I have reviewed the preliminary soils reconnaissance report and have visited this site. I
recommend construction of the home's foundation be permitted, subject to the following
condition:
Currently, the cut slope is dry and appears stable. Should the cut slope show any
signs of excessive sloughing or other distress or should an unusual weather or seismic
event occur before the concrete toe wall is constructed, the contractor shall stop work
immediately and contact the geo-technical engineer.
I understand there is concern that this report does not meet County standards for a
geotechnical report. However, the engineer has specified that a concrete toe wall sha!!
be constructed. I do expect that acceptable calculations for such a wall will contain all
the information required in a geotechnical report. I will need to review these
calculations before I can recommend construction approval for the concrete toe wall
and the house framing.

Gary:
I have some concerns as to how this permit started let alone what
has happened so far.
It seems that Terry Ryan when out to the site to inspect for the
footings and found that the applicant/agent had removed as
significant amount of soil . Terry determined that a grading
permit should be required.
It seems that a error was made by someone who review the original
building permit for planning and didn't condition the permit by
the Critical Resourse Ord. So no geo-tech stuff was required.
Well, once Terry Ryan required the grading permit, I went out to
the site. It was upon returning to the office I stopped by to
see Tami Griffey to talk to her about the permit. That is when I
received a copy of the geo-tech stuff by Rola Bought, P.E.
I then sent the geo-tech to Jerry Hauth, Alan Tahja, Rich Geiger.
I scheduled with Rich to review the site on Aug. 19 to expite the
permit. Rich review the site and also looked at the geo-tech
information submitted by the applicant to building. As of today
I have not received the IDC from Rich about the site visit or the
geo-tech stuff .
Now it seems that the building dept. want to issue the building
permit for the site, which is understandable since that is their
job.
I have had some discussion with Jerry Hauth as to the geo-tech
stuff and he feels that the geo-tech stuff is ok with him.
My concern is that the geo-tech stuff does not meet the policy
and should there for be returned to the applicant and the
building permit be put on hold, until the engineer has developed
the geo-tech stuff to the standards identified in the geo-tech
policy dated Oct. 5, 1998 . I have attached to this note the
building permit, the geo-tech stuff and am awaiting a bulkhead
designed to protech the proposed home. Please get with me when
you can.
I have several other issue to discuss with you, which are the
harvesting of the timber within the buffer areas for aquatic
management areas . Denial of a pier/dock permit at Lake Cushman

Called Anita back. She said it was included on permit. I pulled the permit and read to her the conditions that she signed regarding what was required and what needed permits. During the discussion she
said her permit was still open for the MH. Permit was cancelled by expiration on 12-06-01 and informed her that the last inspection which was prior to that was only for 30 day occupancy. Told her that she
also needs to renew MH permit and get a final. I said I would send her a notice of violation and permit package that would inform her of everything that needs to be done.I will mail it on 6-19-02. Scott
Reeves is no longer involved in this project as they divorced and she now owns the house

Anita called and said ex-husband has included house in bankruptcy. She doesn't know if she will have to move or can keep it and therefore,she doesn't want to spend the$on the deck permit if she has to
move. I agreed to allow her to do a$52.30 temp occ for 180 days on BLD99-00406. I also agreed to allow her to submit the atf deck permit without submittal fees on or by July 18th as originally ordered. I
explained that all the fees would be due when it is ready and since the bankrupcy ends on July 31,the deck permit won't be ready until about then anyway.If she moves out,we will post the MH for non-occ
and require new occupant to take care of deck issues

RE: Preliminary Walk-over Soils Reconnaissance of Lot at Above Address
Dear Ms. Reeves;
The undersigned Geo-technical Engineer, along with Cindy Forbes, Geo-technician,
visited the site of the lot described above on August 15 to evaluate the effect of
the recent excavation cut for your new house foundations.
The upper portion of the lot is gently sloping but the cut face is nearly
vertical (80' to the horizontal by readings on a "Wedge Smart Level".) The cut face is
about 23 ft. in height by direct measurement.
I noticed the soil exposed on the slope was a typical glacial till soil with
most of the fractions classified as silt and medium sand with gravel inclusions.
Photographs illustrating the soils types were taken for our file and record-. Slopes
and details such as vegetation were also photographed for our file.
The nearly vertical cliff at the rear of the home site is a stable conglomerate
soil of sand and silt with gravel inclusions.
Another project I detIagned and inspected the construction thereof in this
vycinity with a similar Geo-technical problem was stabilized by a concrete toe-wall.
This installation has been in place about 12 years and our re-inspection on August 15th
proved this approach to assuring stability to be highly successful.
THE MATERIAL IN THE NEARLY VERTICAL CUT AT THE REAR OF THE HOME SITE IS A
FINE SAND WITH SILT AND GRAVEL INCLUSIONS UP TO 6 INCHES IN DIAMETER. IN MY
PROFESSIONAL OPINION THIS MATERIAL IS A CEMENTED CONGLOMERATE AND WILL BE STABLE
IN THIS NEARLY VERTICAL CUT.
A short about 8 ft. in ht. toe-wall for retention of slough should be designed

and constructed shortly after the house foundations are placed.
