An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal)and to help the agency decide whether an EIS is required. Instructions for Applicants: Governmental agencies use this checklist to determine whether the environmental impacts of your proposal are significant, requiring preparation of an EIS. Answer the questions briefly, with the most precise information known, or give the best description you can. You must answer each question accurately and carefully, to the best of your knowledge. In most cases, you should be able to answer the questions from your own observations or project plans without the need to hire experts. If you really do not know the answer, or if a question does not apply to your proposal, write "do not know" or "does not apply". Belfair Stormwater Retrofit LID, 90% Design Narrative, Appendix F, DOE Wetland Correspondence. Presents email correspondence regarding potential project impacts to wetland hydrology. There are several questions later in this checklist that ask you to describe certain aspects of your proposal. You do not need to repeat those answers on this page. (Lead agencies may modify this form to include additional specific information on project description.) The proposed project includes four stormwater retrofits designed to improve water quality within the Irene Creek drainage area of Belfair. The four retrofits include: Retrofit#1 Roy Boad Road Ditch Improvements Untreated stormwater currently flows in the Roy Boad Road ditch to a 24-inch culvert under Roy Boad Rd., and discharges to a wetland. The ditch will be widened and amended for treatment through infiltration, and future ditch flows will be directed to the Belfair Valley Plaza pond for further treatment. Retrofit#2 Belfair Valley Plaza Detention Pond The Belfair Valley Plaza detention pond currently is not designed to provide a permanent pool (wet storage)or treatment capabilities. This project will grade the pond to the maximum extent feasible to incorporate 4.5 feet of Mason County SEPA Checklist 2 pre-settling depth in the inlet bay(Cell #1). The outlet bay(Cell #2)will be constructed into a stormwater wetland to improve hydro-carbon, pesticide and fungicide, and dissolved metals removal. Retrofit#3 NE Old Belfair Highway Ditch Improvements A portion of the eastern ditch along NE Old Belfair Highway(approximately 200 lineal feet)will be reconstructed to install a Media Filter Drain to treat pollution generating flows from the roadway. Retrofit#4 Biofiltration Swale( Detention Pond Outlet) An existing bio-filtration swale is located at the north-west corner and outlet of the Belfair Valley Plaza detention pond. The swale provides a westerly flow route to the NE Old Belfair Highway ditch. The bio-filter will be graded to allow for a consistent 5-foot bottom width, amended soils added, and re-seeded. A secondary ditch will be provided adjacent to the swale to collect existing surface runoff from the pasture area adjacent to the pond. 13. What is the location of the proposal? Give sufficient information for a person to understand the precise location of your proposed project including a street address, if any, and section, township, and range, if known. If a proposal would occur over a range of area, provide the range or boundaries of the site(s). Provide a legal description, site plan, vicinity map, and topographic map, if reasonably available. While you should submit any plans required by the agency, you are not required to duplicate maps or detailed plans submitted with any P PP permit applications related to this checklist. Retrofit#1 is located along Roy Boad Road between the intersection of Clifton Lane and the Safeway entrance to Belfair Valley Plaza. Retrofit#2 is located on Tax Parcel #123291490112 which lies just north of the intersection of Clifton Lane and Roy Boad Road, and east of NE Old Belfair Highway. Retrofit#3 is located along the east ditch of NE Belfair Highway, in front of Tax Parcel #'s 123291490110 and 123291490111. Retrofit#4 is located within a drainage easement located on Tax Parcel #'s 123291490110 and 123291490111. B. ENVIRONMENTAL ELEMENTS 1. EARTH: a. General description of the site(circle one): Flat, rolling, hilly, steep slopes, mountainous, other(explain). Topographic relief from the upstream end of Roy Boad Road to NE Old Belfair Highway is 22 feet, with the general project area sloping from southeast to northwest. b. What is the steepest slope on the site(approximate percent slope)? The steepest slope will be located within the detention pond, and will not exceed a 2H:1 V slope. c. What general types of soils are found on the site(for example, clay, sand, gravel, peat, muck)? If you know the classification of agricultural soils, specify them and note any prime farmland. A soils investigation has not been conducted at this time. Pond soils are classified by NRCS as (Sa) Saxon silt loam soils. d. Are there surface indications or history of unstable soils in the immediate vicinity? No. If so, describe. j Mason County SEPA Checklist 3 L e. Describe the purpose, type, and approximate