1) Final approval on SWG2013-00139 must have final approval of installation prior to temporary/final occupancy.

3) All construction and demolition debris must be removed from the shore area after project completion. Proper disposal of construction debris must
be on land in such a manner that debris cannot enter or cause water quality degradation of State waters. X

18) 3000 PSI CONCRETE WILL REQUIRE SPECIAL INSPECTION IF THE QUANTITY EXCEEDS 50 CUBIC YARDS< LESSER AMOUNTS WILL
REQUIRE AN APPROVED CONCRETE SUPPLIER TO PROVIDE YOU WITH A BATPH TICKET THAT WILL BE REQUIRED TO BE SUBMITTED
TO THE SITE INSPECTOR FOR THE VERIFICATION OF MATERIAL USED. X
19) Structural observation shall be performed and structural observation report(s) shall repared in accordance with Section 1704.5 of the current
International Building Code. Structural observation is the visual observation of the structural system by a registered design professional for general
conformance to the approved construction documents.
Report(s) shall be provided to the Mason County Building Department at 426 West Cedar street, Shelton, WA 98584.
Reports shall be submitted to the Mason County Building Department prior to framing inspection. A final observation report shall be submitted prior
to the final inspection of the structure. Written statements shall indicate the permit number, type of inspection, and identify any reported
deficien ies.,

20) In addition to the inspections required in IBC, Section 109, the owner, the engineer or architect of record acting as the owner's agent shall employ
one or more special inspectors who shall provide inspections during construction on the types of work listed under Chapter 17 and as specified by
the design professional. The special inspectors duties & responsibilities shall be as specified in Chapter 17.
1.) Field welding must be performed by a W.A.B.O. certified welder.
2.)A325 High strength bolts.
Special inspection reports shall be submitted to the Mason County Building Department, PO Box 279, Shelton WA 98584 and available for
inspection. Inspection reports shall be completed and submitted to the dept. in a timely manner and shall be submitted prior to the framing and
final oc inspections.

21) Any ch nges in construction shall be reviewed by engineer of record and submitted in writing to the Mason County Building Department prior to
construction. All engineering documents are a part of the approved set of plans and must remain attached thereto. If engineering documents are
removed, approval will not be granted. In addition, a reinspection fee, based on the current fee schedule, minimum one-hour will be charged and
collecked by the Mason County Building Department prior to any further inspections being performed or approvals granted.

22) ALL ONSTRUCTION MUST MEET OR EXCEED ALL LOCAL CODES AND THE INTERNATIONAL CODE REQUIREMENTS AND OCCUPANCY
IS LIMITED TO THE PERMITTED AND APPROVED CLASSIFICATION. ANY CHANGE OF PSE OR OCCUPANCY WOULD RESULT IN PERMIT
REVOCATION. CHANGE OF USE MUST BE APPROVED PRIOR TO CHANGE. x
23) Provisions for surface/subsurface drainage control must be implemented with new con ction or development on site and MUST NOT adversely
impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, either private ditches and drains will meet requirements
of the stormwater ordinance or prior approval will be granted to use an existing utility and drainage easement dedicated for that specific purpose.
For further information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a
driveway or access connecting from a Mason County Road, Contact the Mason County Public Works Department prior to construction at Ext 450.
For any construction which is proposed to be located within 25' of a Mason County road right of way, it is suggested to contact that office to review
future planned wQrk which,may affect your project.

24) Changes to a�fbved building plans that affect compliance to the current Washington State Energy Code (WSEC), ventilation requirements),
Buildin /Plu �i /Mechanical Codes and/or Mason County Regulations shall be approved prior to construction.

25) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE
ADOPTED BUILDING CODE.
The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in
conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a
Mason County uilding Inspector shall be made prior to requesting additional inspections.

26) All bui w g permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The
failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being
non-com liant with ),Mason County ordinances and building regulations.

27) All per s expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the
time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control

29) ACC IBLITY EXCEPTION:
Each toilet and bathing room shall be designed and constructed to be accessible to persons with disabilities in accordance with the 2012
International Code and ICC A117.1. Referencing IBC Section 1109 the following exception applies. All elements listed shall apply to qualify for the
exception.
EXCEPTION:
In toilet rooms or bathing rooms accessed only through a PRIVATE OFFICE, not for common or public use, and intended for use by a single
occupant, the following alternatives are allowed:
1) Doors are permitted to swing into the clear floor space, provided the door swing can be reversed to meet the requirements in ICC A117.1;
2)The height requirements for the water closer in ICC A117.1 are not applicable;
3) Grab bars are not required to be installed in a toilet room, provided the reinforcement has been installed in the walls and located so as to
permittee installation of grab bars; and
4)The r guir ent for height, knee and toe clearance shall not apply to a lavatory.

