COM2012-00015 Page 3 of 7 12) CONSTRUCTION PROCESS TO BE FIELD CORRECTED AS REQUIRED PER MASON COUNTY BUILDING DEPARTMENT AND THE ADOPTED BUILDING CODE. The construction of the permitted project is subject to inspections by the Mason County Building Department. All construction must be in conformance with the international codes as amended and adopted by Mason County. Any corrections, changes or alterations required by a Mason C unty Building Inspector shall be made prior to requesting additional inspections. X 13) All property lines shall be clearly identified at the time of foundation inspection. Xi;� 14) All building permits shall have a final inspection performed and approved by the Mason County Building Department prior to permit expiration. The failure to request a final inspection or to obtain approval will be documented in the legal property records on file with Mason County as being non-compliant with Mason County ordinances and building regulations. 15) All permits expire 180 days after permit issuance, or 180 days after the last inspection activity is performed. The Building Official may extend the time for action for a period not exceeding 180 days, upon the receipt of a written extension request indicating that circumstances beyond the control Xtheit holder have prevented action from being taken. No more than one extension may be granted. 16) Pressure treated wood manufactured after January 1, 2004 may contain high concentrations of copper which could quickly corrode metal fasteners, connectors, and flashing. Install metal connectors approved for contact with the new types of pressure treated material. Zoning is Belfair UGA Festival Retail zone. 1VX� Parking shall be sufficient for at least 25 standard parking stalls (9 feet by 20 feet) and 2 handicap parking stalls (12.5 feet by 20 feet)with sufficient maneuvering aisles. Handicap stalls shall be of a smooth surface at level or ramped to entry, located closest to the building entry, and shall be signed with the International Symbol of Access. X14L___ COM2012-00015 Page 4 of 7 23) The type 1 hood and duct along with the heated make up air system is deferred, no work is allowed to be preformed unit reviewed and approved by Mason County. X A soils report is required at the time of the footing inspection, a minimum 1,500 PSF is required per the design engineers calculations. If the report shows a value of less than 1,500 psf, submitted the new report to the design engineer, any changes will need to be resubmitted to Mason County for review and approval. No concrete will be allowed to be poored until county approval. X ,24 Commissioning shall be required of the lighting controls in accordance with WSEC 1513.8 in order to verify that the lighting controls have been installed, and function properly, in accordance with operational requirements. A commissioning compliance checklist completed in accordance with WSEC 1416.4, shall be submitted to the Mason County Building Department upon substantial completion of the building. See Compliance Checklist in the 2009 WSEC, Figure 14B. X COM2012-00015 Page 5 of 7 I 215) WSEC COMPLIANCE: This project is approved for compliance to the 2009 Washington State Energy Code, components shall be installed as follows: ENVELOPE: Compoenent performance 588/593 Roof deck minimum R-30, Wall insulation R-21 or better, slab insulation R-10 or better installed in around the entire perimeter from the top of the slab edge to the bottom of the footing and in for a total of 24-inches, and 3-inch concrete perimeter wall R-0. All windows, including store door(s) shall have a U-Factor of.35 or less and one exterior door, foam core insulated, shall have a u-factor of.16 or less. Windows shall be manufactured by an approved small business using LOW-E wih .07 emissivity, argon and aluminum thermal break.(See WSEC Table 10-6(B)) LIGHTING: Total interior lighting shall not exceed 3400 watts. Wattage shall be calculated using the lamp and ballast or for incandescent fixtures, using the maximum labeled wattage allowed and is approved as follows: 23-2-lamp 32 w fixtures, 21-32 watt can lights, 8-8 watt LED sconces, and 8- max. 66 watt hanging lights. Lights used to warm food or specifically for food preparation and not used in addition to general area lighting is exempt. All daylight zones shown on on plan sheet LS shall be provided separate control in accordance with WSEC 1513. Zone 1, the primart daylight zone shall be controlled separately from the secondary daylight zone. Automatic daylight sensing controls shall be capable of reducing light out put, control luminaires only within the daylighted area, and time delay circuits in accordance with 1513.3. Controls shall be equipped with separate automatic controls to shut off the lighting in all spaces during unoccupied hours. Occupancy sensors shall be capable of automatically turning off all lights in an area, no more than 30-minutes after the area is vacated, or as specified in accordance with WSEC 1513.6. Exterior lighting shall not exceed 500 watts as proposed in the WSEC lighting summary. All exterior lighting shall have automatic controls capable of turning off exterior lighting when sufficient daylight is available or when lighting is not required during night time hours. Lighting not designated for dusk-to-dawn operation shall be controlled by either: a)A combination photo-sensor and a time switch; or b)An astronomical time switch. MECHANICAL SYSTEM: The mechanical system is approved as proposed shown on Sheet M1.0 and the WSEC mechanical summary. This permit becomes null and void if work or construction authorized is not commenced within 180 days, or if construction or work is suspended for a period of 180 days at any time after work is commenced. Evidence of continuation of work is a progress inspection within the 180 day period. Final inspection must be approved before building can be occupied. Sibrrg ure Printed Name irbwner O Agent Date PURPOSE OF SPECIAL INSPECTION Special inspection is the monitoring of materials and workmanship which are critical to the integrity of the building structure to assure that the approved plans and specifications are being followed and that relevant codes are being observed. Special inspections are in addition to those conducted by the county building inspector and by the engineer or architect of record. Special inspectors furnish inspection at all times that their presence is required by the code, the plans and specifications, and the enforcing jurisdiction. Good communication between the special inspector and the designer,contractor, and building department is essential. DUTIES AND RESPONSIBILITIES OF THE SPECIAL INSPECTOR: Special inspectors are individuals with highly developed, specialized skills who observe those critical building or structural features which they are qualified to inspect. Duties of special inspectors and/or inspection agencies include the following: 1.General Requirements Special inspectors shall review approved plans and specifications for special inspection requirements. Special inspectors shall comply with the special inspection requirements of the enforcing jurisdiction. 