2) ALL C RUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY
IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF,,USE OR OCCUPANCY WOULD RESULT IN PERMIT

It is unlawful for any person to cause or allow the demolition (or major renovation) of any structure unless all asbestos containing materials have
been identified and removed from the area to be demolished. Work shall not commence on an asbestos project or demolition project unless the

6) Any changes in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to
construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are
removed, approval will not be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and
collected a Mason County Building Department prior to any further inspections being performed or approvals granted.

7) CONST TION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
XasoakCounty Building Inspector shall be made prior to requesting additional inspections.
8) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
non-c9F
apliant with Mason County ordinances and building regulations.

9) THE FOUNDATION SYSTEM SHALL BE PLACED ON UNDISTURBED, FIRM-NATIVE SOIL.

10) FENCES/GATES
Gates shall be provided with an approved key box/knox box installed in accordance with the 2012 International Fire Code Section 506. Please
contact the local fire district for more information and to discuss inspections for installation of the lock box system.

11) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the
time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control
of the pert holder have prevented action from being taken. No more than one extension may be granted.

12) All ap'pm Z plans are required to be on-site for inspection purposes. If inspection is called for and plans are not on site, Approval WILL NOT be

13) All property lines shall be clearly identified at the time of foundation inspection. X
14) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries, Contractor Compliance
Division. There are potential risks and monetary liabilities to the homeowner for using an unregistered contractor. Further information can be
obtained at 1-800-64�N982. The person signing this condition is either the homeowner, agent for the owner or a registered contractor according to

15) EXISTING PLAYGROUND EQUIPMENT WILL BE REFURBISHED AND RE-LOCATED IN A NEW LOCATION. INSTALLATION OF THE
EQUIPMENT WILL BE PERFORMED BYA QUALIFIED PLAYGROUND EQUIPMENT INSTALLER AND IN ACCORDANCE WITH THE
MANUFACTURER INSTALLATION SPECIFICATIONS FOR STRUCTURES LOCATED IN SEISMIC DESIGN CATEGORY D2.
WHEN MANUFACTURER SPECIFICATIONS ARE NOT AVAILABLE PROVIDE AN ENGINEERED DESIGN, PLANS, AND DETAILS, PREPARED
BYAWASHINGTON STATE LICENSED DESIGN PROFESSIONAL, ON-SITE FOR INSPECTION.

16) Retaining walls needed to support a surcharge such as st res, roads, or to support slopes, shall require a separate building permit and
approval prior to construction of the retaining wall. X

18) The approved site plan is required to be on-site for inspection purposes. If inspection is called for and the site plan is not on site, Approval WILL

19) Compliance with an approved Storm Water Plan [ESA 7/3/2014] shall be subject to an on site inspection, or inspections, by the engineer of record
or an authorized representative. Said inspection(s) shall be performed in accordance with the approved plan, and prior to any modification that
would make a determination of compliance impossible.
Inspection reports shall be submitted to verify all engineering requirements have been completed in accordance with the approved storm water
plan. Reports shall be submitted to the Mason County Department of Community Development prior to each corresponding inspection and final
per royal. Copies of all special inspection reports shall be made available at time of inspection.

20) In to the inspections required in IBC, Section 109, the owner, the engineer or architect of record acting as the owner's agent shall employ
one or more special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by
the design professional. The special inspectors duties & responsibilities shall be as specified in Chapter 17.
Special inspections shall be performed by MATERIALS TESTING & CONSULTING for the work listed on sheet S1.01, including soils, concrete,
and cold-formed steel framing.
Special inspection reports shall be submitted to the Mason County Building Department, 426 West Cedar St, Shelton WA 98584 and available for
inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and
final oc ancy inspections.

21) ALL SURFACE WATER AND POTENTIAL RUNOFF WILL BE CONTROLLED ON SITE AND SHALL NOT ADVERSLY AFFECT ANY ADJACENT
PROPERTIES NOR INCREASE THE VELOCITY FLOW ENTERING OR ABUTTING TO ANY STATE OR COUNTY CULVERTING/DITCHING
SYSTEM ROADWAY.

22) Struc observations shall be performed by the engineer of record, or their authorized representative in accordance with IBC 1704.5 and as
specified in accordance with plan sheet S1.01. Reports shall be prepared for each visit and a copy provided to the Mason County Building
Department.
A final observation report shall be submitted prior to the final inspection of the structure. Written statements shall indicate the permit number, type
of inspection, and identify any reported deficiencies.
Structural observation is defined as the visual observation of the structural system by a registered design professional for general conformance to
the approved conformance to the approved construction documents at significant construction stages and at the completion of the structural
system.

23) The building design for the playshed building shall be reviewed by the foundation design engineer PCS Structural Solutions. Provide written
verification prepared by engineer D. Goodwin confirming that he has reviewed the construction design. Revised plans containing modifications
required as a It of the the actual design shall be available, on-site for inspection.

24) Gate hard� on exit gates identified on sheet A1.02 and detail 1/A1.02 shall be panic hardware or fire exit hardware listed in accordance with
UL305. The actuating portion of the relaeasing device shall extend at least one-half of the gate width with a maximum unlatching force of 15
pounds. (ref IBC 1008)

25) A ispersal area is to be created in accordance with IBC 2017.5:
Where access to a public way cannot be provided, a safe dispersal area shall be provided where ALL of the following are met:
1) The area shall be of a size to accomodate at least 5 sq. ft. for each person.
2)The area shall be located on the same lot at least 50 ft. away from the building requiring egress.
3) The area shall be permanently maintained and identified as a safe dispersal area.
4) The aria shall be provided with a safe and unobstructed path of travel from the building.

Observed the placement of ASTM A615 grade 60 reinforcing stcel into column wraps within the locations noted above.
Reinforcing steel was of proper size,grade,lap,spacing,location,clearance,and secure prior to placement of concrete,pending acceptance
of noted deficiencies.
Items Needing Engineer Of Record Acceptance:Reference Pictures Below.
1.The contractor did not install the 3/4"A307 threaded rods as detailed on 9/S3.00.
2.The#3 horizontal bars were placed and lapped as a standard tie.The hooks on the ends of the bars as detailed on 9/S3.00 were not
provided.
3.Contractor needs to provide an approved mix design for the Hard Rock C40A concrete mix.

One set of 54"x 8"cylinders were cast for compressive strength test specimens.Time,slump,temperature and air content tests were
performed.

Concrete was mechanically consolidated using a high cycle vibrator.
