The Department of Health requires that the utility submitting a Water System Plan(WSP) to DOH must submit a concurrent WSP for Consistency and for the County for signature of the Consistency Statement Checklist(enclosed) completed by the local planning agency when all County issues and been satisfied. Please review the accompanying WSP for consistency with county adopted plans and development regulations,and complete and return a signed Consistency Statement Checklist to Washington Water Engineering Department.Any inconsistencies you might identify will be addressed prior to DOH approval of the WSP. Box 186 426 W.Cedar Street Bldg III Shelton,WA 98584 RE: Allyn Shopping Center Water System,ID#01695 Q Wellhead Protection Area Delineation Mason County As a part of the wellhead protection program for Washington Water Service Company, we are hereby informing you of the findings of our wellhead protection area delineation for this drinking water system. This is in accordance with State regulations(WAC 246-290-135). Our company operates a number of satellite water supply systems which, like this one,rely exclusively on groundwater wells for the production and delivery of drinking water for its customers. Our company provides drinking water to approximately19,000 families. This represents a population of about 50,350 people. Each of our water systems kiss provided a Susceptibility Assessment Survey to the Washington State Department of Health (DOH) for determination of each system's exposure to potential pollutants. The DOH has given this particular system a Moderate Susceptibility" Rating and considers this water supply system to have a Low Vulnerability"to contamination from activities or incidents that take place within the wellhead protection area delineated on the attached map(s). The attached map(s) show the 1, 5 and 10 year time of travel boundaries for our wellhead protection steals). Any groundwater contamination that occurs within the wellhead protection steals) has the potential of reaching our water supply. It is therefore of utmost importance to us,and to the families we serve, that all reasonable steps be taken To ensure that land use activities within these areas do not contaminate this drinking water supply. To the extent that your department or agency is responsible for wellhead protection measures, please include the full extent of the areas delineated on the attached map(s)in your efforts.
