Onsite this date for the requested footing inspection for the proposed new cell tower. While onsite I was
given structural plans by the jobsite foreman (Tracy Hermann)that were not approved by Mason County
and were completed and stamped by an Oregon Engineer. All Structural plans must be reviewed and
approved by the Mason County Building Department and are required to be designed and stamped by a
Washington State licensed Engineer. Contractor and owner are to consult with a Washington State
engineer and have onsite supplied plans or new structural plans designed, stamped, and submitted for
review and approval. Footing inspection failed due to the lack of approved structural plans/documents.
At the request of the contractor and owner, rebar cage and footing hole were looked at using the
unapproved supplied drawings. Inspection was not passed or approved. Owner and contractor stated
getting the plans stamped by a Washington State Engineer was not going to be a problem and are going
to proceeding at their own risk. If submitted structural design does not match installed rebar cage or
footing requirements additional engineering and approvals will be required.
Plans must be submitted as soon as possible for review and approval. No other structural inspection will
be conducted until all required documents have been approved for use by the Mason County Building
Department.

* Contractor registration laws are governed under RCW 18.27 and enforced by the WA State Dept of Labor and Industries,
Contractor Compliance Division. There are potential risks and monetary liabilities to the homeowner for using an
unregistered contractor. Further information can be obtained at 1-800-647-0982. The person signing this condition is either
the homeowner, agent for the owner or a registered contractor according to WA state law.
* By definition, propane tanks and heatpumps are structures, which must meet setback conditions. Please check your
"Approved Site Plan"to ensure these structures meet the setback conditions listed.
* When parcel development requires direct access to state road(s), a Road Access Permit or Approval must be granted and
approved by the Washington State Department of Transportation. For more information contact Washington State
Department of Transportation, at (206)357-2620, ext. 630.
* All construction must meet or exceed all local and state ordinances in addition to the International Codes requirements as
adopted and amended by Mason County and the State of Washington. Occupancy is limited to the approved and permitted
classification. Any non-approved change of use or occupancy would result in permit revocation.
* All RED stamped approved plans are required to be on-site for inspection purposes. If an inspection is called for and
plans are not available on site, then approval will not be granted. In addition, a re-inspection fee (refer to current fee
schedule, minimum 1 hour)will be charged and must be collected by the Building Department prior to any further inspections
being performed or approvals granted.
* Approved per dimensions and setbacks on submitted site plan. Setbacks are measured from the furthest projection of the
structure.
* The international code requires a fire apparatus access road for every facility, building, or portion of a building that is more
than 150'from an approved access road. Roads are required to meet the minimum Mason County Fire Marshal standards
for Fire Apparatus Access Roads up to the point where such roads connect with a county maintained public road or to
another fire apparatus access road which connects to a county maintained public road.
* Owner/Agent is responsible to post the assigned address and/or purchase and post private road signs in accordance
with Mason County Title 14.28 and 14.17.
* All changes to "approved" building plans that effect compliance with the international codes as amended and adopted, or
any other Mason County ordinance or regulation, must be reviewed and approved by Mason County prior to construction.
* When parcel development requires direct access to county road(s), a Road Access Permit or Approval must be granted by
the Mason County Department of Public Works. For more information contact Public Works, at (360)427-9670, ext. 450 or
100 W Public Works Dr. Shelton. The building permit will not be finaled until the permit holder can show proof that the
access permit from Public Works has been finaled and approved.
* All building permits shall have a final inspection performed and approved by Mason County Building Department prior to
permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property
records on file with Mason County as being non-compliant with Mason County ordinances and building regulations.
* All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building
Official may grant a one time extention of 180 days, upon the receipt of a written extension request prior to permit expiration.
Letter must indicating that circumstances beyond the control of the permit holder preventing action from being taken. No
more than one extension may be granted.
* Concrete encased grounding electrodes must be installed and used at each new building or structure that is built upon a
permanent concrete foundation. In Mason County the electrical code is regulated by Washington State Department of Labor
& Industries (L&I).

* The stamped approved site plan is required to be on-site for inspection purposes. If an inspection is requested and the
approved site plan is not on site, approval will not be granted. In addition, a re-inspection fee (refer to current fee schedule,
minimum 1 hour)will be charged and shall be collected by the Building Department prior to any further inspections being
performed or approvals granted.
* All surface water and potential runoff must be controlled on site and shall not adversely affect any adjacent properties nor
increase the velocity flow entering or abutting to any state or county culverting/ditching system or road way.
* The foundation/footing must be placed on undisturbed, firm-native soil. Proper frost depth shall be observed below grade
in undisturbed soils.
* All property lines shall be clearly identified at the time of foundation inspection.
* Provisions for surface/subsurface drainage control must be implemented with new construction or development on site and
MUST NOT adversely impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either
private ditches and drains will meet requirements of the stormwater ordinance or prior approval will be granted to use an
existing utility and drainage easement dedicated for that specific purpose. For further information regarding this ordinance
and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a driveway or access connecting
from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450. For any
construction which is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that
office to review future planned work which may affect your project.
* Must comply with Ecology comments from a letter dated 3/29/23
* The approval of this project is subject to the recommendations and specifications outlined in the attached geotechnical
report or assessment. All applicable recommendations and specifications shall be applied to the development on this site.
Any deviation requires stamped written approval from the registered design professional responsible for the
report/assessment, and may require special inspection by same. Structures and /or land modifications (grading, cuts, fills,
etc.) required in the geotechnical report/assessment, may require a separate permit. The geotechnical report/assessment
shall remain attached to the approved building plans.