30) COMMERCIAL KITCHEN HOOD:
All commercial kitchen hood systems shall be protected with a UL300 compliant automatic fire extinguishing system installed in accordance with
manufacturer specifications and maintained as required by the 2012 International Fire Code, Building Code, and International Mechanical Code.
The system shall be inspected and tested for proper operation at 6-month intervals unless approved otherwise. Maintenance records shall be
available for inspectors during inspections.
As per IMC table 507.2.2 as amended by Washington State the minimum hood required for a church that will be boiling, steaming, and warming
precookeed only is a type II hood.

31) FIRE EXTINGUISHERS:
Install 2A10BC fire extinguishers throughout the building in accordance with chapter 9 of the 2012 International Fire code. The fire extinguishers
shall be installed such that the extinguishers are located not more than a travel distance of 75 feet in any direction and mounted no more than 60
inches above floor to the top of the unit. At least one extinguisher is required on every level or floor.
Not less than 1 type K portable fire extinguisher shall be located in the commercial kitchen area, within 30 feet of the cooking appliances and no
closer than 10 f et.

KEY B REQUIRED:
A key box/knox box is required to be installed in accordance with the 2012 International Fire Code Section 506. Please contact the local fire
district for mor information and to discuss inspections for installation of the lock box system.

32) ADDRES ONDITION

Commercial structure mile point number or unit designation posting requirements:
50 feet or less setback shall post a 6 inch minimum with a 3/ inch stroke.
50 feet or more setback shall post a 12 inch minimum with a 1 'h inch stroke.
Designating unit on a building will be no less than 6 inches in height with minimum 3/ inch stroke.
Designating unit on a door will be no less than 4 inches in height with minimum '/2 inch stroke 100 foot setback or less shall post 18 inch minimum
with 2 inch stroke.

33) POS CCUPANT LOAD
Every room or space that is an assembly occupancy, such as a restaurant, bar, or meeting room, shall have the occupant load of the room or
space posted in a conspicuous place, near the main exit or exit access doorway from the room or space. Posted signs shall be of an approved
legible p rmaperlt design and shall be maintained by the owner or agent. (IBC1004.3)

34) EXITS IT ACCESS DOORS
All doors shall be side hinged and swing in the direction of exit travel. All exit doors shall be illuminated at all times that the building is occupied and
be equipped with approved accessible hardware that does not require tight grasping, tight pinching or twisting of the wrist to operate.
ALL exits and exit access doors shall be marked by an approved exit sign readily visible from any direction of egress travel. All exit signs shall be
intern Ily or externally illuminated at all times and equipped with to ensure continued illumination for a duration of at least 90-minutes.

