An Environmental Impact Statement (EIS) is not required under RCW 43 .21C. 030 (2) (c) . The geo-tech assessment is required due to the steep slopes that exist on the site. The geo-tech assessment will need to be submitted and reviewed by Mason County Public Works prior to the Forest Practice Application (FPA) being approved. The geo-tech assessment can be done by either a licensed engineer in the State of Washington or a geologist with expertise in land slide hazards. 2) MITIGATED CONDITION #2: A Stormwater Site Plan (SSP) will be created and implemented prior to development, clearing, grading, or construction of any portion of the proposed segregation. X----------------------- 3) MITIGATED CONDITION #3: Emergency vehicle apparatus access roads shall conform with the requirements of the Uniform Fire Code and Mason County Title 16. If the access road cannot conform, special conditions will be placed on any building permit issued on property that is served by a non-conforming access road. Contact the Mason County Fire Marshal for details. 4) INFORMATIONAL ONLY: The applicant/agent will create a pump out plan. This plan will set up the tank(s) of the septic system(s) will be pumped and inspected every 3 years. The report generated from the pump out and inspection will be submitted to and acted upon by Mason County Heath Services, Environmental Health Department. If the segregation becomes part of an established district or is hooked up to a Wastewater Treatment Facility, the above statement will be voided. X 5) INFORMATIONAL ONLY: Sewage disposal facilities, as well as water supply facilities, should be provided in accordance with appropriate state and local health regulations. Storm drainage facilities should be separated, not combined with sewage disposal systems. X 6) INFORMATIONAL ONLY: Storm drainage facilities shall be separate from sewage disposal transport facilities and include provisions to prevent uncontrolled and untreated direct entry of surface water runoff into receiving waters.
