Note that all sites having crushing, classifying, or stockpiling activities associated with mining or processing operations must obtain Sand& Gravel General Permit Coverage from the Department of Ecology prior to operating. Refer to Special Condition SL A. on Pages 5-7, and Appendix B on page 44 of the Sand and Gravel General Permit. The application form for Sand&Gravel General Permit coverage is available online at the following Ecology website: http://www.ecy.wa.izov/programs/w(i/sand/index.html For your convenience, I also have enclosed a copy of the Permit Application Form, including instructions.