35) 2012 IECC/WSEC COMPLIANCE:
This project is approved for compliance to the 2012 International Energy Conservation Code/Washington State Energy Code (IECC/WSEC);
components shall be installed as follows:
ENVELOPE: Prescriptive Using Table C402.2
Roof: Metal building with R3.5 thermal blocks, R-11 liner system PLUS R25 continuous non-compressed insulation
Wall insulation min R13 continuous insulation + R-13 wall insulation or better,
Slab insulation R-10 or better installed in around the entire perimeter from the top of the slab edge to the bottom of the footing.
Fenestration including doors and windows, including store door(s) shall have a U-Factor as follows-fixed/non operable windows .38 and operable
windows U-0.40or less, entrance doors U0.60 or less and skylights U-0.50 or less. Solar heat gain coefficient(SHGC) 0.40 or better except
skylights SHGC 0.35 or better.
OTHER DOORS: Other than entry doors U-0.37 or less and roll-up or sliding doors min. R-4.75.
DEFERRED SUBMITTAL IS APPROVED FOR ELECTRICAL POWER AND LIGHTING SYSTEMS and the MECHANICAL SYSTEM.
COMPLIANCE INFORMATION SHALL BE SUBMITTED TO THE MASON COUNTY BUILDING DEPARTMENT AND APPROVAL OBTAINED
BEFORE THE FRAMING INSPECTION.
THE FRAMING INSPECTION WILL NOT BE APPROVED UNTIL DEFERRED SUBMITTAL DOCUMENTS ARE APPROVED AND ISSUED BY
THE MASON COUNTY BUILDING DEPARTMENT.
LIGHTING: DEFERRED SUBMITTAL
Total interior lighting shall not exceed 4637 watts, or provide a design calculation demonstrating compliance to IECC/WSEC C405. Lamp wattage
shall be calculated using the lamp and ballast or for incandescent fixtures, using the maximum labeled wattage allowed and is approved as follows:
Provide a lighting plan showing the layout of all lights and controls along with a schedule that identifies the wattage for ballast and lamps. The plan
and information shall be submitted to the Mason County Building Department for review and approval prior to the framing inspection.
All daylight zones shall be provided separate control in accordance with IECCWSEC. Automatic daylight sensing controls shall be capable of
reducing light out put, that control luminaries only within the day lighted area, shall be clearly identified. Switches shall be equipped with separate
automatic controls to shut off the lighting in all spaces during unoccupied hours. Occupancy sensors shall be capable of automatically turning off
all lights in an area in accordance with IECC/WSEC.
Exterior lighting shall not exceed 750 watts or provide a design calculation demonstrating compliance to IECC/WSEC C405 or as otherwise
allowed in the IECC/WSEC. A lighting plan and summary shall be submitted to the Mason County Building Dept. for review and approval prior to
the framing inspection.
MECHANICAL SYSTEM: DEFERRED SUBMITTAL
A mechanical plan was not included with the plans submitted for approval. Deferred submittal shall include compliance forms attached to the
approved construction plans, details and information about equipment and system, along with plans demonstrating compliance to IECC/WSEC
C403. Mechanical systems and equipment serving heating, cooling, ventilating, and other needs with Section C403.2 and either C403.3 or C403.4.
Walk-in coolers and walk-in freezers shall comply with C403.5. Refrigerated warehouse coolers and refrigerated warehouse freezers shall comply
with C403.6.

MECHANICAL SYSTEM - NON RESIDENTIAL:
Those parts of the mechanical system that are altered or replaced shall comply with Section C403 of the 2012 International Energy Conservation
Code as amended by Washington State (IECC/WSEC), ref C101.4.3.2.
Heating and cooling loads shall be determined. The out put capacity of heating and cooling equipment shall not exceed the load calculated. Heat
and cooling load calculations shall be provided on site for inspection.
HVAC equipment shall meet or exceed the minimum efficiency requirements specified in section C403.2.3 and applicable tables.
Unless approved otherwise:
HSPF rating (heating) shall be 7.7 or better, 3.3 C.O.P or better, and SEER rating (cooling) shall be 13.0 or better
Warm air gas fired furnaces shall have an A.F.U.E rating of 78% or better.

Envirotech Engineering provided a recent letter dated October 8, 2016 (see
enclosed) addressing concerns from Mason County in a letter dated August 15,
2016. This letter further justifies our design and addresses the enclosed
resubmittal per item #15 on the aforesaid County letter.
There are a few discrepancies between our described land use areas with
relation to our stormwater modeling. However, the basis of our original infiltration
pond design utilized an impervious contribution of 1.75 acres, whereas planned
conditions are only 1.17 acres. This 50% increase is extremely conservative, and
provides a pond size adequate for this project. Envirotech reviewed our
calculations utilizing actual impervious areas with the infiltration rate reduced to
2.4 in/hr for both cells of the pond. With a reconfigured cell 1 to meet the
stormwater manual, and the reduced infiltration rate, the infiltration pond is
deemed over-sized after accounting for a 6-inch sediment storage depth and a
12-inch freeboard.
On sheet 1 of the plans, the pond has been reconfigured with a new cross
section showing all pond elements, design criteria and hydraulic profiles. This
includes general topography indicating that the pond is entirely within a cut
section.

Envirotech Engineering is providing this letter to address concerns from Mason
County in a letter dated August 15, 2016. We visited the project site on
September 9, 2016 in order to observe current site conditions. The following
items directly correspond to the aforesaid letter.
1. Sheet 1 of our plans depicted a 10-foot buffer (clearing limits). According
the contractor, it is our understanding that the architectural plans depict
the 10-foot buffer as vegetative landscaping. This is acceptable to
Envirotech as it should be re-vegetated.
2. Per Sheet 2 of our plans, we recommended one construction access off of
State Route 300. Temporary access from Gladwin Road should be
removed by not using, and placing temporary seeding to stabilize the area
per the drainage plan.
3. Stabilizing the Gladwin access and drainfield areas with temporary
seeding will control flow rates. It is our opinion that flow from a 2yr-24hr
construction design storm will disperse and infiltrate prior to leaving the
site. However, the stormwater plans indicates that if drainage is not
functioning as intended, additional provisions/ drainage facilities will be
required such as silt fencing.

