Environmental Checklist and other information on file with the Lead Agency. This information is

Thank you for the opportunity to comment on the determination of nonsignificance for the
Crown Auto Cleanup project(SEP2014-00057) located at 23771 Northeast State Route 3 in
Belfair as proposed by Crown Auto Sales. The Department of Ecology(Ecology) reviewed the
environmental checklist and has the following comment(s):
TOXICS CLEANUP: Joyce Mercuri(360) 407-6260
A report of the independent remedial action must be submitted to Ecology within ninety days
of completion of the cleanup action. The applicant should refer to WAC 173-340 (the Model
Toxics Control Act regulation) sections WAC 173-340-450 and WAC 173-340-515 for
specific reporting requirements. The cleanup report should be submitted to Carol Johnston,
Leaking Underground Storage Tank Cleanup Project Manager. Under WAC 173-340-450,
the applicant is also required to notify Ecology of their intention to begin cleanup. To our
knowledge,'no such notification was formally made by the applicant. If the applicant desires
an opinion from Ecology on the adequacy of the cleanup,they can obtain information about
the Voluntary Cleanup Program at the following internet link:

2. Clearing, grading and/or excavation on sites smaller than one acre that are part of a
larger common plan of development or sale, if the common plan of development or
sale will ultimately disturb one acre or more and discharge stormwater to surface
waters of the State.
a) This includes forest practices (including,but not limited to, class IV conversions)
that are part of a construction activity that will result in the disturbance of one or
more acres, and discharge to surface waters of the State; and
3. Any size construction activity discharging stormwater to waters of the State that
Ecology:
a) Determines to be a significant contributor of pollutants to waters of the State of
Washington.
b) Reasonably expects to cause a violation of any water quality standard.
If there are known soil/ground water contaminants present on-site, additional information
(including,but not limited to: temporary erosion and sediment control plans; stormwater
pollution prevention plan; list of known contaminants with concentrations and depths found;
a site map depicting the sample location(s); and additional studies/reports regarding
contaminant(s))will be required to be submitted.

