Enclosed please find two copies of the form Mason County Public Works is employing to satisfy
Ecology operation and maintenance requirements for engineered stormwater facilities.
The form is intentionally simple. Please fill in the blanks with appropriate responses, sign and
notarize the instrument, record the completed form with the Mason County Auditor's office and
deliver a copy of the recorded document to Public Works for monitoring and administration.
