fall apart, but I do not know what to do from here. Again,thank you for a moment of your time.