2.Signify Presence at Job Site Special inspectors should notify contractor personnel and the enforcing jurisdiction of their presence and responsibilities at the job site. 3.Observe All Work for Which They Are Responsible Special inspectors shall inspect all work and perform or observe all tests requiring inspection and testing for conformance with the building-department-approved (stamped)drawings, specifications and applicable provisions of the code. • 4. Notification of Discrepancies Special Inspectors shall bring all nonconforming items to the immediate attention of the contractor. If any such item is not resolved in a timely manner or is about to be incorporated in the work,the engineer or architect of record and the building official should be notified immediately by telephone or in person,and the item noted in the special inspector's written report. The special inspector shall write a separate report to be posted at the job site regarding noted discrepancies which should contain,as a minimum, the following information about each nonconforming item, •Description and exact location, •Reference to applicable detail of approved plans/specifications, •Name and title of each individual notified and method of notification,and •Resolution or corrective action. S. Reports A. Inspection reports The special inspector should complete written inspection reports for each inspection visit and provide the reports on a timely basis determined by the building official. The special inspector or inspection agency shall furnish these reports directly to the building official,engineer or architect of record,and others as designated. These reports should be organized on a daily format and may be submitted weekly at the option of the building official. In these reports,special inspectors should: •Describe inspections and tests made with applicable locations; • List all nonconforming items; • Indicate how nonconforming items were resolved; • List unresolved items, parties notified, and time and method of notification; and • Itemize changes authorized by the architectlengineer of record and approved by the building department, if not included in the nonconformance items. B.Final reports Special inspectors or inspection agencies shall submit a final, signed report to the building department stating that all items requiring special inspection and testing were fulfilled and reported and,to the best of their knowledge, in conformance with the approved design drawings, specifications, approved change orders and the applicable provisions of the code. Items not in conformance, unresolved items or any discrepancies in inspection coverage(i.e., missed inspections, periodic inspections when continuous was required,etc.)should be specifically itemized in this report. Otherwise a component performance calculation may be required: A) Prescriptively, R-10 slab insulation around the entire perimeter shall be installed with a thermal break. Note that sections and details depict R10 slab insulation but does not extend up to slab edge in detail 10/A4.1 B) Minimum R-21 wall insulation shall be required where the concrete stem wall extends above the slab. The permit number for this case is COM2012-00015. During the plan review the plan reviewer, Larry Waters added conditions to the permit. One of the conditions addressed Fire Marshal comments . The condition is pasted below. A NFPA 13 fire sprinkler is not required because of the size of the building. A NFPA72 monitored fire alarm system is required if the existing fire hydrant does not meet minimum fire flow requirements. Larry Waters, who represents Fire Marshal activities in Mason County, has not heard whether the existing fire hydrant meets required fire flow. I recommend that you contact the Fire District representative Jeromy Hicks to arrange for a fire flow test. This message has been cc'd to Mr. Hicks. In addition please provide a copy of the test report to our office so that we can add it to our file. A knox box is required to be installed per section 506 of the 2009 International fire code. Please contact the local fire district for more information and inspections. X Install 2A10BC fire extinguishers throughout the building per chapter 9 of the 2009 International Fire code, mounted no more than 60 inches above the floor to the top of the unit with a maximum travel distance of 75 feet in any direction. The foundation ground improvement will utilize quarry spalls or recycled concrete 1.5 feet below anticipated bottom of foundation elevation. the particle size of the spalls or concrete should be about 4 to 6 inch and washed of fines. After placement of the spalls, the zone between the spalls and proposed bottom of footing level can be backfilled with a 2,500 psi non reinforced concrete. The purpose of the concrete is to minimize ground water seepage along the bottom of the foundation. The layer of concrete between the foundation and spalls will have a minimum thickness of about 1.5 feet. NLO has provided an illustration of the foundation subgrade improvement, Section A-A, on Page 2 of 2 of this DFR. NLO recommended overexcavation of this material down to the underlying stiff soil condition and the subsequent excavation could then be brought back up to the desired bottom of proposed foundation elevation with a 1.5 ft layer of concrete underlain by either recycled concrete or quarry spalls. The foundation ground improvement will utilize asphalt spalls with a particle size of about 2 to 4 inches washed of fines to backfill the subsequent excavation up to about 1.5 feet of bottom of footing level. The remaining area between the bottom of foundation and the spalls will be brought up to construction elevation with a 2,500 psi non reinforced concrete. The layer of concrete will have a minimum thickness of about 1.5 feet. NLO has provided an illustration of the foundation subgrade improvement, Section A-A, on Page 2 of 2 on DFR#3.