quantities of any filling or grading proposed. Indicate source of fill. Estimated pond excavation and shaping is 550 cubic yards. The purpose of excavation and shaping is to provide permanent storage, allow for pre-settling in Cell #1, and provide wetland treatment of stormwater. f. Could erosion occur as a result of clearing, construction, or use? Yes. If so, generally describe. Roy Boad Road ditch improvements and detention pond improvements will create slopes as steep as 2H:1 V and are considered a potential erosion problem. g. About what percent of the site will be covered with impervious surfaces after project constructions (for example, asphalt or buildings)? No new impervious surfaces will be created as a result of this project. h. Proposed measures to reduce or control erosion, or other impacts to the earth, if any: Erosion control measures are detailed in the attached Construction Stormwater Pollution Prevention Plan, prepared by AHBL, Inc. dated August 2010. 2. AIR: a. What types of emissions to the air would result from the proposal (i.e. dust, automobile, odors, industrial wood smoke)during construction and when the project is completed? If any, generally describe and give approximate quantities, if known. During construction, air emissions associated with construction equipment will result from the proposal. No air emissions are expected after construction. b. Are there any off-site sources of emissions or odor that may affect your proposal? If so, generally describe. No c. Proposed measures to reduce or control emissions or other impacts to air, if any: None. 3 WATER: a. Surface: 1) Is there any surface water body on or in the immediate vicinity of the site(including year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe type and provide names. If appropriate, state what stream or river it flows into. There are no streams, saltwater, lakes, or ponds in the immediate vicinity of the proposed project. An unnamed wetland lies within 100 feet of the Roy Boad Road retrofit work. This wetland is connected to Irene Creek. Please refer to Appendix F of the attached 90% Design Narrative prepared by AHBL, dated August 2010. 2) Will the project require any work over, in, or adjacent to(within 200 feet)the described waters? If yes, please describe and attach available plans. No. Mason County SEPA Checklist 4 3) Estimate the amount of fill and dredge material that would be laced in or removed from surface water or P i wetlands and indicate the area of the site that would be affected. Indicate the source of fill material. None. 4) Will the proposal require surface water withdrawals or diversions? Give general description, purpose, and approximate quantities if known. No. 5) Does the proposal lie within a 100-year floodplain? No. If so, note location on the site plan. 6) Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. No. b. Ground: 1) Will ground water be withdrawn, or will water be discharged to ground water? Give general description, purpose, and approximate quantities, if known. No ground water will be withdrawn. 2) Describe waste material that will be discharged into the ground from septic tanks or other sources, if any (for example; domestic sewage, industrial, containing the following chemicals..., agricultural, etc.). Describe the general size of the system, the number of such systems, the number of houses to be served (if applicable), or the number of animals or humans the system(s)are expected to serve. None. c. Water runoff(including storm water): 1) Describe the source of runoff(including storm water)and method of collection and disposal, if any(include quantities, if known). Where will this water flow? Will this flow into other waters? Runoff sources include portions of NE Old Belfair Highway, Roy Boad Road, a portion of SR 3, the Looney Bin Tavern, a chiropractor's office, and Belfair Valley Plaza. Total tributary area is calculated to be 10.77 acres, of which 1.81 acres are considered pervious. Stormwater is collected in both open ditches and piped conveyances that will lead to the Belfair Valley Plaza detention pond. The pond has an outlet to a bio-swale that flows westerly to the NE Belfair Highway ditch. Water then flows under the Highway continuing west towards Irene Creek. Irene Creek then meets with the Union River and flows to Hood Canal. 2) Could waste materials enter ground or surface waters: Yes. If so, generally describe. Accidental spills, intentional dumping, illicit discharges, failing septic systems are examples of potential waste materials that may enter ground or surface waters. Mason County SEPA Checklist 5 d. Proposed measures to reduce or control surface, ground, and runoff water impacts, if any: Ditch enhancements are proposed to provide some treatment and infiltration capablilities. Pond enhancements are proposed to provide treatment of stormwater prior to infiltration and discharge to bio-filtration swale. Public information and education efforts aimed at source control measures are planned to help prevent pollutants from entering surface waters. 