Project Overview PLANNING 6215 .Wo CEDAR ST.
1. This independent remedial action is conducted in accordance with the Model Toxics Control Act
(MTCA).
2. Work includes excavation of two source areas on the subject property and impacted soil
beneath a parking lot on the downgradient property to the south.The excavation areas include:
a. Waste oil aboveground storage tank(AST)excavation area:The waste oil AST excavation
area is adjacent to the single-story building and the AST has been removed.An
underground telephone line shall be removed and replaced.The excavation shall be slot
cut to approximately 10 feet below ground surface (bgs) and filled with controlled
density fill (CDF)during excavation. Approximately 100 tons of soil shall be excavated
from the waste oil AST excavation area.The maximum detected concentration of oil-
range total petroleum hydrocarbons (TPH-0)was 28,000 milligrams per kilogram
i .
(mg/kg) in the 6 to 7 foot interval beneath the former waste oil AST The soil from the
waste oil AST excavation area has a contained-out determination from the Department
of Ecology(Ecology).
b. Septic field excavation area:The septic field area is partially beneath a drive-through
canopy that is approximately 20 feet above ground surface.A water supply line and a
sanitary sewer line shall be removed and replaced during excavation and backfill as
warranted.Approximately 400 tons of soil shall be excavated from the septic field
excavation area to depths up to 10 feet bgs.The septic field contains chlorinated solvent
contamination.The most contaminated soil sample was from the 2 to 3 foot bgs interval
and contained 0.081 mg/kg of tetrachloroethylene(PCE)and 0.037 mg/kg of
trichloroethylene (TCE). Higher concentrations may be encountered based on
groundwater sampling results, where 0.31 milligrams per liter(mg/L)of vinyl chloride
were detected from the excavation area.The septic field soil has a contained-out
determination from Ecology.
c. TPH excavation area:The TPH excavation area is located beneath the asphalt-paved
parking lot.The excavation area is bound by a storm drain on the east side,which
conveys an unknown creek through the development, and by the underground utilities
in the right-of-way on the north side of the parking lot.Approximately 1,000 tons of
petroleum-contaminated soil (PCS) shall be excavated at depths up to 8 feet bgs.The
PCS from the TPH excavation area was not included in the contained-out request and
determination.
d. The excavation areas may be expanded based on observation,field screening, and
confirmation sampling.
e. The contained-out determination for Ecology(dated September 2, 2014)specifies that
up to 1,500 tons of soil can be disposed to a permitted Subtitle D Waste Management or
Allied Waste Landfill.Trucks shall be plastic lined, and during transport, all loads must
be covered to prevent wind dispersion.
3. The depth to groundwater is anticipated to be about 5 feet bgs.
Site Preparation
1. Five monitoring wells(MW-9, MW-10, MW-11, MW-21,and MW-22)within the excavation
areas shall be decommissioned by a licensed driller in accordance with WAC Chapter 173-160
prior to remedial action.
2. Prior to construction,the remediation contractor shall:
a. Complete waste profile for contained-out soil for disposal at a Subtitle D Waste
Management or Allied Waste landfill and coordinate waste transport to allow direct-
loading of contained-out soil for offsite disposal.
b. Complete waste profile for PCS prior to excavation and coordinate waste transport to
allow direct-loading of PCS for offsite disposal.
c. Receive Grading Permit from Mason County for remedial action and abide by all
required plans and provisions.
d. Prepare Health and Safety Plan for remedial action and abide by all applicable
requirements.
3. During construction,the remediation contractor shall:
a. Install and maintain Temporary Erosion and Sediment Controls(TESC) as defined in
these plans.
b. Provide temporary fencing and mainta.in site security to prevent access to the
excavation,to contaminated materials, and to potential hazards.
c. Maintain designated work zones and ensure that health and safety protocols are
followed.
d. Maintain a means of egress for emergency vehicles.
e. Minimize disruptions to Crown Auto Sales and Dairy Queen restaurant drive-through.
f. Maintain copies of all applicable permits and plans at the construction site.
g. Protect existing structures, utilities, and infrastructure,and repair any damage as soon
as practicable at sole expense of contractor.
4. Following construction,the remediation contractor shall:
a. Restore site to its original condition, remove construction equipment, clean
construction areas and parking surfaces,and remove TESCs.
b. Maintain and provide copies of waste manifests.
c. Maintain and provide a daily log of activities.
Excavation and Grading Specifications for Waste Oil AST and Septic Field Excavation Areas
1. Utility locate shall be performed prior to excavation and subsurface utilities shall be
disconnected as necessary to perform excavation activities.All subsurface utilities shall be
restored to their original condition following excavation and backfill activities.Tracer wire shall
be placed above non-metallic pipes..
2. Soil staging pile shall be constructed on a topographically high location,as feasible,to hold
approximately 500 tons of contained-out soil.Soil staging pile shall meet specifications of soil
staging piles in the Erosion and Sediment Control Specifications. Soil staging pile shall be
covered with minimum 4-mil plastic sheeting at the end of work day or when precipitation
occurs. Plastic sheeting shall be secured with sand bags or weighted material.
3. Excavation area shall be offset a minimum of 1 foot from the building and from the concrete
island beneath the canopy that contains the support columns.
4. Excavations adjacent to building shall be performed using a maximum 3-foot wide slot cut with a
backhoe.The slot shall be filled with CDF to 3 feet bgs and the CDF shall be allowed time to
solidify before adjacent slots are excavated. CDF shall be prepared by mixing imported structural
fill with Portland cement and water.The CDF shall be self-leveling and self-compacting. CDF shall
be prepared to support the surrounding soil and structure within 4 hours after placement and
shall be capable of being excavated with hand tools after one year of hardening.The minimum
unconfined compressive strength is 50 pounds per square inch (psi) after 4 hours and the
maximum unconfined compressive strength is 100 psi after one year of hardening.The Portland
cement content for excavatable CDF is typically in the range of 30 to 50 pounds per cubic yard.
No acceptance testing is planned.
5. All other excavations shall have a minimum slope of 1.5:1(horizontal:vertical) on the sidewalls
adjacent to the building and 1:1 (horizontal:vertical)on the remaining sidewalls without vertical
loading.
6. Excavation confirmation samples shall be collected and evaluated using a photo-ionization
detector(RID). Confirmation samples shall be collected from the bottom and sidewalls of the
excavation areas and shall be submitted to an analytical laboratory for the analysis of the
chemicals of concern (COC)with a 24-hour turnaround time.
7. Excavation shall be performed beneath the static water table to the extent practicable up to
10 feet bgs. Saturated soil shall be staged within the excavation and allowed to drain before
placement in the staging pile or direct-loaded for offsite disposal.
8. Subject to discharge approval by the Director of Utilities and Waste Management in Mason
County, excavation shall be dewatered from a constructed sump and groundwater shall be
pumped into 20,000 gallon storage tanks.Water shall be pumped through bag filters and
granular activated carbon (GAC) and discharged to the sanitary sewer under discharge
authorization by Mason County. Representative water samples shall be collected and submitted
to an analytical laboratory for analysis of permit parameters.
9. If wastewater discharge approval is not granted, excavation shall be dewatered using a vacuum
truck as warranted and petroleum contaminated water shall be appropriately disposed.
10. Quarry spalls shall be placed from the bottom of the excavation to approximately 5 feet bgs(or
above the high groundwater table). Quarry spalls shall be compacted with earthmoving
equipment during placement.
11. Geotextile meeting specifications for separation in WSDOT(2012)Specification 9-33.2(1),
Table 3 shall be placed on top of the quarry spalls with a minimum 2-foot overlap and non-
sealed seams.
12. Excavation above the geotextile at about 5 feet bgs shall be backfilled with gravel borrow
(WSDOT, 2012,Specification 9-03.14(1)) in maximum 6 inch lifts.Soil shall be compacted to 95%
maximum dry density above geotextile.The surface shall be rough graded to match existing
contours and to drain water away from the structure.
13. A minimum 3-inch thick asphalt surface shall be constructed on top of the excavation areas.The
asphalt surface shall be completed flush with the surrounding surfaces and shall drain water
away from the structures.
Excavation and Grading Specifications for TPH Excavation Area

