As requested by the Mason County Department of General Services,this letter serves as our final summary
of special inspection services provided by our firm on the above referenced project. A representative from
our firm provided material testing services for the cast-in-place, reinforced concrete foundation for the
monopole tower. Based upon our observations and testing, it is our opinion that the cast-In-place,
reinforced concrete foundation work was completed in conformance with Mason County approved plans.
Our field report No. 1 and compression test report are enclosed for your records.
We trust this letter meets your current needs. Please do not hesitate to contact us if you have questions
or comments regarding the contents of this letter.
