5) The following special conditions should be made a part of the building}permit:

1. Electrical Permit Required. The electrical system shall be installed under a separate electrical permit issued by the Washington State
D rtme Labor& Industries(L&1). Electrical"final"inspections must be approved by L&I prior to requesting a building final.

2. Special Inspection: IBC Chapter 17, Section 1704. In addition to the iinspections required�y IBC Section 109, the owner or the engineer or
architect of record acting as the owner's agent shall employ one or more special inspectors &ho shall provide inspections during construction on
the types of work listed under iBC Section 1705. The special Inspectors'duties &responsibi ities shall be as specified in 1704. All special inspection
reports shall be provided to the Mason County Building Department in accordance with IBC 1704.2.4. Prior to final inspection, the special
inspection agency shall also provide a final, signed report to the building department stating that all Items requiring special inspection and testing
were fulfiAe and reported,corrections of any discrepancies has been completed,and,to th best of their knowledge, are in conformance with the
appro desi rawings,specifications, and applicable provisions of the code.

3. Speci Inspection Contractor Responsibilities.Third-Party Special Inspection is required n accordance with IBC Sections 1704& 1705 for the
items identified in the Statement of Special Inspections(as required by'1IBC Section 1704,3) In accordance with IBC Section 17 04.4, each
contractor responsible for the construction of the work listed in the stat#-meat of special Inspections shall submit a written statement of responsibility
to the building official and the owner prior to the commencement of work on the system or component. The contractor's statement of responsibility
sha ntain nowledgment of awareness of the special requirements contained in the st tement of special inspections.

4.All Construction:Ali construction shall meet or exceed all local ordinances and the requir ments of the 2012' International Codes as adapted by
Mason County and Washington State, Occupancy is limited to the approved and permitted lassification.Any non-approved change of use or
occ cpuId result in permit revocation and/or Code Enforcement action.
5. Field Correct: The construction of the permitted project is subject to inspections by Mason County Building Department.All construction must be
in conformance with the 2012 International Codes as adopted by Mason County and Washington State.Any corrections, changes or alterations
requir a lriuilding or fire Inspector shall be made prior to requesting additional inspectio s.

6, Final Inspection Required:All building permits shall have a final inspection performed and approved by the Mason County Building Department
prior to permit expiration. The failure to request a final inspection or failure to obtain final approval prior to expiration will be documented in the legal
property records on file with Mason County as being non-compliant with the city's ordinances and building regulations and may be referred to the
City AjGarrr6 tax action,
7, Permit Validity: IBC 105.4/IRC R105.4:The issuance or granting of as permit shall not be nstrued to be a permit for, or an approval of, any
violation of any of the provisions of the building code, or any other code or ordinance of Mas n County. Permits presuming to give authority to
violate or cancel the provisions of the building code or other ordinances of the jurisdiction shall not be valid. The issuance of a permit based on
construction documents and other data shall not prevent the building official from requiring correction of errors in the construction documents and
other data.The building official is also authorized to prevent occupancy or use of a structureiwhere in violation of this code or of any other
ordin s o ' urisdiction. i

8. Pemmi xplration: All permits expire 180 days after permit issuance,',or 180 days after thellast inspection activity is performed. The Building
Official may extend the time for action for a period riot exceeding 180 days, upon the receiptjof a written extension request indicating that

9. Engineering or Architecture:Any changes in proposed construction 61hali be reviewed byte engineer or architect of record and submitted in
writing to the Mason County Building Department prior to construction. All engineering and/c r architectural documents are a part of the approved
set of plans and shall remain attached thereto. If documents are removed,or changes are n iade without approval from the architect or engineer
and the Building Official, approval will not be granted,and a re-inspection fee may be charged by Mason County prior to any further Inspections

10,Sales Taxes: This work is being done In Mason County.All sales tax for materials deliverd to, or work performed in Mason County shalt be

7) Contractor registration laws are governed under RCW 18.27 and enforced by the WA State ept Labor and Industries, Contractor Compliance

11) Provisions or surface/subsurface drainage control must be implemented with new construction or development on site and MUST NOT adversely
impact adjacent parcels. Under the requirements of Mason County Stormwater Ordinance, ether private ditches and drains will meet requirements
of the stormwater ordinance or prior approval will be granted to use an.existing utility and drainage easement dedicated for that specific purpose.
For further Information regarding this ordinance and the REQUIREMENT to obtain an ACCESS PERMIT for the installation/construction of a
driveway or access connecting from a Mason County Road, Contact the Mason County Pubic Works Department prior to construction at Ext 450.
For any construction which is proposed to be located within 25'of a Mason County road right of way,it is suggested to contact that office to review
future planned k pvtch may affect your project.

Dear Mr.Ewing,
Per your request, we have prepared this structural observation letter for new tower attachments at the
above mentioned project. We performed a site visit on August 13, 2018 to visually observe the six (6)
new Verizon Wireless antennas and the six (6) new Verizon Wireless RRUS+A2. For our use in
preparing this letter, we were provided with the approved construction drawings prepared by Camp+
Associates,dated May 24,2017.
Based on our site visit, it is our opinion that the six (6) new Verizon Wireless antennas and the six (6)
new Verizon Wireless RRUS+A2 were installed per the approved plans.
If you have any questions, or if we can be of further assistance, please do not hesitate to contact our

I am on site at 24765 NE State Route 3 Belfair WA. as dispatched for new antenna installation inspection. I reviewed
he on site and approved plans by C Camp+dated 5/24/17.
I walked the site for visual observations and noted the new VZW antennas are in place two per sector,and two new
US+A2 are in place. The Antenna's and RRU units are attached to the new mounting brackets with new hardware. I
btained pictures of the new antennas and RRU units.
The antennas and RRU units appear to be installed in accordance with the on site and approved plans. See the
attached pictures.