4. PLANTS: a. Vegetation will be removed and altered within the detention pond, along the bio-filtration swale, and sections of the roadside ditches of NE Old Belfair Highway and Roy Boad Road. The type of vegetation anticipated to be removed includes: grass, blackberry and small trees (alder). c. List threatened or endangered species known to be on or near the site. None. d. Proposed landscaping, use of native plants, or other measures to preserve or enhance vegetation on the site, if any: Vegetation will be enhanced within the detention pond by planting wetland species, including the use of native plants. 5. ANIMALS a. Circle any birds and animals which have been observed on or near the site or are known to be on or near the site: Birds: hawk, heron, eagle, songbirds, other Mammals: deer, bear, elk, beaver, other Fish: bass, salmon, trout, herring, shellfish, other _ b. Describe any structures on the site. On-site structures include stormwater features such as outlet control, catch basins, manholes,piping and fencing. d. Will any structures be demolished? Yes. If so, what? Fencing will be removed and replaced after construction completed. e. What is the current zoning classification of the site? mixed use, general commercial f. The stormwater pond can be accessed by either of two easements crossing private property: primary access is from NE Old Belfair Highway, and a second easement exists at the intersection of Roy Boad Road and Clifton Lane. II� i I b. Is site currently served by public transit? N/A If not, what is the approximate distance to the existing street system? Show on site plans, if any. c. How many parking spaces would the completed project have? None. How many would the project eliminate? None. d. Will the proposal require any new roads or streets, or improvements to existing roads or streets, not including driveways? If so, generally describe(indicate whether public or private). No new streets or roads will be required, although improvements will be made to a portion of the ditch along the east side of NE Old Belfair Highway, Roy Boad Road, and a portion of the Chiropractor's parking lot lying within the Roy Boad Road right- of-way. NE Old Belfair Highway improvements: Presently, highway runoff is collected by dirt and grass lined ditches. This project will provide water quality treatment designed to reduce downstream impacts to the Union River and Hood Canal. Approximately 200 lineal feet of ditch will be replaced with a Media Filter Drain sized per PP Y P e June 2008 WSDOT Highway Runoff Manual. Roy Boad Road improvements: Currently, a portion of SR 3, the Looney Bin Tavern, a chiropractor's office, Roy Boad Road, and a Safeway entrance at Roy Boad Road discharge stormwater to a wetland without use of water quality best management practices. This project will eliminate untreated stormwater discharge to the wetland by directing surface water flows to the existing Belfair Valley Plaza stormwater pond. Roy Boad Road will be improved by regrading a portion of the ditch and constructing an underdrain system to achieve treatment and infiltration prior to discharge to the Belfair Valley Plaza stormwater pond. Mason County SEPA Checklist 10 Chiropractor's office parking lot improvements: At this time, a portion of the parking lot discharges untreated stormwater to the wetland. The parking lot will be modified by replacing an existing grated structure with a solid lid to prevent untreated stormwater from entering the wetland. A new catch basin will be provided to collect the parking lot runoff and direct runoff to the Belfair Valley Plaza pond. e. Will the project use(or occur in the vicinity of)water, rail, or air transportation? No. If so, generally describe. f. How many vehicular trips per day would be generated by the completed project? If known, indicate when peak volumes would occur. N/A g. Proposed measures to reduce or control transportation impacts, if any: None. 15. PUBLIC SERVICES: a. I understand that the lead agency is relying jon them to make its decisio Applicant Signature: Date: S o I I I Mason County SEPA Checklist 11 ESA LISTED SALMONIDS CHECKLIST This worksheet was designed to help project proponents, and government agencies, identify when a project needs further analysis regarding adverse effects on ESA (Endangered Species Act) listed salmonids. Salmonids are salmon, trout and chars, e.g. bull trout. For our purposes, "ESA Listed Salmonids" is defined as fish species listed as endangered, threatened or being considered for listing. If ESA listed species are present or ever were present in the watershed where your project will be located, your project has the potential for affecting them, and you need to comply with the ESA. The questions in this section will help determine if the ESA listings will impact your project. The Fish Program Manager at the appropriate Department of Fish and Wildlife (DFW) regional office can provide information for the following two questions. 1. Are ESA listed salmonids currently present in the watershed in which your project will be located?YES NO Please describe. 2.