Observation:
Thirty-five (35) #10 vertical reinforcing bars,
thirty-nine (39)#4 horizontal ties, and six (6) U-
;,P. bar horizontal rebars were installed for the new
foundation along with corresponding tower
anchor rods. All rebar was confirmed to be
ASTM A-615 (Grade 60)with a 3" minimum
clear cover. This was completed in accordance
with the modification drawings provided by B&T
Group dated April 23, 2023.

Bridge for the installation of the subject structure. The purpose of this letter is to confirm that all bolts were tightened
and installed in accordance with the structural design drawings completed by Black & Veatch, dated April 12, 2023.
The required special inspections were performed per Chapter 17, sections 1704 and 1705, of the 2021 International
Building Code.
All observations were performed during the construction phase. Tower Engineering Professionals, Inc. was present
during the construction phase. The inspection was performed by Reed Stelten, P.E., C.W.I. of Tower Engineering
Professionals,Inc.All bolts were found to be properly installed per manufacturer recommendations.

Dear Mr. Luck,
In response to the request and authorization of Jay PerryCook representing Eagle Commercial Services, Inc., Krazan &
Associates has provided special inspection and related construction materials testing services for the SE05432F Lake Limerick
project located at 175 E Penzance Rd, in Shelton,WA 98584. Our services were performed by our staff on February 14, 2024
under the supervision of a State of Washington registered engineer.
Our observations were documented in our inspector's daily field reports which have been submitted the owner, structural
engineer of record, contractor, and the Mason County building department. Krazan provided special inspection for the
following types of construction at the project site:
1705.3 Concrete Construction
In accordance with Chapter 17 of the 2018 International Building Code,the work requiring special inspection was performed
in conformance with approved plans and applicable standards of workmanship at the time of completion.
Limitations
We have performed our services in a manner consistent with the standard of care and skill ordinarily exercised by forms of
our type practicing under similar conditions at the time and locality our inspection was performed. Please recognize that
construction monitoring is a technique employed to reduce the risk of problems arising during construction. Provision of
construction monitoring by Krazan &Associates personnel is not insurance, nor does it guarantee the construction of any
type. In all cases the contractor shall retain sole responsibility for the quality of their work, adhering to the approved project
plans and specifications and for repairing any defects,deficiencies or omissions, regardless of when they are found. Our
services were limited to the work documented in our reports. We do not undertake the guarantee of construction or
completion of a completed project conforming to the plans and specifications. No warranty expressed or implied.

As dispatched,I arrived at the SE05432F project located at 175 E Penzance Rd in Shelton.Upon arrival,I met with Jay
PerryCook,the owner of Eagle Commercial Services,Inc.(ECS,Inc.),and we discussed the scope of services for this visit.
The scope of services entailed a steel reinforcement inspection and the observation of the concrete placement for the outdoor
slab and thickened edges,as well as sampling fresh concrete and casting one set of five(5)4"x 8"inch cylinders to test at l-
and 28 days.
The perimeter of the work area was walked and the#4 rebar and#4 stirrups were noted for the top slab and thickened edges
with the correct spacing,lap splice,and coverage per details found on the structural plans prepared by Tower Engineering
Professionals on Sheet C-13.The plastic cover was also noted per the details on the sheet as well.
The 16 cubic yards of Miles Sand&Gravel Mix Design ID 0360A50 was observed as it was placed via boom pump by
Brundage-Bone Concrete Pumping and mechanically consolidated by ECS,Inc.The composite concrete sample was collected
at approximately 6 cubic yards of the total placed from the northwest corner of the thickened edge and tested for slump,air
content,temperature,and one set of five(5)4"x 8"inch cylinders were cast for compression testing.The cylinders were then
placed inside an insulated box on leveled ground located next to the cell tower fence.Refer to Concrete Report
10624031 CR2142024JV for the test result data.
Prior to leaving Jay was informed of today's inspection.

test cylinders by J.Vargas on 2/14/2024.The insulated box was located in its initial cure location and the specimens did not
appear to have been tampered with.The specimens were secured inside the Krazan company vehicle and transported to the
Krazan laboratory in Poulsbo,for further curing and testing.
