1) This application is subj to ffer and Landscaping requirements as established under Mason County Ordinance
1.03.036.X
2) The use, handling and storage of har ous aterials or flammable and combustible liquids in excess of 10 gallons is not allowed without the approval of
the Mason County Fire Marshal. X
3) Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely impact
adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements of the
stormwater ordinance of prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose. For further
information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access
connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any construction which
is proposed to be located wi 'n 25'5a Mason County road right of way, it is suggested to contact that office to review future planned work which may
affect your project.

8) In accordance with the Uniform Building Code, all sites shall have approved numbers or addresses located in such a position as to be plainly visible and
legible from t e street or road fronting the property. Mason County Building Department requires that this be completed prior to calling for any site
inspections. A re-ins ction fee based on rates as adopted by the jurisdiction and the Uniform Building Code will be assessed if the owner and/or
contractor'po address on site prior to requesting inspections.

9) The plan review check list and corrections, along with the Energy Compliance Worksheet (when applicable) are part of the approved plans and must
remain th . It is t e responsibility of the applicant to make the corrections indicated on the plans. Once the plans are marked "APPROVED", they
shall not a ange r altered without authorization from the Building Official. The permit holder is responsible to retain the complete approved set of

10) This struct is app ed as un-heated space. If at any time this building is to be used for anything other than what it is approved for, a change of use

20) All building rm*ts shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure
to request tinlinspe tion or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with
Mason Cc rdin s and building regulations.

21) All permits ex 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for
action for a p not ceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control of the permit
holder have ri vent ction from being taken. No more than one extension may be granted.

This permit becomes null and void if work or construction authorized is not commenced within 180 days,or if construction or work is suspended for a period of 180 days at any time after work is
commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied.Proof of continuation of

The Mason County Department of Community Development,Building Division,has a pending
Enforcement case against you. As a result of the enforcement action,a building permit was submitted by
you and has been ready to issue since 04/08/2002 with fees due of$269.00.During a site visit in February
of this year 2005 with you Mr.Boyce you had agreed to pay$100.00 a month to the Belfair annex until all
due fee's are paid. So the permit could be issued and resolve this matter.
Pursuant to the Mason County Code,this department may issue civil infractions when a violation of Title
14,Building and Construction Code,occurs.Additionally,Title 15 of the Mason County Code also allows
for the case to be heard and penalties assessed according to Section 15.13 through the Hearings Examiner
process.
Failure to provide the information as previously requested and reiterated with this notice within 30 days
from the date of this notice,will result in enforcement action as allowed for by law and as noted above.
If you have abated the violation by removing/demolishing the offending structure or portion thereof,please
contact this office immediately to arrange for a compliance inspection and close the enforcement case
against you.