4. All sediment/temporary erosion controls should be implemented per the
approved plans. This includes silt fencing as necessary and seeding
unworked denuded areas. It is our opinion that the existing pond is
sufficiently sized for the temporary 2yr-24hr storm event, and may be used
as a temporary sediment pond. Upon project completion, the temporary
pond should be enlarged per the drainage plan.
5. Soil stabilization is provided in the stormwater plans, and should be
implemented for this project. Wet season erosion control provisions are
provided in the plans, and should be completed on or before October 1st
6. Slopes must be protected per the erosion control plan. Vegetation and/ or
plastic cover, as necessary should be implemented.
7. Our initial evaluation was intended to be worst-case-scenario, and that the
natural swale on the west side of the property may possibly convey
surface water. We did not observe scour within the swale, but did notice
some erosion in the right-of-way to the north of Gladwin Road near the
swale end. It is our opinion that the established drainage conveyance
system within State Route 300 is adequate for diverting stormwater away
from the project during storm events. We believe that significant runoff that
could enter the site would most likely be from un-maintained drainage
conveyances upslope from the project or from storms exceeding the
100yr-24hr event, which are not designed for. In conclusion, it is our
opinion that the designed ponds are adequately sized for the new
development plus design storms contributing to the retention pond.
8. In-situ infiltration measurements were not performed. Based on soil type
and experience performing field infiltration tests, we believe that our
infiltration rates based on the USDA classification is much more
conservative than performing in-situ measurements. Sheet 1 of the plans
depicted soil logs for each of the five test pit excavations. No mottling was
observed. The pond test pits were observed to a depth of 8 feet, which is
0.5 feet below the final pond bottom. Envirotech recently hand excavated
to a depth of 3 feet below the bottom of the expected pond depth with no
change in materials to report. An operation and Maintenance Plan was
prepared by Envirotech during the design of this project. An O&M
agreement recording should be a condition of permit by Mason County.
9. Unless upslope drainage facilities beyond the project fail, or severe storms
occur above ordinary design storms (see #7 above), we believe runoff
entering the property is negligible. This is also substantiated by periodic
observations over the past 3 years. It is our opinion that the small portion
of the north-east property corner is not significant of an area to generate
runoff to off-site properties, especially after re-vegetating per plan. Bare
dirt areas of the property are temporary until construction is complete and
the ground is either fully developed or revegetated. Although the
revegetated areas will have a little more runoff potential than pre-existing
site conditions, the additional runoff is negated as the runoff from the
developed areas are controlled. The land area that will be developed is
not contributing runoff as it did during pre-existing conditions.
10.The drainage manual recommends that infiltration facilities are at least
100 ft setback from on-site septic components. This appears to be
adhered to. Infiltration facilities appear to comply to the State Department
of Health wellhead protection program. The proposed drainage facilities
are over 1000 feet from Lynch Cove and Ridgerunner water systems.
Furthermore, the hydrogeologic setting is acceptable for the planned
drainage systems not to contaminate any private well systems.
11 . Factoring the design storms and development areas, the proposed size of
the infiltration facility has a volume relationship similar to other
developments within the area. Envirotech will re-examine the facility
sizing, and if necessary, provide a revised pond size along with a re-
submittal per item #15 below.
12. Based on proponent provided information, the use of this project will
render an approximate average of 30 trips per day of traffic. Other than
the low volume of traffic and ordinary landscaping maintenance, other
pollution generating activities are not anticipated. From the drainage
manual (Vol V), this is not considered a high use site, and basic treatment
is acceptable. Our pre-settling basin deviates from the drainage manual's
recommendation for a 3.1 length to width ratio. Mulching and seeding per
the permanent erosion control notes were intended for the water quality
treatment cell. Per #11 above and #15 below, Envirotech will revise plans,
if necessary to incorporate drainage guidance.
13.The catch basin is mis-labeled, and should be called out as a spill control
separator. The pipe tee in the catch basin was designed as an added
defense prior to runoff entering the basic treatment.
14.The construction sequence recommendations per our design must be
implemented, and Envirotech should be retained to perform inspections
and as-builts per permit requirements.
15.Without further cornments from Mason County, we will provide additional
design plans per the County stormwater management plan cornments
separate from this letter.