2. Excavation shall be expedited in the parking lot west of Dairy Queen to minimize business
disruption.
3. Soil staging pile shall be constructed in the parking lot on a topographically high location, as
feasible,to hold approximately 500 tons of PCS. Soil staging pile shall meet specifications of soil
staging piles in the Erosion and Sediment Control Specifications.Soil staging pile shall be
covered with minimum 4-mil plastic sheeting at the end of work day or when precipitation-
occurs. Plastic sheeting shall be secured with sand bags or weighted material.
4. Excavation area shall be offset a minimum of 5 feet from the storm drain on the west side of the
Dairy Queen.This'storm drain conveys an unnamed creek through the development. Excavation
area shall be offset a minimum of 5 feet from underground utilities on the north side of the
parking lot.
5. Excavation shall have a minimum slope of 1:1 (horizontal:vertical)on the periphery of the TPH
excavation area.
6. Excavation confirmation samples shall be collected and evaluated using a PID.Confirmation
samples shall be collected from the bottom and sidewalls of the excavation areas and shall be
submitted to an analytical laboratory for the analysis of the COCs with a 24-hour turnaround
time.
7. Excavation shall be performed beneath the static water table to the extent practicable up to
8 feet bgs.Saturated soil shall be staged within the excavation and allowed to drain before
placement in the staging pile or direct-loaded for offsite disposal.
8. Subject to discharge approval by the Director of Utilities and Waste Management in Mason

14. Parking spaces shall be re-painted with conforming divisions and include similar handicap
markings as warranted.
Erosion and Sediment Control Specifications
1. Silt fencing shall be installed along grassy swale on northeast side of Crown Auto Sales parking
lot in accordance with BMP C233 (Silt Fence).
2. Catch basin filters shall be installed in the catch basins in the parking lot with the TPH excavation
area in accordance with BMP C220(Storm Drain Inlet Protection). Catch basin filters shall be
used for duration of soil disturbing activities. Catch basin filters shall be monitored during
construction activities and after storm events and shall be replaced when they are more than
1/3 full.
3. Soil stockpile shall be lined with a minimum 10-mil plastic liner that is free of holes,tears, or
other defects that compromise the impermeability of the material.Soil shall not be placed
within 2 feet of edge of the liner.At the end of the day and when soil is unworked,the edges of
the liner will be folded over the stockpiled soil and the liner shall be secured with sand bags or
other weighted material.The stockpiled soil shall be covered with minimum 4-mil plastic
sheeting in the event of rainfall and secured during work stoppage.
4. Sand bags shall be placed to prevent stormwater from flowing into the excavation, as
warranted.
S. No sediment shall be tracked onto paved streets or roadways. In the event of tracking onto
pavement,the contractor shall implement measures immediately to correct the situation.
6. Paved surfaces at the soil loading area shall be swept clean on a regular basis.
7. Parking lot shall be cleaned as warranted using a vacuum street sweeper..
Standard Temporary Erosion and Sediment Control (TESC) Notes
1. The implementation of these TESC plans and the construction,maintenance, replacement, and
upgrading of TESC facilities is the responsibility of the applicant/contractor until all construction
is completed and approved,vegetation/landscaping is established, and the entire site is
stabilized.
2. The boundaries of the clearing limits shown on this plan shall be clearly flagged in the field prior
to construction. During the construction period, no disturbance beyond the flagged clearing
limits shall be permitted.The flagging shall be maintained by the applicant/contractor for the
duration of construction.
3. The TESC facilities shown on this plan shall be constructed prior to and/or in conjunction with all
clearing and grading activities, and in such a manner as to ensure that sediment and sediment
laden water do not enter the drainage system or roadways,or violate applicable water
standards.
4. The TESC facilities shown on this plan are the minimum requirements for anticipated site
conditions. During the construction period,TESC facilities shall be upgraded as needed for
unexpected storm events and to ensure that sediment and sediment-laden water do not leave
the site.
5. The TESC facilities shall be inspected daily by the applicant/contractor and maintained as
necessary to ensure their continued function.
6. The TESC facilities on inactive sites shall be inspected and maintained a minimum of once a
month or within the 48 hours following a major storm event or as necessary to ensure their
continued function.
7. At no time shall more than one foot of sediment be allowed to accumulate within a catch basin
sediment trap.
8. All catch basins and conveyance lines shall be cleaned prior to paving.The cleaning operation
shall not flush sediment-laden water into the downstream system.
9. Stabilized construction entrances shall be installed at the beginning of construction and
maintained for the duration of the project.Additional measures may be required to ensure that
all paved areas are kept clean for the duration of the project.

Purpose of Checklist:
The State Environmental Policy Act(SEPA), chapter 43.21 C RCW, requires all governmental agencies to consider the
environmental impacts of a proposal before making decisions. An environmental impact statement (EIS) must be
prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of
this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or
avoid impacts from the proposal)and to help the agency decide whether an EIS is required.