This memo is in response to citizen concerns regarding stormwater management for the new church
located off State Route 300 for Iglesia Asemblea de Dios. The concerns as I understand them are as
follows:
1. Site conditions are different from those identified in the stormwater plan, for example the pre-developed
condition assumes the entire site drains towards the southwest property corner and a 10-foot clearing
limit was established for tree preservation and erosion control;
2. The stormwater plan (and on-site septic system location)do not take into account well locations and
aquifer recharge; and
3. The stormwater plan sheets appear to have a smaller sized pond than the calculations would indicate.
recently reviewed the drainage report, CSWPPP, and plan sheets prepared by Envirotech Engineering
(after the fact), and Hearings Examiner Findings, Conclusion and Decision. I also performed a field site
visit. Based on those reviews and site visit, Public Works requests that a certificate of occupancy permit
not be issued until the applicant's engineer has responded to the attached letter and Public Works has
signed off on the final installation of stormwater facilities.

I recently reviewed the construction stormwater pollution prevention plan
(CSWPPP), drainage report,and plan sheets prepared by Envirotech dated
September 2013 and received by the Department of Community Development in
February 2015 and made a site visit. I also reviewed the Hearings Examiner Findings,
Conclusions and Decision of June 2013. Based on the reviews and site visit, I have
the following questions and comments:

The plans indicate a 10-foot tree buffer is to be maintained along Gladwin
Road. The Hearings Examiner record indicates the applicant planned to
retain as many trees and vegetation as possible. However, it appears that
several large fir trees were removed from the buffer and it is unknown
whether the buffer was ever clearly marked.

2. Page 4/7 2.0 Establish Construction Entrance
There are presently two accesses to the site:one off State Route 300 and another off Gladwin
near the intersection with SR 300. According to Public Works records,the Gladwin access has
never been permitted and has not been stabilized according to the plans. This access may only
be used temporarily during construction and if stabilized per plans. It must be removed prior to
issuing a final occupancy permit.

How will flows be controlled in the vicinity of the Gladwin Road temporary construction access
and drainfield location? It appears that runoff will simply flow down the access,across Gladwin
and onto the downslope property. Please identify how these flows will be managed.

Much of the natural site vegetation has been removed and no silt fence has been installed to
date. It is unknown if the temporary sediment pond has been constructed. There are no
measures in place at this time to prevent silt and sediment-laden runoff from leaving the
property during a rain event. The owner and contractor should be prepared for this situation.

USDA Textural Classification. The report does not describe test pits but plan sheet 1f 3 indicates .
five test pits were located on site. Were any in-situ field measurements performed? Was-any
mottling noted? The pits did not extend to the depth of the proposed pond floor elevation. is
there any reason to believe there may be fine silt, clay or other layer that would reduce the
infiltration rate? Given the use of a low correction factor and uncertainties, at a minimum, an
Operation and Maintenance Plan must be prepared and an O&M Agreement recorded prior to
occupancy.

12. Pages 6,7 5.1 Infiltration Pond with Water Quality Cell
Runoff Treatment:
It appears that basic treatment{s provided and the narrative does not describe the treatment
facility selection process. Please describe the treatment selection process.and why basic
treatment only is appropriate.
The plans include a pre-settling basin presumably designed to pretreat runoff from the new
building and parking lots before flowing into the infiltration pond. The pre-settling basin does
not appear to meet the length to width ratio that will allow good settlement to occur. It also
does not have a treatment liner specified. Given the soils and location within the wellhead
protection area it seems that a liner or soil amendment to reduce the infiltration rate.is needed..
Please revise the plans to show the correct dimensions and provide liner or soil amendment
specifications. Please note this will change the infiltration rate and likely require upsking the
pre-settling basin. What is the volume necessary for the pre-settling basin?

Flow Control:
The narrative describes the use of the model and results, but the project report data, narrative
in 5.2 and the plan sheets do not track. It appears that after subtracting the pre-settling volume
the combined volume of cells 1 and 2 on the plan sheets are at least one half the calculated
volume. Please show how the calculations,narrative and plans are consistent with each other..
13. Page 7 5.3 Catch Basin
A pipe tee typically is not an effective way to provide oil/water separation unless an elbow is
installed. The detail shown on page 3/3 is better described as a spill control separator and is not'
used for treatment purposes, but for temporarily trapping small volumes of oil.
14. Page 8 6.0 Construction Sequence
The construction sequence recommendations and procedure need to be implemented. The
commercial permit conditions state that the engineer of record or authorized representative is
to perform inspections to ensure the plan is implemented as designed. An as-built needs to be
submitted to the Department of Community Development prior to occupancy.

managed particularly from the drainfield vicinity?
